23.01.2022 A negative pressure reflex within the upper airway acts to stabilize the airway and prevent collapse through phasic activation of airway dilator muscles (eg, al...ae nasi and genioglossus) before the onset of diaphragmatic activity. Phasic dilator muscle activity and preactivation delay increases with increasing negative airway pressure or increased central respiratory drive (CRD) to maintain airway patency. In a normal person, the negative pressure reflex that is set up shortly after the sleep onset and after the higher paC02 levels are accepted by the central respiratory generator (CRG). this is driven in part by mechanoreceptors around the exterior nasal valve (nostril) and the airway is maintained via the response to the negative pressure by the CRD by the recruitment of the suprahyoid muscles after genioglossus (tongue) has initially fired. For this homeostatic component to act correctly, the tongue must be in the palate stimulating mechanoreceptors and thermoreceptors and the mouth must be closed. One can see how those with a dulled dilator muscle response (OSA sufferers) need no extra disability from mouth breathing or more importantly, a lack of tongue posture and nasal breathing. https://www.ncbi.nlm.nih.gov/pubmed/1568988

20.01.2022 So... Lab based sleep studies for people who are otherwise healthy are otherwise expensive. Compare this out of pocket cost to the Aer Healthcare home based sle...ep test with Medicare part funded approval. The hospital takes over $400+. We charge $200.00 and provide a "medical -dental " report. Two reports, 1 on sleep 1 on bruxism potential. It's a no brainer. www.onlinelearningcomau.live Book in through our online appontment system. 20.01.2022 The diagram is a beautiful illustration of the importance of the environment (via central thermoreceptors and peripheral arterial chemoreceptors), sleep hygiene..., the brain (via motor neurons) in the throat and tongue, and the correct route of breathing (via the nose for proper peripheral and pulmonary receptor feedback), to homeostasis and quiet nocturnal breathing. We need to look at our patients as an organism multi factorially effected by more than just mouth breathing or snoring. As provided in: Sleep and Respiratory Physiology in Adults Mudiaga Sowho, MD, MPHa, Jason Amatoury, MBiomedE, PhDb,c, Jason P. Kirkness, PhDa, Susheel P. 17.01.2022 https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4319459/ Sleep, reduce calories and exercise if you want to live longer, and now you also can have the benefits of ...anti-ageing. Well-characterized actions of NO include the stimulation of vasodilation, vessel formation (Ziche et al., 1994), and mitochondrial biogenesis (Nisoli et al., 2003) as well as inhibition of smooth muscle cell proliferation, leukocyte adhesion, platelet aggregation, and mitochondrial respiration (Qian and Fulton, 2013). Thus, thanks to its antihypertensive, antithrombotic and anti-atherosclerotic properties, eNOS-derived NO represents a key molecule in vasoprotection (Förstermann and Li, 2011). Following the original study demonstrating that eNOS-derived NO promotes mitochondrial biogenesis (Nisoli et al., 2003), the eNOS-NO-PGC-1 pathway has been shown to be involved in several health-promoting actions. It has been found to be downregulated in adipose tissue of obese mice and rats (Valerio et al., 2006). Consistently, eNOS overexpression exerts anti-obesogenic effects in high fat-fed mice through improved mitochondrial biogenesis and activity in adipose tissue (Sansbury et al., 2012). The eNOS-NO-PGC-1 pathway is also implicated in anti-ageing effects. Besides its activation in calorie-restricted mice (Nisoli et al., 2005) it has an essential role in healthspan extension observed when mice diet is supplemented with a balanced formula of branched-chain amino acids (BCAAs) and other essential amino acids (EAAs) (D'Antona et al., 2010).
16.01.2022 Aer Healthcare looks at sleep studies with a trained eye. Arousal state is a marker that suggests more to our scientists and physician. You get to help your patient to avoid co-morbidity when we help you with sleep testing. www.onlinelearningcomau.live Recurrent nocturnal hypoxaemia and frequent arousals from sleep fragmentation may cause the HPA axis to lose the ability to respond to chronic recurring stress. Nocturnal hypercortisolism accompanied by low morning cortisol lev...el has been found in severe OSA subjects, who had higher insulin resistance when compared to controls. Systemic inflammation, surrogated by high-sensitivity C-reactive protein, fibrinogen, uric acid, and oxidative stress by the expression of inflammatory cytokines, are found to be high in studies of subjects with OSA. The interaction between sleep and the HPA axis is complex and bidirectional. HPA hyperactivity and decreased sleep duration/quality seem to be tightly linked in a vicious circle and could play an essential causative role in the pathogenesis of metabolic and mood disorders. Hypercortisolism, sleep disturbances, metabolic and psychiatric impairments are standard features of several clinical syndromes, such as insomnia, depression, Cushings syndrome, and SDB. The correct understanding of the interaction between sleep physiology and HPA activity appears to be a crucial prerequisite to elucidate the physiopathology of these syndromes better and may lead to new forms of prevention and treatment. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2902103/

