13.01.2022 To access support at home, (HOME CARE PACKAGE/ Commonwealth Home Support Program) you will need to- * Phone My Aged Care and explain your situation and register.... *You will be advised to have a look at the MAC website. (set a day aside for this) *You will need to have an assessment completed in your home with either Aged Care Assessment Team or RAS (this could take hours)) *You may be asked to complete an Income tested Fee form- (you will need to download and print this or go into Centrelink) *The assessor may tell you what services you would benefit from and what her recommendation is. (many times if not specifically requested, you may be given CHSP referral codes, rather than ACAT.) *You will be required to wait for a letter of outcome. This does not mean you are now getting services! *Another letter will be sent (this can take between 2-18 months) to say you can now receive services. *While you are waiting for the 2nd letter, you should be researching providers in your area. You will need to ask some specific questions in order to get the best out of your funding. *If you are approved for CHSP you will need to ask prices and if providers have capacity (Most do not at this time) *If you are approved for a higher level Home Care Package you may be asked if you want to take up a lower level in the interim. *You need to know what fees each provider charges, as this can vary. What/if they charge Basic Daily Fee/Client Contribution? If they have establishment or exit fees? What are the service fees? Do they have staff trained in dementia, wound care, palliative care etc? Do they match staff to client (carer matching) Do they have Allied Health staff within the company (podiatry, physio, chiro) and if not will they charge a brokerage fee if you need this? What do they charge per KM for transport and do they have fleet cars available? Are staff able to help you care for your pets? Do they offer after hours services? If client is taken to hospital on short notice, will the service still be charged (late fee or full service fee) How contactable is the Case Manager? Are you able to access your statements online? Will they pay invoices for external service such as lawn care, or are you required to use the provider? What equipment are they happy to help you access and what is each providers process for this etc etc *When you have your letter of assignment or codes, you then need to book a meeting with each provider to ask the above questions (or confirm) and sign an agreement and give them your referral code. *You may then need to negotiate fees and services as per your needs. This can be extremely time consuming and can make it difficult if a family member is trying to do this from afar (another state) Family members are often working and looking after their own family and just don't have the time. Or in many cases the client, who may be 90, vision and hearing impaired and computer illiterate cannot complete this process on their own, hence so many people going into care too early, because support at home has been accessed too late or not at all!
