25.01.2022 Chatting to a friend recently and we were talking about what being a dietitian meant to me. It sometimes sounds too crude to say I believe in science-backed advice or evidence-based approaches. It is an oversimplification. I DO believe in the scientific method- that we can use known methods of systematic observation, measurement, and experiment, and the formulation, testing, and modification of hypotheses to expand our knowledge about nutrition. (and still accept that some...times we get it wrong). But also, alongside that, I believe in bringing the lived human experience into consultations with clients. Because, however much I know about medical nutrition therapy, the client is the expert at living in their bodies. And science should NEVER be used to undermine the value of the lived experience. I have experienced this as both client and practitioner. Sitting in the client chair, I have learnt much from clinicians in a wide range of disciplines, and outcomes are always more successful when the clinician listens to my lived experience. And as an Accredited Practising Dietitian In my practice this is the way I work with you to help address food and body image issues, disordered eating, gastrointestinal and womens health problems. 24.01.2022 Do you notice physical reactions when uncomfortable situations arise? Physical signs - heart rate, gut symptoms, fatigue that can arise when You are faced with unfamiliar, uncommon, confusing and confronting situations. I do. Thankfully, learning to acknowledge these signs is something I have become fairly practiced at, to notice what they are telling me, to gently bring my body back to a grounded position from where I can act with intention and clarity. For me, I find thi...s approach far more helpful and compassionate, then an approach that sees physical symptoms as undesirable or problematic. I had to ask someone to take some photos for my website. Doesnt sound like a big task to organise, but it took me ages to act. Okay, months. All these questions were whirling around in my head. What look/mood/feel/pose did I want to portray? What message did I want to convey? Can people even interpret pictures as saying something about what kind of person I am, how I work, or whether I can help them? What should I wear? Does that even matter? Am I being superficial even thinking about that? Finally, I asked someone I trust, and she recommended @cath_drawingaday. We met for coffee, and got to know a little about one another. The day of the photoshoot arrived and I was a bundle of nerves. We were just coming into a change in weather pattern after a solid January of hot, steamy conditions. The day before the shoot I was dripping sweat standing in the shade. The morning of the shoot I new that rain was predicted. Was rain going to ruin the shoot? (The rain came, but it didnt start until we had finished). Lucky for me I was able to find a friend to come with me who would be supportive, respectful and fun., someone who would help me get into the spirit of the afternoon in a playful way. Who would exude calm energy. The outcome? The results (I think) are lovely. The website launch is coming next month. 23.01.2022 Yes, I'm late, but as they say, better late .... I've been thinking a fair bit about Irritable Bowel Syndrome these past months, not just because it is the spotlight for the month of April . Its a strange kind of diagnosis, when so much of medicine provides such an incredible amount of specificity and detail (often down to the cellular level), where diagnostic tools are really sophisticated, IBS is handed out as a diagnosis when other things have been eliminated . You h...aven't got bowel cancer, you haven't got inflammatory bowel disease or coeliac disease or gluten intolerance, or a gut infection, (that's all great news) BUT you still experience persistent abdominal pain and cramping gas and bloating diarrhoea or constipation..... ...and often to an extent that interferes with daily living. Bowel disorders without underlying disease process (that can be identified) is lumped in together as IBS or functional gut disorder. Let's call it an anti-diagnosis. But there is some hope at the end of the tunnel. There is an increasing amount of talk about a number of reasons that individuals can develop debilitating unexplained gut symptoms. These include Small Intestine Bacterial Overgrowth (SIBO) Small Intestine Fungal Overgrowth (SIFO) Dysbiosis Pathogens Nervous system dysregulation So a diagnosis of IBS is really a signal that the detective work is only beginning! The best thing you can do is start by finding a clinician that is interested in working with you. 23.01.2022 OK, so I might do the long 'no' to myself, while I continue to act professionally (of course), but you get the idea. There is plenty of confusion around this one, and it's not just people with IBS that get the wrong information. I have found myself explaining to some doctors why following the Low FODMAP diet long term is not a good idea. While this diet is