23.01.2022 Our Community Care team are seeking a highly skilled, compassionate Registered Nurse who is motivated to help make a difference and support people to remain living in their own homes, to provide nursing services and client care coordination. The nursing component of this role will include completing an overall health assessments within the client’s home. This will include conducting vital signs, undertaking wound care, risk assessments such as falls and skin assessments, co...mpleting advanced care plans, undertaking dementia assessments, liaising with other health professionals and making appropriate referrals. The care coordination component of this role includes being responsible for intake, initial service coordination and care plan development of CHSP clients, HACC PYP clients, Brokerage Clients, NDIS clients and other Westmont Community Care clients as required. The position is also to monitor feedback from clients and staff and respond in a solution focused manner to ensure quality care and services are delivered and feedback is actioned and forwarded according to client needs, in a timely manner. Demonstrated Abilities: Minimum five years’ experience in Community aged care Coordination for the provision of delivery of care and support Excellent time management skills to complete home visits, OHS Inspections, referrals for all clients within time frames and to meet KPI’s Attention to detail to ensure completion of care plans, service agreements, fee declaration and documentation of all client records Strong understanding of consumer directed care and broad knowledge of the Aged Care Act Personal Attributes: Possess strong understanding of working in a holistic environment Display empathy and maturity to deal with a diverse client base, acute medical needs whilst possessing emotional intelligence Be calm and respectful to clients and coworkers at all times A strong commitment to caring for the elderly and the vulnerable Employment is subject to a police check, pre-employment physical and current RN registration with AHPRA. 20.01.2022 Ladies and gentlemen please meet Mrs Isabel (Jean) Modra. Jean and her husband lived in Papua New Guinea for 17 years with their 6 children! Everyday she has her crossword puzzle which she loves. She wanted to let everyone know she is happy and well. And might I say at 102 Jean is looking absolutely fabulous and beautiful! What an inspiration!!

Their contribution, especially during the Covid lockdown, is immeasurable. 09.01.2022 Meet the lovely Lorna. Always happy, always smiling. Once upon a time she was a dairy farmer. She also loved jumping in the caravan for holidays to South Australia. Lorna is just a sweetheart.

07.01.2022 Ruth is enjoying the Tovertafel Magic Table. It is an interactive light projector that was originally developed in The Netherlands. Its a specialist piece of technology designed to support residents living with dementia. It is proven to help promote stimulation through specialised games. The colourful light projections respond to hand and arm movements, enabling residents to play with the light itself, increasing both movement and stimulation. Westmont is the first aged care provider in the area to have a Magic Table.

04.01.2022 Say hello to Charlie! He is always happy to say hello and have a chat. Charlie loves the bus outings every week and is always first in the foyer waiting to go! Charlie worked on a farm when he was young and had a pet cat. You cant walk past the central lounge without saying hi to Charlie!

02.01.2022 Always busy times in the Homestead. Our residents loved hearing The Man From Snowy River, our activities staff are happy buzzing lady bugs and bumblebees and the laundry staff just love posing for photos! Hope you are all having a great week.

01.01.2022 Nesta is busy writing a book about the farms she and husband ran. The books are for her two children. Nesta has the most beautiful handwriting. We can see why her kids wanted handwritten versions!

01.01.2022 Introducing the lovely and independent Lyla Richards. A self confessed feminist, Lyla moved to Wodonga 18 years ago to care for her grandchildren. Lyla has 12 grandchildren and 10 great grandchildren! Lyla was also a commissioned Chaplan working in hospitals. Lyla and her husband used to own a corner store and she loves to read. What a busy and exciting life for this lovely lady.
