23.01.2022 A negative pressure reflex within the upper airway acts to stabilize the airway and prevent collapse through phasic activation of airway dilator muscles (eg, al...ae nasi and genioglossus) before the onset of diaphragmatic activity. Phasic dilator muscle activity and preactivation delay increases with increasing negative airway pressure or increased central respiratory drive (CRD) to maintain airway patency. In a normal person, the negative pressure reflex that is set up shortly after the sleep onset and after the higher paC02 levels are accepted by the central respiratory generator (CRG). this is driven in part by mechanoreceptors around the exterior nasal valve (nostril) and the airway is maintained via the response to the negative pressure by the CRD by the recruitment of the suprahyoid muscles after genioglossus (tongue) has initially fired. For this homeostatic component to act correctly, the tongue must be in the palate stimulating mechanoreceptors and thermoreceptors and the mouth must be closed. Mouth breathing at night with a nose that does not work results in a night of more disrupted sleep, even if the AHI is lower- because the AHI is not a measure of sleep disruption. Diabetic neuropathies are a heterog...eneous group of disorders that affect different parts of the nervous system, including generalised symmetrical polyneuropathies, multimodal and focal neuropathies, and autonomic neuropathy. Axonal lesions related to nocturnal hypoxemia can further progress this peripheral neuropathic comorbitidty. This pathway may go towards a better understanding of fibromyalgia in some patients. A medical examination to determine the extent of pre diabetes via the insulin resistance blood count method can indicate the potential progression of the disease. Proteinuria, a sign of renal disease, can be flagged in cases of OSAS and hypertension, when pre diabetes is also in the comorbid mix. Diabetes and pre Diabetes can contribute to the onset and severity of central apnoea. A sleep test can flag central apnoea in otherwise non obese patients.