sometimes used in the management of people with Irritable Bowel Syndrome, and can be responsible, in some cases fo...r symptom reduction. It was never meant as a long term way of eating. Remember, sensitivity to some foodstuffs can very well be temporary, and is different to a food intolerance or allergy. Performing a challenge to individual foods after a period of time is important to ensure food and fibre diversity is maintained, and the microbiome is well-fed. 21.01.2022 Are you curious about food neutrality? It is our birthright to eat intuitively and to sense our bodys needs, but our food and diet obsessed culture takes that away from us. We are surrounded by food morality, instead of food neutrality, and we suffer because of it. Food becomes good, evil, bad, toxic, addictive, dangerous, or allowed. Diet culture does that, and it moves us away from a way of living in our bodies and nourishing them that is life affirming and celebrator...y. Food neutrality allows us to describe food and meals by their innate characteristics, not imposed moral judgements. Satisfying Delicious Tasty Appetizing Nourishing Filling Adequate Sufficient Crunchy Warming Cooling Messy These are just some examples of neutral ways to describe snacks and meals. I am not saying that the way we eat over weeks or months doesnt affect our health. Of course, it does. But lets just not judge EVERY SINGLE FOOD OR MEAL CHOICE. For many susceptible people FOOD feels like the bull in the china shop their relationship with food feels disruptive, wrecking, awkward, uncomfortable, destroying, uncontrollable, disastrous. It just feels wrong, and finding any control over the situation seems elusive. How to bring the china shop back to a calm, quiet safe space? The thing with food is that it is necessary for survival, it can be delicious, its social glue, its highly symbolic for some, and for others it is part of our identity. We also can have all sorts of emotional attachments to food. We dont have the option of giving it up! So, the only way forward involves making peace with it and finding food freedom. 21.01.2022 Yes, our bodies are wise, but unfortunately our culture can f**k up the signalling. This is particularly true when it comes to food and body image. Taking the time to learn the language of our bodies is worthwhile and valuable for staying in tune with our bodies needs, healing and learning new ways of doing/being when old ways no longer serve us. What have you learnt from your body? About what to eat that is deeply nourishing? About how your body wants to move? rest? be ca...red for? We can spent years listening to what others tell us is best for our body, what we think we SHOULD be doing, how we SHOULD be eating or moving our bodies. Listening to the experts, the guidelines, the latest fad diet, and following external cues and stimulus. How about given our bodies some credit for its intelligence? It wont come instantly or overnight. There is learning involved. For some this may seem like hard work. But boy-oh-boy the results just might be magnificent. 20.01.2022 Put up you hand if you havent heard of intuitive eating (IE)! Its all over social media, right? And yet, there are still plenty of misconceptions being shared about it. IE is an approach to health and food that has nothing to do with diets, meal plans, discipline or willpower, focuses on tuning into your body signals, and breaking the cycle of chronic dieting, and ... teaches you how to get in touch with your body cues like hunger, fullness and satisfaction while learning to trust your body around food again. The IE approach was created by two American dietitians, Evelyn Tribole and Elyse Resch, in 1995. (4th ed of the their book to be released mid 2020). There is currently a flurry of new titles under the non-diet approach umbrella, many of which contain ideas derived from/informed by IE principles. From a non diet dietitian perspective, intuitive eating is a framework that helps us keep nutrition interventions behaviour-focused instead of restrictive or rule-focused. This is a positive step, as rule-focused/restrictive approaches can do harm. E.g. assoc with development of disordered eating. IE doesnt offer a quick fix, and there are certainly times when there is a need for an individual to work with a nutrition professional with a tailored plan to move slowly towards IE. The good news is that because IE is now 25 yrs old, it is the non-diet approach to health and food that has been best researched. One recent paper (Hazzard et al, 2020) has shown that, in a pop of nearly 1500 Indiv followed over 8 years from adolescence to early adulthood, IE had benefits for psychological and behavioural health, including a reduced association with both disordered eating behaviours and body dissatisfaction. 