25.01.2022 Some cases require more then one test case. This patient is very demanding. After already 3 printed try ins we are now closer to a final outcome. This time we have milled a Pmma and finished it with some composite gum. The patient can use this for a week and it will last that long. No Ti bases and only gum on the buccal.... The implants have been placed 2 years ago. Upgrade in Ti Zircon and replacement of a failing hybrid from elsewhere. 23.01.2022 Here is a reminder why we believe that a permanent full arch implant supported restoration should not be made from PMMA denture resin and from denture teeth. As the word denture explains it is meant to be used for dentures which can be removed by the patient and properly cleaned. The use of a titanium bar or a chrome bar is recommended. However bracing the implants is one thing and using a lasting material for the teeth and gums is another. The restoration is only as strong... as its weakest link! Restorations made from zircon or ceramics, stained or layered are designed for permanent placement in the oral environment. We know this from crowns and bridges. So it makes sense to use the same material for full arch implant restorations. Like AllOn X From our experience looking back at over ten years of using zircon and ceramics for full arch implant supported restorations we highly recommend to use zircon with ceramic for a long lasting result. Most patients are overwhelmed when changing from a mostly very deteriorated dentition to the new set of teeth. We believe it takes time to identify what tooth set up and arrangement they would actually love. The temporary also allows to test the occlusion and the intaglio surface can be improved over time. We do not believe in giving a patient a permanent titanium bar with only acrylic teeth and gums after two days of surgery. Why ? This Pekkton PMMA will brace the implants during integration allow the bite and the occlusion to be tested. The soft tissue will settle over the next few month and most important the patient will be able to give us feedback on the aesthetics. So when it comes to making the final all of the above will be taken into consideration. A zircon restoration with some minimal layering like the gum is the only prostheses that will last and stay clean for a very long time. Acrylic is a material for removable dentures! However if same day or next day millings are required for immediate restorations we will mill in house. 04.01.2022 Using a titanium or chrome cobalt bar inside a zircon structure has its benefits. However a monolithic polished zircon surface is superior for the soft tissue health. So when we produce the Ti Zircon option we make sure the interface between the zircon and the bar fits within microns. Using photogrammetry like the Pic Dental System we can ensure a totally passive fit of any restoration. This allows us to promote the monolithic option in many cases.... Contra indications are, limited restorative space, poor implant positioning and large cantilevers. Keeping in mind that a monolithic restoration if it breaks, can not be repaired.
