22.01.2022 10 Days New It always is such a precious time the days and weeks after birth. This was my first postnatal visit yesterday after being a part of the journey of pregnancy, labour & birth with Sarah & her family. As her Doula it is part of my care to allow mumma to debrief her birthing experience and check how she is handling her transition to motherhood of now being a mumma of three. I also do a lil check on little Orlandos progress on feeding, sleeping and growth. And of co...urse now little Olly is doing amazingly well and is healthy I got to fit in a little cuddle . Why is debriefing important and why do I do it??? Birth debriefing provides the birthing woman with the opportunity to talk about her birthing experience. Even for mummas who have been through birth before will know with every birth comes a new experience. Allowing mum to ask questions about what happened, alleviates any confusion, or trauma or fear and help her to gain much more of an understanding. Mumma may have experienced something unexpected, or the birth wasnt what she expected it to be like. Having unanswered questions can lead to unresolved, negative feelings about birth and increases distress. It can have long term consequences for the woman and her family. A woman who isnt able to express or talk about her birth experience is left with intense feelings of guilt, shame and failure and creates more trauma and fear in not being able to trust her body to birth again in the future. Having the opportunity to understand what happened with her care providers and who was present during her labor and birth can help answer the very important question, Why?. 15.01.2022 Breastfeeding & Hormones Oxytocin is a powerful hormone and is responsible for breast milk "letting down," or moving to the front of the breasts. Oxytocin is the same chemical released in the brain when a person falls in love! When baby is suckling at the breast,they trigger tiny nerves in the nipple.These nerves cause hormones to be released into your bloodstream. Another hormone called prolactin acts on the milk-making tissues. Its then that the oxytocin causes the bre...ast to push out or let down the milk. The let-down reflex makes the milk in your breasts available to your baby. Cells around the alveoli contract and squeeze out the milk, pushing it down the ducts towards the nipple. Oxytocin also makes the milk ducts widen, making it easier for the milk to flow down them. When labor progresses & becomes more active, mummas tend to lose their appetite, so having something easy and quick on hand is perfect. Eating and drinking regularly is very important during labor to help maintain blood sugar levels and energy. Some hospitals still have a policy of no eating during labour in case you have ...to go to surgery. These policies are outdated. Whether you're actually using the toilet emptying your bowels to help make more room for baby to move down, or just sitting on it, the position is known to be helpful in active labor and transition. Its the best way to release and relax a pelvic floor and help a baby descend into the pelvis. When we allow these muscles to soften, all of the hard work our uterus is doing pays off by allowing our ce...rvix to thin, dilate, and get us closer to meeting our baby. However, I advise against staying on the toilet for a long period of time. The hard toilet seat can cause some swelling, pins and needles and cramps in your legs and feet. 07.01.2022 Labor Lunges Lunging in labor looks different than it does during exercise.The lunge is a technique to rock open the midpelvis between the ischial spines it allows more room between the ischial spines, or the narrowest part of the interior pelvic passage. This makes the most narrow part of the pelvis plus the outlet open as the laboring woman leans towards her foot. This can also help baby move down & therefore progresses labor & can assist on getting baby into a better posi...tion for birth. It is best to be used in labor during a contraction for...Helping an occiput posterior baby rotate or descend, Helping an asynclitic baby descend, Overcoming a stall in active labor. Before performing a lunge be sure that she is in active labor first. Once she is getting regular contractions, she will be able to move into the lunge position as the contraction begins and put her foot down after the contraction finishes to relax her leg & prevent tired legs. This simple technique can be used by a laboring woman if she can stand up and put one foot on a stool or a chair, whilst she is facing forward. The chair (or stool) is to her side, not in front. She can either use the back of the chair, the end of the bed or a support person to hold onto her for stability. Using a support person make sure they stand in front of her giving her eye contact it gives her a sense of comfort, stability, security and strength. Be sure her foot is flat on the chair and that the chair is stable. During a contraction, she rocks towards her knee and back to standing straight up again. Repeat through contraction. Make sure she is breathing deeply and slowly when possible. Be sure she is relaxing the belly and the buttocks, the shoulders and neck. It is important that she does not extend her knee past her foot. She could strain her knee ligaments if she does. Just rocking gently towards the knee and away. It doesnt work if the mothers knee is directly in front of herself. Between contractions, she can put her foot down and rest her knee and move her legs for circulation. From the beginning we just clicked. Tamika spent 24 gruelling hours labouring with us, she is equipped with endless enthusiasm and techniques to support you throughout your labour. Her strength and encouragement was astounding. All the little touches prenatal and postnatal positively prepared me and has allowed me to recover with ease. Words could not express how incredibly proud I am of Sarah (Mumma) who despite having high hopes at achieving a vba2c her body & little Olly had other plans. Sarah had a real emergency cesarean but that meant that little Olly was born safely as the universe intended. It really is amazing how powerful a mothers intuition is, that even being fully dilated, pre...pped & prepared to attempt a forceps delivery, & so close to achieving her vba2c, Sarah knew baby Olly needed out ASAP & that call is what saved her babies life. You are amazing Sarah & should be incredibly proud of yourself because you fought against all odds through this whole journey. 01.01.2022 Doula Network Australia COVID-19 Statement Doula Network Australia Inc. (DNA) is the national association for birth and postnatal doulas in Australia. We oppose the exclusion and restriction of doulas from healthcare services during infectious disease outbreaks, including that of COVID-19. Doulas are professionals who provide essential support services to women during pregnancy, birth and postpartum. Removing womens doulas at a time when they are exceptionally vulnerable is ...dismissive of their needs and desires and violates womens and babies human rights to life, health, privacy and equal treatment (doula support decreases morbidity and improves outcomes for mothers and babies). Current CDC policy notes that individuals essential for a patients emotional well-being and care should be exempt from exclusion/restriction situations. Furthermore, ACOG and AWHONN recognise doulas as essential support people and, along with RCOG, oppose restrictions on doulas being present during labour. Throughout previous pandemics (SARS and H1N1) doulas were not restricted from providing labour support. RANZCOG notes that COVID-19 seems to hold less risks for pregnant women than influenza, and preliminary research suggests that children are less susceptible to severe effects of the disease. DNA has discussed with and supports our members to follow WHO safe practices to minimise the spread of disease including vigilant hand washing/sanitising, wearing PPE as necessary, practicing social distancing and self-isolating when unwell or after contact with carriers of disease. Doulas respect, protect and promote the health and well-being of their clients. While we recognise the risks posed by the current health crisis, doulas also provide support prior to attendance at hospital, thus negating any effect that exclusion from hospitals would provide. Indeed, by supporting women at home until they require assistance, doulas help reduce the load on the healthcare system and the potential for COVID-19 transmission. DNA encourages healthcare services who have implemented exclusion policies to revise these and welcomes the opportunity to work collaboratively and flexibly with hospital maternity services for the benefit of birthing women and their families. https://www.doulanetwork.org/covid-19-statement
