25.01.2022 I thought today will be a great opportunity to increase awareness of this important condition. . . Unfortunately seen far too frequently in clinical practice. The information I would like to put out there is a lot therefore divided in multiple posts. . ... . Premature ovarian insufficiency (POI), refers to loss of ovarian activity that occurs before the age of 40 years. . . POI occurs in approximately 1% of the population but may differ between countries. . . 7090% of the time the cause of POI is unknown. Occasionally it could be linked to genetics (XO), autoimmune, infectious, metabolic, toxin-related, and iatrogenic i.e chemotherapy, radiation. . . Family history, early menarche, cigarette smoking, low or nulliparity and low BMI, hysterectomy are all linked with POI. . . Women usually present with secondary amenorrhea or oligomenorrhea, subfertility, and estrogen deficiency symptoms. Some women have absolutely no symptoms and others can have severe symptoms worse than natural age of menopause. . . Diagnosis is usually by history and blood tests. . Most guidelines recommend confirmation by two elevated FSH tests 4-6 weeks apart. Most utilised limit is >40IU/l. Some societies accept 30IU/l and even 25IU/l. . Anti-Mullerian hormone(AMH) produced from antral follicles is thought to be the most reliable measure of reduced ovarian reserve. Although no diagnostic level is established and AMH can be undetectable years before menopause ceases. Also it is cost precludes it use as a universal test. . . Long-term health consequences of premature ovarian insufficiency is a greater risk of: . Infertility . Cardiovascular disease . Osteoporosis . Psychological distress . Genitourinary and sexual dysfunction . Memory, learning problems and dementia. . . 25.01.2022 Premature ovarian insufficiency (POI) once diagnosed requires multidisciplinary management. Optimal management should involve a baseline assessment of insulin resistance, and bone health assessment(DXA scan). Frequency of the tests is based upon risk factors. Lifestyle changes such as ceasing smoking, weight bearing exercise, healthy diet will reduce cardiovascular risk and maintain bone strength.... Hormone therapy (HT) at least until the average age of menopause is the first-line treatment unless contraindicated. Replacement can be with the combination oral contraceptives(COC) initially if contraception is required or because of personal preference, but in the long term HT is recommended to optimise bone and metabolic health. (COC containing 17-B estradiol is preferred) Hypertension, migraines, fibroids endometriosis, obesity are not contraindications to use HT for POI. Along with the excitement of raising a family, there sometimes is substantial legal and medical hurdles to face. . . It is useful to be informed prior to your specialist appointment about available options. I will discuss lesbian couples this time. .... . For female couples, your choice ultimately depends on the age and health of each partner, as well as the desire of each partner to carry a child. . . It is important to remember that same-sex couples may encounter some of the same infertility issues as heterosexual couple. . . Along with social legislation developments, fertility treatment options have also expanded. . . The available options are: Donor insemination ( DI) Clinic-based DI utilises sperm collected from a donor who has been screened for various health problems. . Known donor: This donor may be someone you know and have chosen, who has ‘donated’ sperm to you through a medical facility. . Anon: Donor who has been recruited by the clinic. Collected sperm is frozen for three months. Blood tests are performed before and after the quarantine period to ensure safety, and once cleared for use. The sperm may be administered using procedures such as intra uterine insemination (IUI). . . In vitro Fertilisation/ (IVF- ICSI) During IVF, higher doses of medications are often used to stimulate the development of a larger number of eggs. These eggs are then collected from the ovaries and fertilsed with the donated sperm,using intracyto plasmic sperm injection(ICSI),in the laboratory. They are then allowed to grow in to embryos and ideally on the fifth day ( blastocyst) transferred back into the woman’s uterus. The remaining embryos are frozen for future use. . . Reciprocal or Partner IVF: In this option, one partner provides the eggs, which are fertilised with donor sperm, and the other carries the pregnancy. 11.01.2022 All about sperm donors? . .. :Who is a known donor ? .... : A known donor is someone known to you. A sympathetic friend or partners relative may be willing to assist. Advertising is allowed with prior permission from the health minister. The advertisement has to be approved before it is published. . It is important to consider preserving extra semen for subsequent pregnancies. This allows future siblings to have the same genetic characteristics. : What donor options do I have from the clinic/ anonymous donor? . : Clinic recruited donor can be local Australian or International. . Australian donors: All donors are altruistic. There are currently seperate waiting lists for IUI and IVF. . IUI: Our recommendation is to see the specialist have your suitability assessed and work up done. Have your treatment plan ready to go