24.01.2022 Yesterday morning I was up and on deck at 4.45am presenting at the AAPD virtual conference (American Academy of Pediatric Dentistry) on the 'ARM-ER approach' to using silver fluorides in dental care management plans. I got up at 4am again today to watch a fantastic 3hr presentation on kids dentistry and the modern approaches that some of us prescribe to using silver fluorides and other minimal intervention stratagies. One gem that captured the essence of all who are moving f...orward in this area was shown in the slide below! THIS IS WHY I AM IN THE GAME! Thanks Jeanette MacLean Budd and your co-host Yasmi Crystal for an excellent lecture, for this slide and for your care of the kids.. Whatever their age is!

23.01.2022 Today... And for the next 3 days... I'm attending the world's first symposium on molar hypomineralisation. This is something dear to my 'clinician heart' because silver fluoride has made a HUGE difference to patients who have this condition! Mike Hubbard and his team at the D3Group are sure to have some interesting info on how the world is pulling together to tackle this problem. I've been fortunate to bring along some of the best in the business to help share the knowledge and experience that they have as well and how silver fluorides are benefiting their patients. 17.01.2022 Is there anything better than (yet another) a good news story? Every day in my clinical practice I get to see the results of using silver fluoride to mitigate the effect of dental decay in my patients...many of whom are really little people! (Every day there are more and more clinicians around the country who are seeing the same thing by the way!) Here is another great follow-up case where CSDS silver fluoride was used to help with the caries management in this young fellow. ...It also outlines the importance of using x-rays for early identification AND having something on hand which will stop the decay process IN ITS TRACKS! For this little guy, one application of the 'magic water' was all it took, and he returned for the cleanup and restoration at another visit. The dead decay (I call that the 'burnt toast') was simply cleaned out and the layer underneath was hard and remineralised. The filling was simply placed on this solid base and off this little one went! (The decay in this tooth was once very soft and was heading straight to the nerve of the tooth!) No pain, no needles and no big invasive filling...and one happy camper and thankful family! (And happy to have the news shared in this way too ).The BEST part though is that this tooth had a small filling and the rest of the tooth retained its strength. Over 12 months later, there has been no progress, there is a real 'arrest' line where the decay was stopped in its tracks and, if you look a the top teeth, the decay process was stopped early enough that it did not require any further treatment! Simple, easy, safe and conservative! Check out the pics below. 15.01.2022 What times we are living in! Unprecedented, disruptive, challenging and with SO many changes! Across the world, most of the dental community has been up-ended and restricted as to what is 'acceptable practice'. Here in Australia, we need to operate at 'Level 3' conditions, and this has resulted in many dental clinics closing their doors, having to stand down almost all of their staff, and make decisions that they were never prepared for! I've been a dental clinician for 40 ...years and never have I seen anything like what we are currently experiencing...let alone even heard of 'Level 3'! Within these guidelines, It's not up to clinicians to decide who/what they can treat in the clinic, what needs to wait until we all return to our 'normal' clinical practice or what can/should be treated now under 'emergency care' only. It is a tough call to make and many clinicians are greatly disturbed at having to allow treatment to be left, knowing that they cannot do anything else. (Personally, I think that 'normal' practice will be redefined after the crisis is over.. and I have no idea what that will look like!) HOWEVER, there are a group of patients who I was able to contact this week with good news! I called all my patients who had been pre-treated with silver fluoride (yup, good old 'black diamonds') and reassure them that they were the lucky ones. Yes, they might have had the appointments cancelled and we have no idea when things will return to 'normal', BUT they are in the best position they could be! Silver fluoride has already dealt with the bacteria, has arrested the lesion. and is working on allowing remineralisation to occur and has also resolved any sensitivity. I just reminded them how to take care of things with great cleaning and using fluoride toothpaste to help with the continued remineralisation, and we would see them on the 'other side' of this season! This was reassuring for them and also for me! Silver fluoride and the ARM-ER approach were made for a time such as now. We proved it last year. These are a few pics of a patient who was pre-treated with silver fluoride just once in December and was due to return in late January to have the filling...but then Townsville had 'the floods'. Life was in disarray for thousands of folks and this patient did not find her way back to have the filling done until May. (Not at all unusual at all last year.) During that time, there were no symptoms, the very deep caries (decay) had arrested, the underlying layer of dentine was very remineralised and she had no sensitivity during that time. This was not an unusual story and the outcome could have been VERY different. This is why 's #blackdiamondsrock!

