24.01.2022 Patient requested a home Podiatry visit whilst suffering from an aggressive gout flare up. The long term history of gout resulted in the Patient having excessive midfoot erosions and collapse of the right MLA. - Patient unable to weight-bear disclosed that the pain was progressing and the MLA collapse has began to to occur bilaterally. - Our external client incorporated a medial flange into the orthotic prescription to offer the patient an enhanced level of control and support without compromising comfort.

23.01.2022 Medially deviated subtalar joint axis - more than just a flat foot. . The subtalar joint is a functional joint that acts as a mechanical link between the foot and lower extremity. . Medially deviated subtalar joint axis is a term that was first described by Kevin Kirby in 1987.... . It is a term used to describe a variation in spatial location of the subtalar joint (STJ) axis. Pronation and supination moments are produced across the subtalar joint axis by the effects of ground reaction force and muscular contractile force. . A foot with a medially deviated STJ axis causes a net increase in pronation moment, whereas a foot with a laterally deviated STJ axis will result in a net increase in supination moment. This concept is described as STJ rotational equilibrium. . During weight bearing examination, feet with a medially deviated STJ axis present with a convex shape to the medial aspects of the rearfoot and mid foot, excessive STJ pronation, dorsiflexed 1st ray and forefoot abduction. . Common conditions associated with a medially deviated STJ are tibialis posterior tendon pathology, plantar fascia pathology and sinus tarsi syndrome. . Locating the STJ axis via palpation is the key to success with orthotic therapy. Prescription should be based on unloading effected structures. Reference: Dr. Kevin Kirby.

Tissue stress theory refers to the kinetic analysis of pathological stresses that can occur within the structural components of the foot and lower extremity. . Three goals of orthotic prescription when using tissue stress theory are:... Reducing the pathological loading forces on the injured structural components of the foot and lower limb.
