25.01.2022 FOREARM PAIN Elbow Pain - Tennis or Golfers Elbow? These common tendinopathy elbow complaints consist of two similar, yet different injuries. Both injuries are degeneration and/or inflammation within the tendons of the forearm.... Tennis Elbow (Lateral Epicondylosis) refers to the common extensor tendons that attach to the outside of the forearm at the elbow. These tendons connect to the muscles that extend your wrist and fingers. Similarly, Golfers Elbow (Medial Epicondylosis) refers to the common flexor tendons that attach to the inside forearm at the elbow. These tendons connect to the muscles that flex your fingers and wrist. With both lateral and medial tendinopathy the pain typically will develop gradually. Beginning as pain and stiffness after exercise but often settles with heat. However, as you continue to exercise, the tendinopathy progresses and the pain increases, until you lose strength in the forearm with difficulty lifting or gripping objects. Management Both Golfers and Tennis Elbow do not get better on their own. This is because microscopic tears have developed within the degenerative tendons and continuing exercise will only make the pain worse. It is important not to ignore the problem and consult your physiotherapist for a correct diagnosis. Jayd graduated in 2016 with a Bachelor in Exercise Science from the University of Wollongong, he then went on to completing his Master of Physiotherapy at the University of Sydney graduating in 2018. ... During his undergraduate degree he was involved with the St George Illawarra Dragons undertaking a 3 year internship in load management and strength and conditioning with the 1st grade NRL team. Since joining the team at the end of 2018 he has shown a keen interest in sports related injuries as well as the general population seeing a variety of musculoskeletal and neurological conditions. 19.01.2022 CORE STABILITY What is core stability and how do you activate? Achieving better activation and control of your lower abdominals will assist in supporting your lower spine and improve general strength and stability. If you have good control of your deep abdominal fibers you can decrease the direct load from your lower back and disperse the force throughout the entire trunk. Effectively controlling your abdominals can be integral in the ability to decrease low back pain.... Anatomy You have four main abdominal muscles. At the front you have rectus abdominals, which are your six pack muscles. These muscles fibers are vertical, help you sit-up, are excellent to show off down the beach but are not very effective in supporting the lower spine. In addition you also have external and internal obliques more to the sides of your trunk with muscle fibers in both diagonal direction. These help you twist and side flex and offer some core stability. Your deepest abdominal muscles are your transverse abdominals. These attach to you lower spine and have transverse (horizontal) fibers that attach to the front to the abdominal facia. This muscle sits in a position similar to the corset women used to wear in the past. If these transverse abdominals are activated effectively it can aide is stabilising and supporting the lower spine. How to activate If you place your thumbs just above your hips and slightly towards the front, take a small cough and you should feel the lower abdominals activate. This is a co-contraction of your abdominals as you use your breathing muscles (diaphram and accessory muscles). You will be trying to activate your lower abdominals WITHOUT activating your upper respiratory muscles and your ribs. If you are performing this activity properly, no-one should be able to see you turning your lower abdominals on or off. Imagine you have a piece of string about 2 inches below your belly button and you are pulling the lower stomach in towards the spine and gently lifting towards your head, your lower belly should draw in about 1cm and your should feel the abdominals (where you are placing your thumbs) tense up gently. When contracting your lower abdominals you should be activating about 5-10% of a full contraction, not as hard as you can. Over-activation of your abdominals will lead to poor control and significantly increase intra-abdominal pressure. You should be able to hold your lower abdominals comfortably whilst breathing normally. It is important that you relax your upper chest. 17.01.2022 NECK PAIN Neck pain can arise from a direct accident, gradual onset, or from simply turning the neck. How does it happen?... Neck pain results from damage to the structures of the neck and can often happen from relatively simple movement such as flexion or turning the neck. Where Do You Get Pain? Neck pain can be experienced on one side or both, occasionally feeling symptoms in the head and shoulders. The pain can be a dull ache or sharp pain which is made worse by movement. In some cases it can prevent full movement of the neck. Sometimes you can experience headaches and pressure behind the eyes. Pain can also radiate into shoulders and sometimes arms. What Should You do? If you experience neck pain from an accident, turning your head or simply waking with pain you should consult Thirroul Physiotherapy or medical practitioner for treatment. Management You should not ignore the problem as the symptoms may increase, leading to a prolonged recovery. You should also avoid activities that aggravate your symptoms. Neck pain often does not produce any long term effects but it is important to get properly diagnosed to ensure an appropriate and effective treatment. Neck pain does have a tendency to come back so it is important to also know self management strategies to help in the acute stages. Load management has fast become an essential component of injury prevention in both the sporting arena and general population (physical labour, gardening, house work etc.). We refer to load as the stress placed on the body during activities which can vary in intensity and duration. For example, a sprinter (high intensity load and short duration) vs distance runner (low-moderate intensity... load and longer duration). Errors in load management has been found to be a significant risk factor in the cause of injuries such as bone stress, tendons, cervico-thoracic and lumbar dysfunctions. Following a period of time-off a particular sport or activity, it is vital that the return to activity program follows a progressive loading plan. Load management is highly individual and dependent on multiple variables including but not limited to pathology, training history, time-off activity and competition goals. This will be assessed and discussed with you by your treating physiotherapist. We wish to advise that we are open and operating