25.01.2022 21 March 2020 COVID-19 update for Alexandra Hills Chiropractic Centre Id like to give you an update about our Clinic with regards to the Corona virus.... I am deeply concerned with the increase in confirmed cases throughout South East Queensland. For the past few weeks Libby and I have been utilizing extra cleaning measures throughout the Clinic including in my treatment room, and at the moment we are able to continue seeing patients. However given the recent jump in cases, we will need to make some changes. From Monday 23rd March the following will be in place: 1. The number of available appointments will be reduced to minimize patient crossover and reduce time spent in the waiting room 2. Waiting room chairs will be reduced to ensure that patients are placed the recommended 1.5m apart For those patients attending the Clinic in the upcoming week, please follow the Government recommendations which include (*): 1. Clean your hands regularly with soap and water or alcohol-based hand rubs. 2. Cover your nose and mouth with a tissue or bent elbow when coughing or sneezing. 3. Avoid touching your face, nose and mouth. And avoid shaking hands. 4. Stay home if you are unwell. 5. Practice social distancing, which includes staying 1.5m away from others as much as you can. 18.01.2022 Chronic pain & tissue damage seen on clinical imaging are poorly correlated Arthritis on an X-ray can occur with no pain A person can have pain & a n...ormal X-ray Yet imaging findings, such as meniscal tears, rotator cuff tears and degenerative discs are often interpreted as causes of pain, triggering medical and surgical interventions. Treatments to remove/fix the tissue or targeting tissue regeneration, such as arthroscopy, stem cells or platelet rich plasma, are often no more effective in reducing symptoms than sham treatments Knee arthritis seen on an MRI (cartilage defects, meniscal tears and osteophytes) are present in up to 60% of those aged >60 years) Most of these findings should be viewed like wrinkles on the skin a normal part of aging that dont typically require fixing. Disabling or severe pain are the primary reason people seek treatment. Typically Pain With Movement leads to interventions aimed at tissue offloading to prevent injury or degeneration. Counterintuitively, offloading tissues in the context of musculoskeletal pain may be more problematic than beneficial for tissue health. 1/3 of children who develop knee pain are sedentary Exercise can have acute sensory effects improving pain by reducing a sensitised nervous system through a reduction in cortical inhibition Pain can persist with reduced or modified tissue loads. Even when persistent pain displays mechanical behaviour, this could represent the early or unnecessary warnings from a sensitised nervous system, as much or more than any threat to tissues. We need to consider more than just mechanical effects of increasing or decreasing tissue load, because positive adaptations with gradual progression of load can also influence sensitivity & behaviour Pain should not typically be a barrier for tissue and joint loading, but it is an important clinical consideration when planning how much load will provoke a sensitised nervous system. For patients with chronic knee and hip pain, a program of progressive loading (strengthening) resulted in sensory adaptations leading to a considerable decrease in exercise-induced pain flares Even in acute injury settings, where traditional first-line injury management was rest (the R in RICE), guidelines have now shifted and advocate protect and optimally load (the POL in POLICE) There is need for vigilance but not fear of gradual progressive loading interventions for people with persistent musculoskeletal pain. 16.01.2022 Whilst the current clinical guidelines recommend that imaging should be limited only in cases of suspected serious underlying pathology or trauma, the use of X-...rays within the chiropractic profession remains a hugely contentious and varied issue. The rates of X-ray use vary from 8-84% of chiropractic patients being X-rayed as part of a chiropractic consultation. Justification for imaging also vary widely - ruling out pathology, assessing biomechanical changes, determining treatment options, reassuring patients, medicolegal reasons - and these are balanced against the risks. Clinical guidelines "...recommend that diagnostic imaging is used only when there is clinical suspicion of serious pathology or when imaging findings are likely to lead to a beneficial change in management, improvement in patient outcome, or decrease in patient harm..." In summary: "...Current evidence supports the use of spinal X-rays only in the diagnosis of trauma and spondyloarthropathy, and in the assessment of progressive spinal structural deformities such as adolescent idiopathic scoliosis. MRI is indicated to diagnose serious pathology such as cancer or infection, and to assess the need for surgical management in radiculopathy and spinal stenosis. Strong evidence demonstrates risks of imaging such as excessive radiation exposure, overdiagnosis, subsequent low-value investigation and treatment procedures, and increased costs. In most cases the potential benefits from routine imaging, including spinal X-rays, do not outweigh the potential harms. The use of spinal X-rays should not be routinely performed in chiropractic practice, and should be guided by clinical guidelines and clinician judgement..." An interesting, and informative read.
