24.01.2022 Robots are becoming more common in the operating room. I have been recently been experimenting with using "Gaston", a French pneumatic robot surgical assistant in DAA hip replacement. "He" is very obliging and will hold surgical retractors still, without getting in the way of the surgeon, without losing attention and without telling any bad jokes :). 24.01.2022 We are pleased to announce the opening of Bayside Orthopaedics - Specialist Orthopaedic Practice onsite at the new Holmesglen Private Hospital. Mr Freedman has a special interest in the Direct Anterior approach for Total Hip Replacement and Patient-Specific Partial and Full Knee Replacement, as well as Hip and Knee Arthroscopy. 24.01.2022 A hot-off the press paper (Journal of Arthroplasty Nov 2017) investigated early functional recovery after Direct Anterior vs Posterior Approach Total Hip Replacement in a randomized control study. The authors reported lower pain scores, lower serum (blood test) markers for muscle damage, better implant positioning and better early functional recovery at 3 months post surgery with the Direct Anterior Approach group. They did report that these functional outcome scores equalise...d with (DAA vs posterior) by 6 months post surgery. Proponents of the posterior approach will conclude that the approach used for a hip replacement doesn't matter as functional scores will eventually equalise - and on that point they're totally correct. A good long term result can be achieved via any approach and there are many valid and successful ways to do a hip replacement. However, my practice is interesting in that elderly patients are the exception rather than the rule. I see plenty of working-age adults with severe arthritis who want/need to get back to work quickly. Of those not working, many are healthy retirees who want to golf or do that long awaited world cruise or other active pursuits. For these patients a shorter vs a prolonged recovery is not an academic exercise but rather a very real and relevant consideration. Even for the genuinely elderly/frail patient, recovery time is not trivial.The approach used for a hip replacement does not affect the long term success of the surgery and there are many valid ways to do hip replacement surgery. However, my personal experience mimics the results of this study in that DAA (in my experience) facilitates fast early recovery.

The attached photo is of a recent total knee replacement patient sitting comfortably out of bed only a few hours post op after knee replacement, with outstanding early knee flexion and no pain. I look forward to presenting how this is achieved.

While there is currently a temporary pause on elective surgery, access to a bulk-billed (no out of pocket charge) consultation gives patients access to an orthopaedic surgeon for assessment, diagnosis, advice on non-operative pain... management strategies and commencement of preoperative health optimisation and medical workup for those who need surgery once elective surgery recommences. Mr Freedman's practice focus is Hip and Knee Arthritis. Common surgeries include Anterior Approach Hip Replacement and Patient-Specific Instrumentation (PSI) Partial and Total Knee Replacement. To access a bulk-billed appointment you will need to have a valid medicare card and GP referral. 07.01.2022 For patients with arthritis who are not good candidates for surgery (e.g. due to serious medical problems that could potentially make surgery risky or because the arthritis is not yet severe enough to warrant operating) a new treatment known as "Cooled Radiofrequency Ablation" may an option to consider. Essentially, radiowaves are used to desensitise the sensory nerves from the joint and thereby decrease the level of pain caused by the arthritic joint. This treatment has been approved by the FDA and has published supporting evidence of efficacy. I am pleased to now offer this through my practice. 04.01.2022 A new article in press at the prestigious Journal of Arthroplasty (July 2017) reports an interesting new randomised prospective study which compared outcomes from Direct Anterior (DAA) vs Posterior Total Hip Replacement. The authors report that: The DAA group had a shorter incision length that the posterior approach patients (9.1 vs 13.1 cm), shorter hospital stay (2.8 vs 3.3 days) and lower self-reported pain scores. Additionally, serum inflammatory markers and muscle dama...ge markers were lower in the Direct Anterior group. The DAA had significantly lower variance in implant positioning (cup inclination and anteversion). Similar rates of intraoperative complications were identified in the two groups. The Direct Anterior Approach was associated with better functional recovery at 3 months based on the Harris hip score, UCLA activity score, and gait analysis. 02.01.2022 I am pleased to introduce a new "Patient Optimised Pathway" smartphone app and wearable Garmin tracker for our patients undergoing Hip and Knee Replacement. The new app will guide patients through educational content, pre-operative preparation, a daily "agenda", post operative rehabilitation goals and also provide easy access to important contact information and frequently asked questions so that our patients are optimally supported though their surgical journey and enhanced recovery program. The wearable app will monitor step count, sleep quality and other measurable metrics so that we can closely follow patients' recovery from surgery. It will also allow us to set individualised patient goals to optimise recovery from surgery.
