23.01.2022 Have you had your annual skin check?- Can you name all 5 skin lesions?- CASE STUDY: A 37 year old man attends clinic complaining of 2 lesions that have just appeared on his face. (3 & 5). ... He wants to know what they are? I tell him that I notice 5 lesions on his face I want to look at closer and consider giving advise about & possibly treat. 1/ Solar Lentigo - a sunspot . These can be left and monitored after making sure that they arent cancerous. They can be treated with lasers or chemical peels if concerned but the results depend on how deep the pigmentation goes. Plan: We decided to review it at his annual skin check. 2/ Pre-Cancer (Solar Keratosis) - this can be treated by removing the top layer of skin via burning or freezing. This has a 10% chance of becoming cancer. Plan: We decided to freeze it with cryotherapy. 3 & 5/ Wisdom Spots (Seborrheic Warts) These occur more as we get older. They can be itchy. These can be removed using cautery, cryotherapy or excision. ON DERMOSCOPY , Picture 3&5 they have an appearance a bit like a brain and can feel like they have been stuck onto the skin. Plan: To remove using electrocautery. 4/ A Compound Naevus - a mole that extends into the deeper layers of the skin. This is often better just left. Robert Rickett developed the E-line (esthetic line) which proposed that the lips should not protrude past the E-Line and ideally should be 4mm, for the top, and 2mm, for the bottom, behind the E-line.- In the 3rd photo, I demonstrate Cecil Steiner...s S-Line. Cecil Steiner believed that lips would be ideal, from the lateral perspective, if an ideal aesthetic line drawn from the middle of the columella to the pogonion of the chin hit both of the lips.- The 4th, and final photo, shows the Goode Method which proposes the ideal projection of the nose. The nose should project out 0.55 times the ratio of Line A in comparison to Line B.- It is also important to remember that the columella show should be about 4mm.- In the end, there is an ART to cosmetic injections.
