22.01.2022 How good is Mirena IUD for Adenomyosis? Mirena is actually a quite well researched treatment for adenomyosis. One study has shown Mirena could be as effective as hysterectomy for controlling heavy periods. A 3-year study has shown Mirena is also effective for period pain. However, satisfaction rate was only 56.3% due to side effects, such as prolonged light bleeding (25%), irregular bleeding (14%), and weight gain, ovarian cyst, lower abdominal pain and acne. Mirena is less effective if the uterus is larger than 150ml. Learn more about adenomyosis and its treatment, visit https://www.sydneyfibroidclinic.com.au//adenomyosis-treat/

19.01.2022 Does ablation reduce heavy bleeding? Will ablation worsen period pain? Should ablation be used to treat Adenomyosis?! Endometrial ablation uses heat energy to destroy the inner lining of the uterus, so bleeding will be reduced. However, glands in the muscle (adeno= glands; myosis in the muscle) will be trapped. Some has described this as sealing off the juice on the BBQ hot plate. Bloating and period pain can get worse. Even with modern technology, adenomyosis deeper than 2.5mm cannot be treated by ablation, according to a 2015 study. (BTW if MRI is used for diagnosis, the depth is 12mm minimal!). There is higher rate of subsequent hysterectomy if ablation is used to treat adenomyosis.

15.01.2022 How is Adenomyosis Diagnosed? The symptoms of adenomyosis are typically heavy menstrual bleeding and period pain. The uterus maybe enlarged and tender on inter...nal examination. These clinical features however are not specific for adenomyosis. Therefore before advent of high quality ultrasound and MRI, diagnosis was only made after hysterectomy. To-date, transvaginal ultrasound can suggest the diagnosis. However signs on ultrasound can be subtle and easily missed. Focal adenomyosis can be mistaken as fibroid. MRI is more accurate than ultrasound, especially when fibroids are also present. There is no specific blood test for adenomyosis. Adenomyosis coexists with fibroids in about 50% of cases. Adenomyosis coexists with endometriosis in 80-90% of cases. Adenomyosis can be present in 40-70% of hysterectomy specimen. In other words, adenomyosis is far more common than clinically recognised. In women with severe heavy menstrual bleeding and period pain, adenomyosis is highly likely to be present. If in doubt, MRI should be performed. 13.01.2022 How effective is Progesterone-releasing IUD (Mirena) for adenomyosis? . . Mirena is an IUD that slowly releases the sex hormone Progesterone over 5 years. A Tu...rkish study showed Mirena could be as effective as hysterectomy for controlling heavy menstrual bleeding. A 3-year follow-up study from China showed Mirena could be effective for pain control. However satisfaction rate was only 56.3% at 12 months. The side effects were prolonged, light bleeding (25%), irregular bleeding (14%), and weight gain, ovarian cyst, lower abdominal pain and acne. Mirena is less effective in women with extensive adenomyosis when the uterus is larger than 150ml in volume. Discontinuation rate of 70% is noted in women with a uterine volume larger than 314ml. 13.01.2022 Adenomyosis Treatment 101 Pain killers treat symptoms only. Hormone treatment suppresses adeno. Ablation seal off deep adeno and can make pain worse. Embolisation (UAE) kills off adeno but normal uterus remains alive.... Hysterectomy removes the organ and adeno (but might not be the cure for endo). 11.01.2022 Dear All, Sydney Fibroid Clinic is a boutique private specialist clinic. We see a very small number of patients each session and all consultations are by appointment. All our patients are otherwise well, other than their menstrual / fibroid issues. All our staff have not travelled out of Australia in the last 14 days and have been instructed to stop coming to work if they have any cold or flu like symptoms. Therefore, our clinic environment is safe, and we are still consultin...g in our rooms. All our hospital-based clinics are within private hospitals that mainly deal with elective surgeries and therefore NOT expected to be dealing with CoVid 19. However, if you are unwilling or uncomfortable to come near any hospitals, our St Leonard Square Rooms is in a brand-new commercial building that is less than 20% occupied. Therefore our St Leonards Square rooms possess the lowest risk of any potential exposure to CoVid 19. However, if you have travelled outside Australia in the last 14 days or have any cold or flu-like symptoms, please DO NOT come to our clinic, but ring and reschedule your appointment or change your appointment to a telephone consult with Dr Liang. Take care, Kind Regards, Dr Eisen Liang & Anna Office Manager / PA Sydney Interventional Radiology / Sydney Fibroid Clinic
11.01.2022 Adenomyosis and Fertility Sadly not only adenomyosis can cause debilitating symptoms, it also has negative impact on fertility and pregnancy. Women with adenom...yosis are more likely to have premature birth. Adenomyosis can also have a detrimental effect on IVF outcomes. Unlike fibroids, adenomyosis is an infiltrative process and cannot be surgically removed. UAE (uterine artery embolisation) is a uterine preserving non-surgical procedure that is highly effective in controlling symptoms. There is however not enough data to support its use in women still seeking fertility. Study has shown that GnRH before IVF improved the pregnancy rate. GnRH treatment could be an option for women seeking symptom control and immediate fertility. 11.01.2022 In this down time it is nice to share a good news story : In November 2015, after suffering with a disturbingly heavy, painful and highly irregular cycle for a few years I had the Mirena IUD put in. That largely solved the problem for about a year and a half before the problems returned. Following an MRI, it was discovered that I had rather severe adenomyosis of the entire uterus and a UAE (uterine artery embolisation) was offered to me.... Its a very simple procedure that, along with removing an overwhelmed Mirena, has from the first period after the procedure resulted in regular and much shorter periods (2 3 days), very light flow, next to no pain at all during any stage of my cycle and a confidence that I will no longer make a mess, get caught out or suffer with the excruciating pain of the past. I had a follow-up MRI to discover my adenomyosis is completely gone! My cycle has remained this way for well over a year now and it has seriously improved my life in a way I could never have dreamed. I will be forever grateful for this treatment. N.C #adenomyosis #sydneyfibroidclinic #women #womenshealth #uterus #alternativetohysterectomy #health #radiology #painfulperiods #Mirena
