24.01.2022 "Although induction at term could prevent rare cases of fetal death," write SeijmonsbergenSchermers et al (2019) in the BJOG, "all induced women will be expose...d to potential disadvantages. Women whose labours are induced have a higher risk of postpartum haemorrhage, uterine rupture, hyperstimulation resulting in fetal distress, and perineal injuries (Miller et al. Lancet 2016;388:217692). Furthermore, more women need pain medication and have limited freedom of movement, a longer labour, and a negative birth experience. There is increasing evidence that suggests negative consequences of synthetic oxytocin administration. This may influence maternalfetal bonding, the maternal psyche, and neonatal preparation on being born. A large cohort study found higher rates of jaundice, feeding problems, infections, metabolic disorders, and eczema up to 5 years of age among children born after induced labour (Peters et al. Birth 2018;45:34757)." This is just the beginning of half of a debate about whether or not induction should be offered to all women at term. It's fascinating to see that the publishers have made the "pro" induction paper freely available, and yet one can only see the "against" argument, which begins with the paragraph above, by either paying or having academic access. That's why I'm sharing this today. Women deserve better than this. The evidence on which pro-induction arguments are based is not as sound as it could be. We need to understand that waving a research finding around isn't good enough. These days, "evidence" is cheap. We need analysis and careful thinking, especially when the issues are as complex as they are here. And I am constantly meeting practitioners who work on labour wards who laugh at the finding that 'induction reduces caesarean rates'. Not in their experience it doesn't. Induction will be the right decision for some, and not for others. Some women regret agreeing to induction, and others are happy. We need to share the viewpoints that are being suppressed. 24.01.2022 Today our Australian Birth Place study has been published in the BMJ Open . Thanks to the wonderful leadership of Caroline Homer and Vanessa Scarf and a great m...ultidisciplinary team. This was a NHMRC funded study and has been nearly seven years in the making. You are nearly 6 times more likely to have a normal birth at home and nearly three times more likely in birth centre compared to hospital. Maternity services should offer all women with low-risk pregnancies the choice of having their babies at home, say researchers behind Australia's biggest-ever birthing study. The Australian researchers analysed 1.25 million deliveries across every state and territory and found that planned home births had the highest rate of normal no-intervention deliveries (95.1 per cent) followed by midwife-run birthing centres (89.2 per cent), and labour wards (78.6 per cent).

23.01.2022 Researchers looking at the effectiveness of prenatal yoga on delivery outcomes have undertaken a meta-analysis, which, "identified that yoga improved vaginal de...livery, decreased premature delivery and birth weight of newborns, shorten the labor duration." They concluded that, "Prenatal yoga is an effective complementary medicine to improve delivery outcomes and not to increase the risk of fetus, which is worth recommending to pregnant women." They noted that the studies involved in this meta-analysis were not all of high quality. The themes that emerged from the interview data were communication, information-seeking behaviour, out of sight out of mind, choice and trust, and patient advocacy. RhD negative women want information and to be involved in the decision-making process in the prevention of RhD alloimmunization. Working with RhD negative women to develop decision-aids and/or other educational tools to aid in the decision-making process are warranted." But when those women who could benefit from hospital care, but feel safer birthing unattended, it shows the depth and breadth of how flawed this system is.