as per usual as allied health professional services are deemed an essential service. ... It is important to remember that movement is medicine however, this movement needs to be specific to your condition and needs. ... The following types of exercise are recommended for musculoskeletal pain: Graduated training the best medicine is to stay active and gradually start walking at a low-moderate pace. This will assist in maintaining not only aerobic capacity, but assist with being able to complete activities of daily living Postural correction an accredited exercise professional will be able to provide simple cues and exercises to improve and maintain correct posture throughout day to day activities. Strengthening people with MSDs need to re-educate their body how to move without pain, therefore exercises catering in re-educating the body how to move correctly, is extremely important. Muscle strength, particularly in the small muscles that stabilise the lower back, does not return automatically when low back pain eases. Flexibility along with strengthening exercises, stretching or flexibility training needs to be incorporated in the program to restore full range of movement and improve their ability to complete functional activities. 03.01.2022 We are open! As an essential health service, our clinic remains open. Physiotherapy appointments are continuing as per usual, with the additional option of phone or video consultations available.... While we have had to cease group exercise sessions due to the restrictions, we are still running Exercise Physiology services including assessments and 1:1 sessions. Sessions are running on a 1:1 basis and Telehealth services are available for those patients that do wish to come into the clinic for face to face sessions however, require home exercise programs. We hope everyone is staying well and that these options mean all of our patients will be able to continue exercising in a safe manner. 02.01.2022 Have you met Steff? Following on from our Meet the Team series, we would like to introduce you to our lovely Womens Health Physiotherapist Stephanie Holroyd. | B.Phys, APAM... Stephanie completed her Bachelor of Physiotherapy at Newcastle University in 2012. Stephanie has worked in the private practice settings since graduating and is dedicated to delivering quality care and treatment to all age groups. Stephanie has special interest in sports injuries and has completed further study through the Australian Physiotherapy Association and Sports Medicine Australia. Completed courses include Lyn Watson Shoulder Course Level 1, Segmental and Dry Needling, Sports Level 1 and 2, Upper and Lower Limb Casting and McKenzie Level 1. Along with Stephanies interest in Sports Physiotherapy she has also undergone post graduate study through Monash University in the area of Paediatrics. Stephanie is qualified in treating newborns through to young adults in acute, chronic and complex injuries. Most recently Stephanie has completed qualifications in Womens Health assessment and treatment. This includes pre/post-partum assessments and return to sport programs, recovery of pelvic floor dysfunction and urinary incontinence. Stephanie is able to perform Pre- Pointe assessments and Dance Screenings. Stephanie is competent in Vestibular Assessments and retraining including treatment of Benign Paroxysmal Positional Vertigo, Horizontal Canalisthesis and problems with dizziness. As a practitioner, Stephanie is dedicated to providing current evidence based practice, ensuring that every client is treated optimally for their condition. Providing a healthy and safe environment for all of our clients and staff is our major priority, so we encourage you to take the time to read the following. While there is no current need to cancel classes and appointm...ents, we will continue to closely monitor the government and WHO updates to stay prepared. At Thirroul Physiotherapy and Sports Injury Clinic we ask you to support the community by taking the following precautions: - Be kind to yourself and take the opportunity to rest at home if you are experiencing any symptoms of sickness - Please do not attend the clinic if you have travelled in the last 14 days - Do not attend the clinic if you have been in contact with a person who has been found to be positive with the COVID-19 - If we can access hand sanitiser we will endeavour to provide it to all of our clients, otherwise washing our hands regularly is the best preventative measure. - If you would like your Physio to wear gloves please do not hesitate to ask, some of our therapists will be wearing gloves for all patients. - Frequently touched surfaces will be cleaned regularly with hospital grade disinfectant sprays and wipes following each class or after each patient. For clients attending our gym and group exercise classes: - If you have travelled in via public transport we ask you bring a change of clothes to exercise in. - Staff will be asking if you would prefer tactile cues/physical prompts during these classes, if you would prefer nil physical contact staff will use verbal cues to guide you. - Staff will be adhering to guidelines on social distancing within the gym space. Mats and equipment will be spaced out to give 1.5m. - All clients will be required to wash their hands thoroughly with soap and water before and after each class. - Please ensure you bring your own large towel to each session if attending Pilates or one of our other gym based exercise classes. - If you are a member of our gym and attending independently, we will require you wipe down all equipment after use. Please remember that we are a small, independent business and we are diligently focusing on keeping our spaces clean and safe. Peter McIntyre | BAppSci (Phys), Mens Health, MAPA Peter McIntyre graduated from Sydney University in 1990 with A Bachelor of Applied Science in Physiotherapy, initially being employed with NSW Health in the public hospital system and moving into private practice at Thirroul Physiotherapy in 1993. Peter has extensive experience in the diagnosis and treatment of sports injuries having completed post-graduate courses with Sport Medicine Australia, the Australian Physiotherapy Association, University of California San Diego, University of Florida, Newcastle University, National Rugby League and Australia Institute of Sport. Completing qualifications in mens health assessment and treatment in 2014, including pre and post prostate surgery, Peter has been using real time ultrasound to assist these patients recovery from pelvic floor dysfunction and urinary incontinence. Professional sports interests include rugby league, soccer, cycling, motorcycle racing, snow skiing and boarding, surfing and ballet dancing.
