25.01.2022 Case #8 - Separated File - Fills the whole canal Here is an interesting case I would like to share. The referring dentist had an unfortunate event with an endodontic file failure. The separated file section was long - almost 16mm long - filling the entire root canal space and slightly protruded into the pulp chamber of the lateral incisor. Fracture of file in the larger diameter section of the working end is usually not torsional fracture, but a sign of fatigue related fract...ure. A torsional fracture, that occurs due to file size:canal diameter discrepancy during rotation, the separated (at thin region of) file is wedged tightly in the narrow / curved root canal. Whereas a fatigue-related file fracture occurs due to multiple uses of endodontic files in (various degrees of) curved root canals and the separated (at thick region of) file is mostly sitting loose in the canal space. Files separated with the fatigue resistance failure can be removed using thin ultrasonic endodontic tips vibrating around the separated end. - Minor dentine removal around the separated file-end (with thin endodontic ultra-sonic tip) may be required to create space to further vibrate separated file, however excessive dentine removal (using normal periodontal scaler tip) must be avoided. - We need to keep in mind the remaining dentine thickness / furcation zone in the cervical part of the tooth. - Like searching / negotiating MB2 canal, make sure not to PANIC and loose PATIENCE during this minor (mostly reversible) complication. - Trying to remove separated file QUICKLY will results in EXCESSIVE dentine structure loss (making tooth non-restorable or increasing risk of root fracture over short-to-medium term) and / or results in FURTHER FRACTURE of the same file within the canal. The file in the picture was removed slowly with medium intensity ultrasonic vibration over 10-12min.

22.01.2022 Case #7 - Furcation bone loss Primary Endodontic - Secondary Periodontal disease The tooth had an interesting furcation bone loss. Check out the radiograph and CBCT image that show intact buccal and lingual cortical plates but obvious periapical and furcation bone loss. No periodontal pocket associated with the furcation bone loss, pulpal necrosis & PA lesion led to conclude endodontic origin of the furcation pathology. The floor of the chamber was viewed under high magnifi...cation of the operating microscope but no furcation canal openings were visible. The root canal system was thoroughly disinfected and medicated with Ca-hydroxide paste for 10 weeks. The progressive hard-tissue healing was noted and eventually the canals were filled.

07.01.2022 Case #6: Dens-in-Dente Interesting case of unusual dental anatomy - Dens Invaginatus or Dens-in-Dente. This case depicts the importance of active irrigation for effective disinfection and warm vertical compaction for satisfactory root canal filling.

01.01.2022 Dear Doctors, ADA-level 3 restriction are downgraded to ADA-level-2 guidlines starting from Monday 27th April 2020. This is a good news for patients as well as our dental community as we can now perform some additional procedures that were previously restricted under ADA-level-3 guidelines. ADA-Level-2 guidelines provide green signal to perform all urgent and emergency root canal treatments. However, surgical interventions as well as elective treatments are not allowed. ... Safety of patients and my team is extremely important. The practice dynamic has been specifically amended to accomodate the social distancing and grouping measures. Further, all the necessary PPEs and other recommended precuations are enforced to minimise the risk of cross-infection.