14.01.2022 Homebirth Newborn Resuscitation ~Potential trigger- newborn resuscitation~ One of peoples greatest concerns about homebirth is-... What if my baby isnt doing great at birth? These photos give an insight into homebirth resuscitation. The differences with hospital resus? The baby stays attached to the mother. In most hospitals in Australia the first thing that happens is the umbilical cord is cut and the baby is taken to a resuscitare on the other side of the room. At home, the cord is left in tact so the blood flow from the placenta continues to go to the baby, giving them essential red blood cells to oxygenate their blood until their breathing is established. The baby is moved and massaged, blown on and talked to to stimulate their reflexes. If this doesnt work, room air is bagged into them through a mask (or mouth) to help trigger their lungs to breathe. In a lot of cases, this is enough. If it isnt, there is a tank of oxygen close by, ready to administer. Youll see in these photos, the baby stays attached to her mother, everyone keeps their calm and the midwives methodically step their way through these processes. You will also notice the mother holding on to the cord to check it is still pulsating, giving reassurance her baby is receiving what it needs. You will also see the midwives assessing the babys heart rate throughout with both touch and stethoscope. Everyone is informed every step of the way, and everyone is involved. I am all too aware that a resus is not always this straightforward, and that babies can need a lot more help. What happens when, for example, the baby needs chest compressions (which are necessary when the fetal heart rate is below 60 bpm or if ventilation doesnt increase the heart rate to above 100 bpm)- in that case the woman is asked to get out of the pool and CPR is still commenced with an intact cord. As the first line of action- doesnt leaving the cord in tact make a whole lot more sense? . A recent study found it does. Intact cord resuscitation versus early cord clamping in the treatment of depressed newborn infants during the first 10 minutes of birth (Nepcord III) a randomized clinical trial- found that the newborns oxygen saturation (Sp02) and APGAR scores were higher at several measured points after birth along with other benefits such as both initiation of breathing and respiration rate when a necessary resuscitation was done bedside with the cord intact on an infant who was not breathing at birth. A few days ago I had the pleasure of attending the beautiful HOME BIRTH of this BIG man. His mama saw a Licensed midwife throughout her pregnancy and prepared physically and emotionally for the birth of her second baby. During her 4 hour long labor she rocked, walked and danced her baby down, she instinctively pushed in the standing position for 15 mins and delivered all..... are you ready for it.... 14 lbs 7oz of him! She was also able to deliver this chunky baby with no tears. After doing their research this family chose a midwife to guide them through their pregnancy and birth and were able to have an amazing experience in the comfort of their own home. This baby stayed with his mama on her chest breastfed within the first hour. Mama and baby are doing amazing. People are capable of delivering their baby, even when they are big! Way to go mama! 01.01.2022 Today is International Home Birth Day, and the global pandemic that we are still in the middle of has finally made it obvious that home birth with community-bas...ed midwives is a public health imperative. Not a "nice to have addition" not a "frill" not an "additional service" - but a basic service that all other maternity services are based on. Coronavirus had laid bare all the structural issues with maternity care in settings all around the world, including our reliance on obstetric care in big, centralised hospitals and how quickly this can become a problem. While there will always be women for whom it is safer to birth in a hospital, the vast majority of women need to have more flexible options available in their own communities. Coronavirus has also been the time when opponents to home birth in countries like Slovenia and in Canada (the province of Nova Scotia) use a the excuse of a pandemic to outlaw previously normal home births. While these were extreme country and region-wide policies, smaller communities also saw their home birth services cut or temporarily closed. The question of out of hospital and home birth around the world is not a simple yes/no, it is "how do we make our maternity services agile enough to handle unexpected pressures and emergency situations" and "how do we make pregnancy and childbirth safer for women cannot reach a hospital". The answer is investing in midwives and community midwifery services. Happy Home Birth Day to the families who chose to welcome their babies at home, to the families who would like to but don't have the option, to the families who considered it for the first time during the pandemic, to the families who do not have access to hospitals and most of all, to the doctors who advocate for home birth services and the midwives who make safe home birth possible, even when systems and governments try to make it impossible.

What we want is medicare funding pushed thr...ough faster and publically available access to free / low cost birth at home. This will not only support our metro women but our indigenous, rural and remote sisters too. We know the government can do these things easily and quickly. We are being swamped with women reaching out wanting to birth at home and stay away from hospitals. We are getting reports that women are being told by their midwives and doctors to prepare for birth at home. Women that would have not considered a birth at home before Covid-19 are now wanting one and scrambling to get one. Some cannot access or afford support to birth at home. Many are preparing to birth alone rather than go to hospital. Women are desperate for safe support, help and alternative options for birth. Women do not want to go to hospital. WOMEN ARE NOT GOING TO GO TO HOSPITAL. Why is the government state and federal ignoring womens voice? AND the risk that they are placing on women and babies through inaction?! The mental health stats for women BEFORE the pandemic were: - Suicide #1 cause of ALL maternal deaths - 1 in 3 experience PTSD from pregnancy and birth - 1 in 10 depression / anxiety from pregnancy and birth These numbers will increase if women are not supported to feel safe and birth how and where they feel safest.
