25.01.2022 Hi again! COVID really is running wild here in Melbourne at the moment so let’s just focus on something that we actually have some control over! Today’s chat- Radiation Fibrosis. This arises during and after radiotherapy. In short it’s an overactive healing response to healthy tissue as well as tissue healing after surgery. It can actually continue on for many years after treatment. Main areas of concern? Our lymphatics! ... 1) Interestingly radiotherapy doesn’t directly affect the lymphatic vessels but overtime fibrosis and hardening of neighbouring tissues can compress and block lymphatic flow. 2) Did you know that unlike the Lymph vessels the Lymph nodes targeted with radiation can turn into fatty tissue - this in turn as you can imagine can impact the normal lymph flow in the area. 3) Radiotherapy can also limit the regeneration of new lymphatic vessels. Take home here? Get on to Lymphoedema prevention strategies ASAP and lots of manipulation of that lymph! Early intervention to focus on Lymphoedema prevention, exercise and stretches, as well as the use of negative pressure techniques such as Cupping and Kinesiotaping can help improve lymphatic flow. 24.01.2022 Playgrounds are back open again in Melbourne this week. Didn’t think I’d ever be saying those words together in a sentence but how free we felt today as our boys ran around, and were back challenging there little bodies again on surfaces other than our flat, safe apartment floor! Seeing my 5 year old on this balance wheel has got me reflecting on the last months and I’ve thought of a topic for my next post! 18.01.2022 Ok I admit, probably not the most enthralling thing to be chatting about BUT it really is quite important. Follow my next few posts and we’ll get our heads around this healing process that can have some significant ramifications on how we move and groove and especially on how our lymph gets around. Fibrosis is a healing response. It’s a hardening, a thickening or a scarring of the tissues in our body. It can be a response to surgery, radiotherapy or any other insult such as ...an infection, or injury. When fibrosis occurs in response to an injury the term scarring is used. However, fibrosis can also occur as a result of other pathological processes. Over the next few days we’ll talk about the different types of fibrosis relevant here and why it’s all so important. 16.01.2022 Over the last months with many of us working from home there has been the growing issue of less than ideal ergonimcs causing less than ideal postures and less than ideal musckuloskeletal problems. With no standardised workstation setup at home, with no colleagues around to incentivise sitting up straighter with both feet planted on the ground & with a highly disruptive virus that’s not going away, your posture at your desk is probably not high on your priority list. But focus...ing on how we hold ourselves is soo important, as our body tends to adopt positions that require the least work because of weaknesses & inflexibility. So this is when YOU the posture police steps in. 1) Indentify the problem. Catch yourself in the act of bad posture. Like when that Zoom camera goes on & you wonder how long you’ve been slouched like that for or you notice you’ve had a leg up on a chair and now have a severe case of pins&needles or when you are sitting on the edge of a chair to finish typing one last thing before getting up to do the laundry. All positions that cumulatively contribute to tighter & weaker muscles. Do you have aches at the end of the day because of how you’ve been positioned. Is there anything that can be changed? Is your back needing to be strengthened? Is there tightness that needs to be stretched? 2)Future proof the problem: Set up visual prompts such as a mirror nearby, or post-it reminders at the workstation to get up & move frequently, or sit on a chair with wheels or a swissball to encourage movement, or fix the lighting in the area so you can see your screen clearly. Move away furniture that encourages bad posture. And if your are working on a beanbag, don’t. 3)Fix the problem: along with regular trips away from your desk (which is a no brainer for those of you with kids!!) & keeping up your exercise routines, other go-to moves to break up that desk posture are: >>Gentle neck & shoulder rotations >>Getting on the floor for a change of scene & posture. Add on a few snow angel stretches on your back with or without a foam roller for extra relief >> Deep breath often Be your own posture police, be body aware and reduce your chance of injury! x

13.01.2022 A surgical incision results in the body going into a wound healing response. Generally wound healing takes place in 4 stages. Stage 1 Haemostasis: immediately after an incision is made, your body creates a clot to stop the bleeding. Stage 2 Inflammatory: 1-2 days after surgery your body ships the clean up crew to the site to get rid of dead cells, debris and fight any infection as best as it can.... Stage 3 Proliferative: up to 3 weeks post-op your body starts laying down the building blocks of healing to fix things up. The main building block here is Collagen. Stage 4 Maturation: starts 3 weeks after surgery. Here the collagen is realigned to maximise on strength and function. This is the start of scar formation. However unlike an ordinary wound a surgical incision is understandably a lot more complex. Wound healing takes place over a much larger area than is visible on the surface, because a large matrix of connective tissue has been worked on under the skins surface during the surgery. Over this larger area as structures heal the body creates internal scars referred to as scar tissue and can often lead to hardening or fibrosis. The extent of the scar tissue & how it heals can vary depending on a variety of factors. Comorbidities such as diabetes, circulatory conditions or autoimmune conditions can impact the healing. As well as how the body responds to some medications including chemotherapy. An infection post operatively can also impact the scar formation. What’s the take home? 1) Ensure this scar tissue is worked on starting at 3 weeks after surgery with regular manual therapies to avoid hardening & fibrosis as much as you can. This includes daily massage in or after the shower to help collagen realignment, encourage healthy blood circulation & also keep lymph fluid moving along. A Lymphoedema therapist can guide your technique depending on your presentation. 2) Check to see if there’s irritation with tight clothing or bras on the skin as this can impact scar tissue formation under the skins surface. 3) Stretching programs as provided by your Physio can help keep the area moving as it should. 11.01.2022 Right so this one is especially important and relevant for all stages of Lymphoedema. Lymphostatic fibrosis actually starts very early on even before there is clinical signs of Lymphoedema. It goes a bit like this...... Surgical/ Radiation Fibrosis Lymph fluid gets a bit stuck An inflammatory response occurs but this time instead of fixing things the stagnant lymph fluid starts attracting fat cells Fat cells start sticking to the tissue around it A squidgy gel like area is created Overtime the tissue continues to harden and fibrose and progress into later stages of Lymphoedema (unless managed appropriately!) Take home here? Management of fibrosis soon after you’ve healed from surgery and radiotherapy can limit functional changes, tissue fibrosis and progressions of Lymphoedema down the track.
