23.01.2022 Published earlier this year in the BMJ Military Health Journal is a discussion on the basic principles of management of torso injuries for the immediate military or humanitarian first responder. In Iraq and Afghanistan injuries to the thorax and abdomen accounted for approximately one quarter of all injuries seen by the UK Military, with 55% of those with abdominal wounds succumbing to their injuries and 14% of those with thoracic injuries. Comparatively Medecins Sans Fronti...eres has reported unintentional trauma makes up 10-42% of their surgical cases and violence (such as GSW and blast) make up between 7-10% in a humanitarian context. It is therefore reasonable to suggest that all first responders working in both military and civilian systems in a humanitarian, austere and combat setting have an adequate understanding and training to respond and manage traumatic injuries to the torso.

19.01.2022 As it’s been said time and time again 2020 has been uniquely challenging for many, this includes our police. Communities have continued to call upon their police through bushfires, floods and pandemic in addition to their normal duties and they have continued deliver their service. . Each year on 29 September, we pause to remember fallen police employees who have made the ultimate sacrifice. . Victoria Police are now live on YouTube for a virtual National Police Remembrance... Day Ceremony as we remember those who have dedicated their lives to community safety. . The service will remember and honour the lives the 23 Victoria Police officers, Protective Services Officers, Police Custody Officers, Victorian Public Service staff and Police Chaplains who died between 29 September 2019 and 29 September 2020. . This includes Leading Senior Constable Lynette Taylor, Senior Constable Kevin King, Constable Josh Prestney and Constable Glen Humphris who were tragically killed in the line of duty on the Eastern Freeway in Kew on 22 April 2020. . Their dedicated service to the community is respected, and they will never be forgotten. . #NPRD2020 See more

18.01.2022 A unique characteristic of the first responder, emergency service and military community is the camaraderie that goes with it. Recently, one of our brothers has been dealt some devastating news. Subsequently, Sean will undergo surgery to remove his tongue with radiation therapy to foll...ow. Sean & his wife Jess are expecting their first child in October making this news even more difficult to fathom. Sean, Jess and their family remain positive, strong and ready to take on this massive challenge that lay ahead but any support is appreciated. A number of ATMA members and committee have had the privilege to work closely with Sean and can attest to his truly admirable character. Sean, our thoughts are with you and your family mate and we will be there to support you along the way.

09.01.2022 Employment in the emergency services and the military is physically demanding by its nature. The accumulation of wear and tear on the body or an injury (or both) can amongst other things result in ongoing pain. The Australian Tactical Medicine Association has been approached by Christine Sutherland, a clinical researcher, who is conducting an expanded third clinical trial on chronic pain reduction through Sensory Disruption of Reconsolidation (SDR) therapy. Christine is seek...ing people who are suffering from chronic pain to take part in the trial to build on the evidence from previous trials with the goal of developing a novel technique to help those suffering from chronic pain. ‘The aim of this study is to investigate the efficacy or otherwise of a novel treatment method that has as its aim the switching off of pain signalling amongst a specific group of chronic pain patients, in comparison with treatment as usual. There is currently a lack of any existing treatment which benefits the majority of chronic pain patients, and it is hoped that this novel approach may provide an inexpensive, convenient, and easily deliverable treatment with a high efficacy rate’ Due to COVID-19, the trial will now be conducted remotely with support from Christine and her team at Lifeworks Group based out of Perth. If you would like to be involved in the program or would like to know more, please visit the SDT trial webpage (https://www.sdrtherapy.org/clinical2020/) to see a short video discussing SDR therapy, details of the study, inclusion and exclusion criteria and trial applications. Christine can be contacted directly at [email protected] ATMA team

05.01.2022 In case you needed anymore reason to withhold the #pastawater @max.dodge83 Hyperchloremia is an excess amount of the electrolyte Chloride (Cl) in the blood. Normal values are between 98-108 mmol/L. 0.9% Sodium Chloride has 154 mmol/L of chlorine in the form of NaCl (sodium chloride), more than physiologically normal. So what happens when you administer too much chlorine? . Most of the time, in healthy well-perfused patients with working kidneys, nothing. However, if your p...atients have certain underlying medical conditions or are in a state of hypoperfusion (shock) then you may not want to use Sodium Chloride for resuscitation. Let's dig in. . ANION GAP . The difference between your most abundant positive ions (Sodium, Potassium) and your most abundant negative ions (Chlorine, Bicarbonate) is called your serum anion gap. This is generally around zero when your electrolytes are in balance. (The actual number depends of the lab doing the calculation, but is usually somewhere between 4-12). When the amount of negative ions (anions) increases, this number increases and you develop what is called an "Anion Gap Acidosis". You will generally find this measurement as part of a basic metabolic panel (BMP). . The reason a net negative charge in your body leads to acidosis involves a little biochemistry and the law of electrochemical neutrality. This says that if you measure all cations (positive) and anions (negative) the result must be zero. If it's not zero, that means something besides what was measured is balancing the charge. In the case of a net negative charge, we have plenty of positively charged Hydrogen (among other things) to equalize the charge and make acid. . RENAL PERFUSION . An excess of chlorine in your blood causes decreased blood flow to your kidneys, reducing your glomerular filtration rate (GFR). GFR is the rate at which your kidneys filter your blood. Your kidneys specifically spare chlorine to maintain your body's internal neutral charge. The amount of chlorine present in the blood is used to determine how much fluid should be spared, and high levels of chlorine indicate to the kidneys that too much fluid is being lost. See more

02.01.2022 As many would be aware a devastating blast has occurred at the port of Beirut in Lebanon. Initial reports suggest that the death toll is currently at 60 which includes 1 Australian, and over 3,000 people were injured in the blast . Lebanese emergency services and hospitals were quickly overwhelmed by the influx of patients with many likely suffering complex multi systems trauma associated with blast injuries. We extend our immediate thoughts to those injured and affected by... these horrific events and all those first responders and health professionals that have been managing the effects. As investigations commence these events serve as an unfortunate and timely reminder that in a world that has been so COVID-19 focused that significant mass casualty events can continue to occur throughout the pandemic placing many strained health systems under additional stress, making preparedness, medical and disaster planning more important then ever.
