20.01.2022 STRUGGLING WITH PERINEAL & UROGENITAL CASES? Dear Colleagues, Surgery of the penis and bum can be tedious, sometimes smelly and confronting. Get too close to the nerves and you may leave the pet with an incontinence issue. That ends up putting a lot of pressure on yourself and can make for an incredibly stressful surgery.... We’ve put together a few Perineal & Urogenital tips that might be able to help you on your way! Tip 1 : Digital rectal examination is the main-stay of diagnosis for perineal hernias. Palpate both sides even if the external swelling is only obvious on one side. A tip for the digital rectal exam is to make your finger into the shape of a hook and point it laterally. As you withdraw your finger, if the finger slides right out then it is not a hernia; if it catches then it is a perineal hernia. Tip 2: Anal sacculitis is most commonly seen in small-breed dogs but can present from a wide variety of signalments. Surgery to remove the anal sacs is indicated in neoplasia or when there has been a failure of appropriate medical management including: manual expression, lavage, instilling topical antibiotic-corticosteroid preparations +/- systemic antibiotics, adding fibre to the diet and treating concomitant dermatoses. Tip 3: When suturing the urethral mucosa in a perineal urethrostomy make sure you identify the edge of the urethral mucosa accurately. Sterile cotton buds are useful to blot the haemorrhage away to help identification of the mucosal edges (don’t use electrocautery). Take a bite of the urethral mucosa, a small bite of the tunica to help compress the intervening cavernous tissue, and then appose the mucosa to the skin. Would you like to learn more? 18.01.2022 , . R U OK Three simple words that can mean all the difference for about 20% of the population (if not more). That’s about 5,109,328 Australians who experience a mental illness in any given year. With Covid affecting our everyday lives significantly, this number could be even higher.... I was visiting my doctor the other week. As you can imagine with Covid about, the reception staff were wearing the brunt of all the upset patients. So it was really lovely to learn that my doctor had spent a little time with the staff, supporting them and being that compassionate ear. BUT it’s not just the disgruntled people that can bring you down. It‘s also the changes in life, illness in the family, fights with your loved ones, breakups, and so many other things. I work a lot in this support realm within my capacity as a Peer Support Officer for the State Emergency Service. Sometimes it is related to an activation and other times it‘s something completely unrelated to the job. It is an honour to be trusted with the confidentiality of my fellow colleagues for what they are experiencing. Sometimes it’s just being that compassionate ear, other times it’s affirming that what they are experiencing is very normal. Most importantly, it is recognising that what they are dealing with may benefit from speaking with a professional. These days the health of your mind is becoming more of an open conversation and it’s ok not to be ok. Caring for the health of your mind is just as important as your car getting serviced, or getting that morning cuppa while it’s hot. Please Ask. Listen. Encourage further help. And check in again to see how it is all going. Set yourself up right. It is not just about asking the question, it’s about listening with an open mind, not trying to fix them or pass judgement. Ask them how would you like me to support you or How have you managed this in the past? Then set a reminder for a couple of weeks to check in, see how they are. If you think this is a big issue or they have been feeling down for a few weeks, encourage them to see a health professional. You can make a difference every day, not just today. ? 04.01.2022 Feline urethral obstruction is a relatively common condition, accounting for up to 10 - 20% of feline cases presented to small animal referral and emergency clinics. Now let's say about half of the cat owning population visited the vet in any given year. With around 3.9 million cats being owned across Australia alone (approx. half are males) that means around 97500 cats will present with some kind of LUTD.... Most of the time the emergency care consists of a cystocentesis, epidural and urethral catheter placement. Then determining the cause of the blockage and taking medical or surgical steps to eliminate or manage it. So now it's "Simba's" third time in and he is blocked again. What now?? Refer? Catheterise him again? Offer a more permanent solution? If your cat has multiple occurrences of a blocked urethra that can't be managed medically, you may want to recommend a perineal urethrostomy ("PU"), or surgical widening of the urethra. This procedure is intended to provide a permanent opening that allows crystals, mucus plugs, or small stones to pass out of the urethra; this minimizes the chance of re-obstruction. No surgery is without complications. Bleeding, swelling, stricture, and urine leakage under the skin, this can all lead to recurrence of the blockage. If the surgery is performed properly, it is unlikely that cats will develop subsequent urinary obstructions. Are you offering PU surgery? Or would you like to be offering this surgery? 04.01.2022 One of the incredible things about wounds, is how they heal. The magnificence of the body's ability to regrow cells right before your eyes, if you're happy to stare at it for a number of days straight, that is! As much as they are fascinating, wounds can also be scary. There are so many things to think about. Moist wound management, but not too moist or the skin will slough off. Open wounds versus closed wounds with a drain and closing the dead space. Simple surgical wounds, ...second intention healing or surgical closure. How do you decide what path to take? How do you manage a wound that won't close? What about when it encompasses a limb and the body? The bandage changes, phone calls and the rechecks. On average a significant wound will take 30 days to heal and then some - which means patient and client management is going to be a long relationship. Harnessing the strengths of your team can make all the difference to the success and support of your patients and clients. Approaching wounds together can help balance the workload and improve outcomes.
