23.01.2022 Plantar Fasciitis What is it? Plantar fasciitis is the inflammation of the Plantar Fascia, a thick fibrous band of strong connective tissue that runs along the sole of the foot, attaching from the heel to the toes, and creating the arch of the foot. ... What are the symptoms? Pain under the foot or heel, often felt in the morning with the first few steps, after prolonged standing/sitting or rigorous activity such as running or jumping. The pain can be sharp, like a knife, or produce a burning sensation felt in the arch of the foot. How is it diagnosed? Your Doctor or Physio will evaluate your pain, health history & symptoms to diagnose the condition. Certain populations are more prone to the condition which helps the diagnosis. Plantar fasciitis is more common in certain sports such as running and dancing, people with flat feet or a high arch, people working a job requiring long periods of standing, older populations, overweight or pregnant people. What treatment can be done? Soft tissue release through the calves, shins and feet, often with the use of myofascial cupping to target the Plantar Fascia. Gentle stretching done with the help of the Therapist or by the client at home and rolling a spiky ball under the arch of the foot can also help. Physiotherapy exercises help to strengthen the intrinsic muscles of the ankles and feet to help with support and load distribution and referral to a Podiatrist who can assist with Orthotics or improved footwear.

10.01.2022 STRETCHING v STRENGTHENING One of the more complicated issues for any practitioner working in a musculoskeletal field (ie. physio, chiro, massage therapist, exercise physiologist) involves determining if a client/patient will receive more benefit from stretching or strengthening a muscle that is showing pathology. Practitioners are fundamentally taught to stretch a muscle which is TIGHT and strengthen a muscle which is WEAK. A straight-forward example if you are a runner exp...eriencing knee pain and are found to have tight quadriceps which pull on the patella and alter it’s tracking then you will need to have the muscle released with massage and do some stretching at home. Sometimes this can alleviate the issue and sometimes it won’t totally fix it, which is when the other side of the equation comes in; the strengthening. Could it be that the hamstrings are weak and the quadriceps are tight because as they are overworking in the gait cycle? Could it be that part of the quadriceps (which is made up of 4 muscles) are overworking for another part which is under working. This is where the practioner needs to determine which is tight and what is weak and act accordingly. To further complicate matters, muscles can be both tight AND weak and therefore need both strengthening AND stretching. The first muscle group which comes to mind is the glutes, which can often get tight from prolonged sitting, creating trigger points and adhesions, and also can become weak and under-active in this common scenario.

04.01.2022 Piriformis Syndrome What is it? The piriformis muscle is located deep in the buttock region beneath the gluteus maximus and is responsible for primarily moving the hip into external rotation, and assisting in abduction and extension. When this muscle is injured through trauma or becomes chronically tight it can cause buttock of hip pain and spasm and can also pinch on the sciatic nerve which runs through the area. ... In some people the sciatic nerve actually travels between the two muscle bellies of the piriformis muscle, so when tight the muscle compresses directly on the nerve. Women are also 6 times more likely thean men to experience this condition. What are the symptoms? Symptoms displayed are buttock pain, stiffness, tenderness, inflammation, numbness or tingling locally or referred down the buttock, thigh, knee and lower leg, all of which cause difficulty sitting for prolonged periods. Although presenting similar nerve-type symptoms to Sciatica, Piriformis Syndrome is distinctly different as Sciatica is related to the Lumbar (lower spine) nerve roots and isn’t caused by the Piriformis muscle. How is it diagnosed? Questions on medical history, symptoms, exercise, lifestyle habits followed by physical examination involving range of motion, muscle strength, muscle weakness and nerve conduction tests. What is the treatment? Muscle release through deep tissue and trigger point techniques, focusing on releasing the piriformis, glutes, lower back etc Hip stretches to do at home Muscle strengthening exercises for related muscles