19.01.2022 Embodiment is concerned with the body as who we are, the subjective, body-felt sense from the inside-out - a first person perspective and the application can be to wellbeing, (and also to relationships, leadership, psychotherapy, education, etc.) In the medical model the body is viewed as an object - a third person perspective. First person experience is usually considered to need validation whereas scientific understanding is privileged. The embodied paradigm views mind..., which is not only the brain and body as inseparable, having a bi-directional relationship as opposed to being disconnected as in conventional Cartesian dualistic thinking. We can think of our experience of wellbeing on a continuum: at one end we have embodied wellbeing and at the other performative health, the approach to wellbeing that is particularly valued in modern medicine. Examples include: - dieting, exercise routines, running programs, taking pills, annual checkups where the acts are focused on a purpose or goal: to lose weight, reduce blood pressure, gain strength, get a clean bill of health, etc. However, when that focus becomes a singular one, and there is no longer space to experience wellness in our bodies, to feel what our body wants to eat, how it wants to move, when it wants to rest, or express itself, we are missing out on valuable input to achieving a true sense of wellness. We are often taught not to trust these feelings, that they will lead us down a pathway of gluttony and sloth. On the contrary, I think that argument underestimate our whole selves. Try replacing I should..... with I feel like......and take a first person perspective on your wellbeing. This aligns with the principles of Health at Every Size - by removing the stigma and shame that comes along with a focus on weight, people can focus on true self-care that feels right for them." Non diet dietitians can work with you to identify achievable health goals by focusing on behaviours, not the scales. This actually has benefits for everybody. The dominant health care approach is still the weight normative approach, where the assumptions are that thin=healthy, weight loss is achievable and sustainable, and that reminding people that they are fat (even in a health care setting) isnt shaming and fat phobic. 17.01.2022 Alongside the explosion of information about nutrition and health that modern nutrition science, dietetics and medicine has brought us, there is unfortunately an avalanche of diet myths nutrition disinformation weight loss scams snake oil supplements ... false promises and dressed-up quackery It DOESNT mean you are stupid or weak or slow if you have fallen for any of these. It happens to the best of us! Unfortunately there are still many health professionals who get taken in by gimmicks, not-well-thought-through science, and interventions with short term results with a side order of harm. There are powerful interests shaping what passes as acceptable advice, acceptable interventions and even acceptable surgery. And even within the realm of evidence-based practice professionals differ in how they approach medical nutrition therapy. 16.01.2022 Colonisation and decolonisation are common concepts used within Indigenous Studies, yet these words and the meaning they convey uncommonly appear in papers by nutrition and dietetic professionals. Decolonising Dietetics as a project focusses on how nutrition and dietetic research, services and client work can better serve populations who sit outside the mainstream, especially BIPOC. Colonisation, as a historical and contemporary process, has been seen to be at the root of... health and other inequities experienced by Aboriginal and Torres Strait Islanders (and other First Nation peoples) and other minority populations. The legacy of colonisation lives on. We see time and time again that BIPOC are MADE vulnerable by public policy, social inequity, bias and stereotype. Around 50% of adult First Nations people live with one of the major chronic diseases such as cardiovascular (heart) disease, kidney disease or cancer. Almost one-quarter have two or more of these chronic conditions. Traditionally, dietetic research and clinical practice has largely served the white, dominant culture. Western cultural perspectives have been privileged. Western health and illness paradigms are, more often than not, the default paradigms rolled out, even when there is plenty of evidence that they are largely ineffective in BIPOC communities. This cannot be surprising when nutrition and dietetic faculty are largely white. When the vast majority of students recruited to dietetics programs have been historically white. This needs to change. Things are changing slowly. We have seen this past week space made on instagram for BIPOC voices in dietetics, as part of the #amplifymelanatedvoices campaign. Proponents of HAES believe that restrictive dieting is harmful, and that weight stigma is an important public health issue that needs to be addressed. Basic components of the HAES(R) approach include: ... Celebrating body diversity; Honouring differences in size, age, race, ethnicity, gender, dis/ability, sexual orientation, religion, class, and other human attributes. Challenging scientific and cultural assumptions; Values body knowledge and lived experiences. Finding the joy in moving ones body and being physically active; Eating in a flexible and attuned manner that values pleasure and honors internal cues of hunger, satiety, and appetite, while respecting the social conditions that frame eating options There is a growing body of academic literature showing that a HAES approach to health is more beneficial to overall health, and more protective of mental health than weight normative approaches to health . 