05.01.2022 I refer to an article in the Collegian from 2019 (Collegian 26 (2019) 3339 ) which discusses preventable deaths in hospitals indicating that Most preventable deaths in hospitals are a result of failure to recognise patient deterioration and intervene and that evidence shows that seriously ill ward patients often go unrecognised and receive sub- optimal care resulting in serious adverse events (Collegian 26 (2019) 33-39) It also refers to patients who suffer potentially pr...eventable deaths are almost always in the general wards of hospitals. Nurses play a critical role in identifying deteriorating patients and calling for medical attention in general wards Evidence suggests that ward-based nurses do not always recognise and act upon patient deterioration appropriately. . and the problem of unattended patient deterioration remains Exploring nurses understanding of patient deterioration indicates how poorly patient deterioration is recognised (Lavoie et al., 2014). For the full article and references to the above click on the link below Are there still barriers to MET callsMetropolitan and regional nurses and midwives perspectives? Clinical review criteria and MET calls are vital to safe health care in our hospitals, the issue is are all nurses capable of recognising early warning signs of the deteriorating patient, I think the article answers this question. This is a serious issue for patient safety, a patient goes into hospital for a routine procedure but ends up in ICU for an issue that if it had been recognised early could have been managed appropriately without further deterioration, too often nurses are relying solely on charts to tell them when to act rather than make a clinical assessment decision. just waiting for a BP or pulse to go outside the indicators on a chart is not good clinical assessment, in order to recognise and respond appropriately the nurse must have good clinical assessment skills. The Clinical Assessment course developed by Health Education Collaborative with expertise of several clinicians that facilitate simulation training at major teaching hospitals teaches these essential skills.
