14.01.2022 MEDSTOCK: LAYERS OF THE SKIN Epidermis This is the outer avascular layer of the skin that is made up of epithelial cells. It regenerates every 2-4 weeks, subject to an individuals age and friction forces applied to the skin. All the nutrients are supplied from the dermis below it.... Dermis This is the middle layer of the skin. It is a very vascular layer containing nerves, connective tissue, collagen, elastin and specialized cells such as fibroblasts and mast cells. It is a very important level responsible for the inflammatory processes which occur in response to trauma and infection. Hypodermis The innermost layer is commonly known as the subcutaneous layer. It has a supportive role to the superficial layers. It is made up of adipose tissue, connective tissue and blood vessels. It also has a very important role in storing fats, protecting organs and temperature regulation. Skin Appendages Skin Appendages include hair, nails, sebaceous glands and sweat glands.

04.01.2022 MANAGING SKIN TEARS | STAR ACRONYM Stop Bleeding and Clean - Control bleeding with saline soaked gauze and gentle pressure or Calcium Alginate. - Gently remove any congealed blood from flap.... - Clean with warm water or normal saline to protect granulating tissue. - If wound is dirty, clean with a low strength povidine iodine solution and leave for 3 mins then wash off. Tissue Alignment - Assess % of loss of skin or flap colour and surrounding skin (STAR). - Unfold and smooth out flap completely. - If the flap is torn but still attached, reposition over the wound covering as much as the exposed surface as possible. - If wound has dried out, moisten area with saline soaked gauze for 5-10min prior to flap re-alignment. Assess and Dress - Assess % of loss of skin or flap colour and surrounding skin (STAR). - If a skin flap is present but appears non-viable, debridement may be considered. - Choose an appropriate dressing that suits the stage Skin Tear and wound environment. - Apply a tubular bandage to facilitate haemostasis and for ongoing protection. - Assess factors that will negatively effect wound healing. - If the skin tear is on an oedematous leg assess suitability for compression therapy. - Inspect the surrounding skin and environment for re-injury risk. Review and Re-Dress - If skin flap is pale, dusky or darkened, reassess in 24 to 48 hours to ensure the flap is now viable i.e.. Attached and pink in colour. - If the skin flap is not viable remove the non-viable tissue with sterile scissors. - If the flap is viable review the wound in 5 - 7 days. - Monitor for changes in wound status (redness, pain, or increase in exudate). - If dressing is full of blood/exudate, remove & replace with a new dressing.