13.01.2022 Why freedom? Much earlier in my professional life, I had the urge to distance myself from my profession, to run from my chosen career, shed the name (I didnt feel it fitted who I was), & try something new. I took a break, which ultimately led me to a University post, a PhD and life as an academic for 2 decades. Reflecting back on that juncture in my professional life when I took that break, I made that decision for 2 reasons . First, I knew my University training had...nt prepared me for the work I was doing in community health (which I loved), but I didnt know where to turn to fill the knowledge and skill gap. Secondly, peoples response to me telling them that I was a dietitian was so often met with: "I couldnt image having lunch with you, you would be judging what I was eating" As a dietitian, you must have a perfect diet!" I bet you dont ever eat unhealthy foods!" Are you good all the time?" I bought you another vegetarian cookbook for your birthday because you must be a vegetarian, right? It got under my skin. But more than that, there seemed to be this huge schism between what others thought I should be, and what I felt I was becoming. That felt uncomfortable. At the time I didnt have what was needed to resolve those feelings. Ive grown a bit since then. What people say mostly tells me something about them, not about me. Ive gathered skills to equip me with the challenges of working with people with food and body issues. I still love being a nutrition professional I love being a non-diet dietitian I love helping people who want Freedom from judgement about our own and others bodies and weight Freedom from judgement about what we and others eat Freedom to make food choices that honour their health, culture and taste buds Freedom to heal our relationship with our body and food Freedom to really listen to your bodys needs. 10.01.2022 Food can support our lives in a myriad of ways. We can think of the nutritional value of food, and its ability to support growth, repair, physical exertion, and all the many action required to keep our bodies functioning and reproducing. But of course there is more. Food sustains us emotionally, spiritually and socially. Food can entertain us and delight us, and be used to create lasting bonds between couples and groups. The foods that have meaning for each of us personally... can vary enormously. As a nutrition professional, Its good for me to keep this bigger picture in mind, to practice moderation, and invite food to be part of my life in the broadest sense possible. Sometimes, things go awry. An individuals world can become dominated by a focus on food in a way that blurs out much that is good and enjoyable in their lives. Food morality overshadows every decision about what to ingest. Each food is forensically examined for its value in terms of calories or macronutrients. Food binges can substitute for lack of alternative metaphorical nourishment. These are instances where food consumes a life. Eating disorders do not discriminate. There is not one body type affected. Individuals do not do it to seek attention. 09.01.2022 Thats right. Attributing a moral value to foods makes no sense. Foods are not inherently good or bad, they cant be. We would be better off rejecting the idea of good food and bad food, and stopping describing foods as a guilty pleasure or guilt free. I know, we see this language everywhere, its commonplace: but that doesnt mean it serves the common good. I would argue that it does far more harm than good. The notion of foods carrying a moral value is not only unn...ecessary and unhelpful, but can be confusing, misleading and harmful. Repeatedly hearing or using good foods/bad food language and dichotomy can disrupt our intuitive eating skills (and everybody can benefit from honing these skills). Your food choices say nothing about your value as a person. Further, it promotes the idea that a foods worth is measured in only one dimension its nutritional value. On the contrary, beyond nutritional value, every single food has some value and significance, benefit, purpose, and/or function. Categorizing a food as good/healthy or bad/unhealthy limits the way we can pursue health. Enjoy your food. Enjoy every snack, meal or feast you prepare, or is prepared for you. 08.01.2022 Trauma-informed care means that as a practitioner I am alert to the possibility that clients may have had experience of childhood or other past or current traumas, which could be affecting their health, wellbeing, daily interactions and