24.01.2022 Dentists are regularly asked, "How much does an implant cost?" I understand that this question is really an innocent question that stems from the patient not understanding the process. It is not dissimilar to someone asking, "how much will it cost to buy a car?" There are a myriad of reasons why cars will cost differing amounts - and so it is with dental implant therapy. Some of the potential reasons that need to be assessed prior to an accurate quote are: 1) Which tooth is ...being replaced? Are there going to be extra aesthetic demands like when one compares the restoration of front and back teeth. This translates into whether we, as dentists/surgeons/periodontists, are going to have to consider gum/bone grafting to bolster the volume of the "pink" part of the aesthetics - this is the scalloping architecture of the gum which is part and parcel of good aesthetics. This process requires an immense amount of skill and expertise, which, can only be attained by years and years of learning & experience. 2) The quantity & quality of bone that is left in the area where the original tooth was removed. This, often is related to the reason why the tooth needed to be removed in the first place. Was there infection there & for how long? How skillfully was the tooth removed? Other questions that have to be raised, especially when viewing an x-ray such as the one we see here, include: 1) Are all brands of implants equal? 2) Have certain brands/design of implants got advantages in certain situations? How does one judge that the operator has taken this into account? 3) Are all operators equally skilled? 4) Are all operators driven by the well-being of the patient? 5) Should one base your decision on price alone? 6) Is it safe to have one's surgery done overseas? 7) Would this patient go back to the same operator to have all this work redone.

16.01.2022 Thought for the day: No matter how skillfully the wrong treatment is accomplished, it is still the wrong treatment. A comprehensive diagnosis is only arrived at by taking a very detailed and accurate history of all the relevant facts that relate to the problem. Special tests become relevant and, only then, can one embark on exploring the relevant treatment options. With this in mind, I saw a lady this last week who attended West Coast Dental Care with the request that I perf...orm the crown on her cracked tooth that had been previously prescribed by her previous practitioner. She had been referred, specifically to me, by another patient of mine who was very happy with the treatment that she had received. After a careful and comprehensive assessment, we noted that: 1) There was evidence that she was a grinder/clencher of her teeth whilst she slept. 2) Her bite (the way her teeth came together between her upper and lower jaws) was aligned such that there was more pressure on that one tooth than the others especially in side movements. 3) The placement of a crown is an irreversible procedure that is quite destructive on tooth tissue. Before we jumped into this procedure, I suggested that we should try something that involved just some minor adjustment of her bite? She agreed. So, to summarise, I can report that there has been a complete cessation of her symptoms by this minor adjustment that took all of 3 minutes. i.e. The right treatment has been performed because we came to the right diagnosis. What a great outcome for a lady that avoided an unnecessary destructive procedure and at a fraction of the cost. She has also been advised to consider an occlusal splint that she would wear at night to protect her teeth whilst she grinds. See more

15.01.2022 This Saturday and Sunday, Dr Asheen Behari and myself are presenting a 2 day "Hands-on" Advanced Ceramics course at the UWA. It will be an intense 2 full days. I am really looking forward to sharing the most up-to-date cutting edge concepts and various preparatory & other techniques to the 15 other dentists, who have chosen to sacrifice their weekend and come and partake in this programme. Apart from the lecture component, which has taken many hours to consolidate, this programme allows the delegates to try out these new techniques on phantom heads within the University Operative Techniques laboratory. 11.01.2022 I would like to share something that has happened a few times over the last 12 months. There has been a few patients who have attended West Coast Dental Care either: 1) For a second opinion, after having allegedly been told that they need root canal treatments/ crowns for cracked teeth - all due to some tenderness/pain etc whilst chewing or, 2) Looking for some help with tender/sensitive teeth with the thought that they would need a filling or something more drastic because t...heir teeth were sensitive to pressure and cold water or air. After careful assessment, the diagnoses were made that the tenderness could be from them grinding/clenching their teeth at night, whilst they sleep. Every patient, without fail, seemed a little sceptical, that the symptoms they were having, could stem from grinding/clenching their teeth. After some discussion, we fabricated custom occlusal splints for them, which they would have to wear at night. I am happy to report that every single one of these cases have subsequently reported a complete resolution of their symptoms. For patients such as these, it is my job to design and fabricate these occlusal splints so that they are comfortable to wear - this will certainly help in allowing the patient to do their job, and wear them every night.