and then wait for the donor to come available. . IVF: There is currently no wait list for IVF. Please note same donor cannot be used for IUI and IVF. . International donor -l: Monash IVF is in partnership with some large international sperm banks. This is another good option where sperm can brought into Australia from overseas. This offers more options to patients including donor options from different ethnic backgrounds. Usually 6 vials are brought at one time but more can be brought it if needed. The main expense is the shipping costs for the special cryo- container sperm is shipped in. The sperm can be used for both IUI and IVF. This may appear to be slightly more expensive option however may be appealing to many as treatment can be commenced straight away, ability to move to IVF if IUI does not work, share the sperm between partners and fortunately more donors to choose from. Situation may change due to COVID-19. What Information do I get about the donors ? . Usually a list of available donors is sent with physical characteristics, ethnicity, level of education and hobbies. I came home and searched for a local artist who would convert this vision into reality. And Rhys from nacho station was the guy. Rhys is so easy going. No job is too big or small for him.( Also I agree this is way better than graffiti ) . . Just like everyone else Lockdown was the time for the job - and Alicia completed the art work for us in record time! . . It was a beautiful backdrop for zoom consults but we only realised the full impact of it when patients started walking back into our office!! . . Every single person has complimented it, loved it and we, never get bored of looking at it. 08.01.2022 Premature ovarian Insufficiency(POI) or Premature menopause affects all aspects of a woman’s life but when a woman is trying to conceive and is diagnosed with POI the news is devastating. I cannot emphasise how important counselling and psychological support is in this situation. . . Medical management usually involves: .... . Prevention: If there are risk factors such as strong family history of premature menopause, genetics, smoking- early presentation to care providers and referral to fertility specialists is useful. Blood tests can be undertaken to assess baseline hormone levels and ovarian reserve. With recent advances in egg freezing. Egg freezing is an extremely viable option for women to preserve their eggs particularly when social circumstances don’t allow early family planning. . . Treatment options: . . Medication- Once the diagnosis is made, there are no proven treatments to increase the rate of pregnancy for women using their own eggs . Some trials have used oestrogen +/- gonadotropins to induce ovulation although the success rates are extremely low. . . Egg donation : Currently the best chance of achieving a pregnancy for women with POI is oocyte donation. The options in Victoria include Known donor: This can be a sister, relative or friend who be willing to go through IVF procedure and donate eggs. Clinic recruited donor: There is usually a waiting list as donors altruistically donate eggs. Monash IVF has 1-3 altruistic egg donors per year (and sometimes none). The clinic cannot find donors for patients. The world egg bank(TWEB) : Monash IVF is in partnership with TWEB which means they can help import frozen donor eggs from the USA. The egg donors don’t get paid but provide the eggs as a gift. The details can be found on Monash IVF website. . . We usually undertake investigations for both partners unless specifically advised otherwise. . . : If I do have potential fertility problems, how will this impact on who will carry the baby? . : The age of the partner donating eggs and the severity of the disease are all discussed. Personal choice however plays the predominant part. . . : Based on the test results, what are my treatment options, and how much do they cost? A routine cervical screen detects presence of HPV, prior to the development of abnormal cells, which over a period of time can go on to form cervical cancer. So, it’s time to catch up! 01.01.2022 Effective from 1st Feb 2021. . . There will a significant change to the management of women at intermediate risk following detection of HPV, non 16/18 in the changes to National Cervical Screening Program (NCSP) .... . Women with a 12-month follow up HPV (not-16/18) - non high risk virus result with LBC prediction negative, pLSIL or LSIL (intermediate risk result) should be recommended to undertake a further HPV follow up test in 12 months’ time following their previous HPV test instead of referral to colposcopy. . . This means that women can be followed up until a third test without referral for colposcopy. (currently referred at second) . . The changes have been proposed after review of data which showed low likelihood of histologically-confirmed high grade squamous intraepithelial lesion (CIN2/3) or worse in this group of women. . . Exceptions are * Women who are overdue for 2 yrs at the time of the initial screen. * Women who identify as being of Aboriginal or Torres Strait Islander * Women aged 50 years or older Certain group of patients fall outside these guidelines * Women undergoing Test of Cure * Women who are DES exposed * Women who are immune- deficient * Women 70+ ( exit- test) * Self collected sample . . . .