functioning . As an Accredited Practising Dietitian this is important to me in 3 key areas: First, understanding the neurobiology and impacts of trauma, gives me another way to view and interpret disordered eating practices, body dysm...orphia and functional gut disorders. (it might not always be relevant, but it doesnt hurt to use this knowledge as a starting point when dealing with long term difficult-to-treat problems ) Second, reframe challenging emotions and behaviours as adaptive responses to trauma (labelling anybody as non complient is never ok or helpful) Third develop strategies which enhance client safety, trustworthiness and transparency, sourcing peer support, collaboration and mutuality, empowerment and choice, and addressing cultural, historical and gender issues. Many people (myself included) have had interactions with health care providers that have not lived up to standards for trauma-informed care. At worst these interactions can lead to re-traumatisation and at best the experience can be uncomfortable, disempowering, and leave the client questioning their own truths. So my message is to trust yourself, and value your gut instinct. There are health professionals that understand, and that know this stuff. Yes, it is about having cancer, but this is a truly uplifting book about an extraordinary woman. I loved it. 03.01.2022 Yes, Im late, but as they say, better late .... Ive been thinking a fair bit about Irritable Bowel Syndrome these past months, not just because it is the spotlight for the month of April . Its a strange kind of diagnosis, when so much of medicine provides such an incredible amount of specificity and detail (often down to the cellular level), where diagnostic tools are really sophisticated, IBS is handed out as a diagnosis when other things have been eliminated . You h...avent got bowel cancer, you havent got inflammatory bowel disease or coeliac disease or gluten intolerance, or a gut infection, (thats all great news) BUT you still experience persistent abdominal pain and cramping gas and bloating diarrhoea or constipation..... ...and often to an extent that interferes with daily living. Bowel disorders without underlying disease process (that can be identified) is lumped in together as IBS or functional gut disorder. Lets call it an anti-diagnosis. But there is some hope at the end of the tunnel. There is an increasing amount of talk about a number of reasons that individuals can develop debilitating unexplained gut symptoms. These include Small Intestine Bacterial Overgrowth (SIBO) Small Intestine Fungal Overgrowth (SIFO) Dysbiosis Pathogens Nervous system dysregulation So a diagnosis of IBS is really a signal that the detective work is only beginning! The best thing you can do is start by finding a clinician that is interested in working with you. 02.01.2022 OK, so I might do the long no to myself, while I continue to act professionally (of course), but you get the idea. There is plenty of confusion around this one, and its not just people with IBS that get the wrong information. I have found myself explaining to some doctors why following the Low FODMAP diet long term is not a good idea. While this diet is sometimes used in the management of people with Irritable Bowel Syndrome, and can be responsible, in some cases fo...r symptom reduction. It was never meant as a long term way of eating. Remember, sensitivity to some foodstuffs can very well be temporary, and is different to a food intolerance or allergy. Non-diet dietitians adopt a non-diet approach which Honours diversity in body shape, weight and size and Holds the client as the expert in their lived experience.... Encourages noticing and acting on internal body cues to rebuild body wisdom. Encourages awareness and mindfulness of external eating drivers. The non-diet approach avoids weighing and measuring your food and your body. Other behaviours like recording detailed food diaries, intricate planning of and rigid meal scheduling, fixed energy intake, specific weight loss goals and the use of diet foods that serve to further separate you from your internal drivers to eat are not part of the non-diet approach. While non-diet dietitians often work with people with eating disorders and disordered eating, this approach actually has much wider applications in dietetic practice. Remember, non-diet dietitians are anti diet culture and anti weight stigma. They are NOT anti health but rather adopt evidence based, diet-culture-free interventions based in the Health at Every Size approach. Non-diet is NOT anti medical nutrition therapy (MNT). MNT is a necessary part of the management of conditions like diabetes or coeliac disease, and IT IS possible to do this in a non-triggering way and without introducing diet culture. Non-diet is NOT anti people who want to lose weight or have lost weight. Living in our culture it is really hard not to be touched by diet culture. It is possible for our behaviours to still be affected by diet culture as we are on the journey to be rid of it.