14.01.2022 Well after a fantastic day in Newcastle presenting to clinicians on the 'ins and outs' of silver fluoride applications I thought I would present a story here on how silver fluoride was used recently to help with a rather 'touchy' tooth of my own! (In this case, CSDS was used) There is nothing as strong as your own healthy tooth tissue (every dentist knows this!) which is 'designed' to last the 100+ years that we are now expected to live! However, if the teeth are filled, t...hose fillings need maintenance over the years ...and often replacement. When doing a replacement filling the area filled gets a little larger each time and the tooth consequently becomes weaker. I had a bunch of fillings done when I was 17 (I was an exchange student in the USA and my diet really changed!). The majority of them have actually lasted the distance (thanks to great oral hygiene practices) but one larger one has needed a few repairs and replacements over the years. 17 was a looong time ago! To keep this tooth 'intact' I recently had a beautiful gold crown placed, and not long after the tooth became sensitive around the edges. (I love a bit of long-lasting gold bling!) Despite the quality placement, every time I brushed the tooth it decided to let me know it was 'alive'! Even the changes in kinds of toothpaste I tried did not succeed in letting it settle for any length of time. So into the dental chair I hopped and said 'bring on the good stuff'! And you know what? ONE three minute application of silver fluoride has done the trick...no more sensitivity, no weird feelings and no more 'messages' from that tooth! (I never doubted it!) There are needs and applications for the use of silver fluoride in ALL age groups! So many different situations to use it in...so many areas of need in the adult population too! 11.01.2022 For the past two years. through education and professional development seminars, I have been fortunate to lead clinicians across Australia and New Zealand in the use of silver fluoride materials in their own clinical practice. For some, it was a totally new concept and for others, a welcome re-introduction to something we once used on a regular basis. I have had the joy of presenting some amazing case studies thanks to the fact that my own patients have been so generous in... allowing me to tell their stories of how 'black diamonds' have made significant differences to the way we have managed some really challenging dental situations. It’s was once said that ‘black diamonds’ allow ‘lions to be turned into lambs’! I have to agree! It’s now been three years since I started using silver fluorides again (I first used it in the 1980s) and over that time, I’ve learned a lot more too! I have learned to ASK lots of questions and really LISTEN to the stories that the patients (and their parents) tell me about a problem tooth BEFORE I look at the tooth and take the x-ray! Why? Because when you look first and then ask questions, evaluations are more likely to be based on what you see and not on presenting symptoms! (It’s a dental thing!) That often leads the treatment towards ‘traditional’ problem-solving options which may be very invasive, difficult or even require the tooth to be extracted. Remember, once upon a time, that was me too! However, using silver fluorides to help stop the decay process, remineralise (harden) the affected part of the tooth first, AMAZING results are possible in the modern dental setting! So now we work with the symptoms first and ‘give teeth a chance’! This is just what we did with the little fellow in the video below. Here was a very young fellow with a sizable hole in his tooth. We wanted to save this tooth to keep the space open for his permanent tooth to come through in the correct position but, because of his age, this might have been very tricky (and expensive)! . The results will surprise many dental folksbut I have come to expect it now! A simple, easy, painfree solution for this situation. It has changed the world for this little guy and for so many of my patients and their families - young AND old! Merry Christmas everyone and I look forward to training up more dental folks in 2020! 07.01.2022 I had the absolute pleasure of presenting on silver fluoride at a full day fluoride seminar today at the Zealong Tea Estate 's beautiful conference centre. Thankyou Waikato District Health Board for the opportunity to educate your clinicians and to their leader Rob who appreciates the need! (and for the samples of tea to take home!) My lovely patients who are so generous in allowing me to tell their stories should be proud to be part of this journey too! Here are some pics to show them!

05.01.2022 While contemplating the current 'Covid 19' situation facing the general population and our dental community this morning, I reflected on all the clinical 'modifications' that might be necessary and the impact that we may well face in moderating/managing clinical care protocols to protect ourselves and our clients. I then realised that just over 12 months ago our city (Townsville) was faced with an unprecedented flooding situation that rendered approximately 1000 homes uninhab...itable, destroyed businesses and changed the lives of thousands of locals for months afterwards, and consumed a considerable amount of their time and finances in the aftermath. What I experienced through this period was that clients in a current course of care and who were delayed in seeking continuation of the course of care for up to six months AND who had begun their dental treatment with a pre-treatment of silver fluoride (applied to begin the arrest/remineralisation process) did not experience progression of their dental caries during that time, and we just picked up where we left off when they could come in for treatment! I talk about this in every seminar because it is valuable experience and understanding, and I feel is particularly pertinent in the current situation. So whether your client base is young or old, with the intent of this being a 'public service message', I'm showing you 'Slide 66' and encourage you to ponder this aspect for your clients too. ARM-ER, of course, is the clinical pathway I use to help explain the use of silver fluorides to clients so that we can all be on the same page as far as future treatment planning goes.
