19.01.2022 What is the Diagnosis? 52-year-old woman was seen at the outpatient clinic for adults with congenital heart disease. Cardiac auscultation revealed a single and loud S2 and a soft pansystolic murmur of tricuspid regurgitation. On clinical examination, clubbing of the toes was noted and cyanosis was present only in the lower half of the body, whereas her fingers did not present evidence of clubbing or cyanosis (Panel A). Using the same pulse oxymeter model simultaneously at he...r fingers and her toes while breathing room air, differential cyanosis (SO2 toes 80%, SO2 fingers 95%) could be confirmed (Panel B). On 10 L O2 via nasal cannula SO2 on the lower extremities remained unchanged, whereas SO2 increased in the upper extremities. 19.01.2022 Giant C-V Waves of JVP is a sign of Tricuspid Regurgitation. Jugular venous pulse reflects the pressure in the right atrium consisting of: a wave - "Atrial contraction" with Tricuspid valve open, precedes S1 ... c wave - "Bulging of close Tricuspid valve" into atrium. As such a tricuspid regurgitation will result in blood flowing from the right ventricle back into the right atrium and then the jugular vein during systole resulting in a "giant" c wave. x descent - "Atrial relaxation" with Tricuspid valve closed, precedes S2 v wave - "Atrial filling" with Tricuspid valve y descent - "Atrial emptying" with Tricuspid valve open Source: http://www.nejm.org/doi/full/10.1056/NEJMicm1103312