15.01.2022 MOUTH POSTURE CAN BE AFFECTED ADVERSELY BY AN ORAL APPLIANCE, SO HOW DO YOU DEAL WITH THAT?? AER HEALTHCARE HAS THE ANSWER.
15.01.2022 More Aer Dorsal devices going out
15.01.2022 A blocked nose at night will cause more arousals in sleep than a patent nose. Mouth breathing at night with a nose that does not work results in a night of more disrupted sleep, even if the AHI is lower- because the AHI is not a measure of sleep disruption. Oral appliances that promote and encourage mouth breathing need to be assessed on the outcome of fewer arousals during sleep. Arousal state can adversely affect cardiovascular risk in some OSA sufferers. Oral appliance choice must never include mouth breathing. https://www.ncbi.nlm.nih.gov/pubmed/3356654

13.01.2022 This is a direct quote from the attached article. "New variations in customized OAm designs may enhance effectiveness in the future. A recent cohort study tested the addition of tongue protrusion, via an anterior tongue bulb on an OAm device and showed greater AHI reduction compared to mandibular advancement alone.37 Simultaneous advancement of both the tongue and mandible, for example, may prove to increase therapeutic effect." 12.01.2022 Why mouth breathing at night is a problem. When the mouth is recruited during night time quiet breathing, the regimen of quiet breathing is altered. View quiet breathing here. https://www.dropbox.com/s/40ta26nkpn/quiet%20breathing.mp4 The movement of the ribs during quiet breathing is described below. 11.01.2022 Don't make an appliance that forces the patient to breathe through the mouth. More than this, most oral appliances that are made for OSA DO FORCE oro-nasal bre...athing. WHY? The face of most OSA patients with skeletal deficiencies that cause the smaller airway does not support lip seal when the oral appliance is in the mouth. Oronasal breathing then can cause excess swallowing that then distracts the tongue from the palate. AER HEALTHCARE has the answer with the Aer Dorsal and Aer Shield. Look at www.snoringtreatment.com.au to find out more. We help your patients breathe better more often. Swallowing frequency during sleep can increase with increasing OSA severity in most OSA patients. These events are predominately associated with respiratory event-related arousals and are more frequent when preceded by oronasal breathing. The observed swallowing under high ventilatory needs may compromise the maintenance of the pharynx as a conduit for airflow in OSA patients. https://www.ncbi.nlm.nih.gov/pubmed/25056664

08.01.2022 Aer Healthcare can help you with sleep studies so that you can help your patients to identify if they are at risk of the cardio vascular pulse transit times tha...t are only witnessed at night. These PTT events can be as a result of absolute and relative desaturations of oxygen and this is associated with sleep disruption as much as it can be with airway events. The cortical arousal index is a powerful tool thay gives you the informarion you are seeking more than AHI. The cardio vascular risk index is not interdependent to AHI, as AHI does not measure SpO2, and more specifically paCO2. Aer Healthcare does part Medicare funded home sleep studies without the hassle. We can organise the GP referral for your patients when they come to see us. All in one appointment and one place. At the present time you patients can see us in two convenient locations and two more depending on where they work. Call 80904358. The act of notifying the patients of their risk by action trumps information any day. Thanks Chris Kelly See more
08.01.2022 It is important for women to have yearly blood tests. The pre-diabetic condition in women can cause peripheral neuropathies. Diabetic neuropathies are a heterog...eneous group of disorders that affect different parts of the nervous system, including generalised symmetrical polyneuropathies, multimodal and focal neuropathies, and autonomic neuropathy. Axonal lesions related to nocturnal hypoxemia can further progress this peripheral neuropathic comorbitidty. This pathway may go towards a better understanding of fibromyalgia in some patients. A medical examination to determine the extent of pre diabetes via the insulin resistance blood count method can indicate the potential progression of the disease. Proteinuria, a sign of renal disease, can be flagged in cases of OSAS and hypertension, when pre diabetes is also in the comorbid mix. Diabetes and pre Diabetes can contribute to the onset and severity of central apnoea. A sleep test can flag central apnoea in otherwise non obese patients. Call 0280904358 and book in for a preventative sleep test exam if you have any concerns about you nerve pain, sugar tolerance, or unexplained day time tiredness.

04.01.2022 Nasal breathing is the only way to have nitric oxide delivered to the lungs. The local deposits assist with vasodilation and the balance of ventilation and perfusion match. Oral appliances that promote mouth breathing in the absence of patent nasal breathing destroy the potential for this system to work effectively. https://www.ncbi.nlm.nih.gov/pubmed/18951492
03.01.2022 Happy Easter to all of my dental colleagues. The Easter eggs that have been devoured this weekend will be potentially linked to the bread and butter trade of the field. So it is the same in sleep medicine. The effects of bad sleep will not be felt in the most serious outcomes of co-morbidity for years after these issues have been suffered. Reading sleep tests can feel like reading tea leaves to the uninitiated. Here is a great little article I found that could be the start... of a beautiful relationship. If you want to get the most out of the sincere effort you put into trying to work through the field of sleep medicine, my online "on demand" webinars are just your ticket to success. You can send me your sleep reports (with the permission of the patient) and I will analyse them during the webinar. All privacy assured. Contact me at www.onlinelearningcomau.live for more information. I hope to hear from you soon. Christopher Kelly https://doi.org/10.3402/jchimp.v4.24983
