24.01.2022 About us Axcess Disability has been providing support to people living with a disability or a mental health condition for more than 20 years. Our professional and friendly staff are trained in many areas of disability work and are here to provide you with individualised ongoing support.... Since the NDIS rolled out in 2013, Axcess has been working closely with people who live with a disability to support them to utilise their funding in order to link to supports that are in line with their goals. We provide individual support to ensure that our clients are fully prepared for their NDIS planning meeting. We take time to fully explain your plan and assist you to select and engage with your chosen service providers and support you to coordinate all of your supports. About the role We are currently seeking an experienced casual Case Manager/Coordinator to join our dedicated team in the Hawkesbury. In the role, you will strengthen our client’s ability to connect with, and maintain supports outlined within their NDIS plan. You will also be developing professional relationships with participants, families, and other service providers. Ideally, the desired person will have experience in case management and have a sound knowledge of the NDIS and local services. In this role you will Establish appropriate networks Establish relationships with clients and families (plan nominees) to understand support needs Consult and collaborate with key internal and external stakeholders Maintain appropriate records and evidence of NDIS plan implementation Work to funding allocations Build capacity and implement supports in a participants NDIS plan Report back to the NDIA on the outcome and goals as required About you Have excellent interpersonal and customer service skills The ability to oversee and coordinate projects in the context of competing demands To be excited about the opportunity to make a positive difference through capacity building Applicants must have experience in NDIS Support Coordination or NDIS Planning Have a demonstrated understanding of the NDIS and experience managing funded plans Ability to manage and deliver to multiple deadlines Demonstrated experience in interpreting and implementing NDIS plans Previous, sound experience working within the disability industry NDIS Support Coordination experience and an understanding of NDIS Line items, supports and guidelines. Necessary: Minimum 3 year’s experience Tertiary qualifications in a relevant field Current driver's licence Working with children check or willingness to obtain National Police clearance or willingness to obtain First Aid Certificate or willingness to obtain

21.01.2022 Axcess had the pleasure of supporting Marcia at her annual NDIS review meeting last week. Marcia has been a participant of Axcess since 2017 Marcy has worked closely with her support coordinators at Axcess over past two years, Since "Marcy" (as she likes to be called) has been supported by Axcess she has successfully purchased a new home in a retirement village this is much more suitable for her current needs. This has been a long journey for Marcy working closely with a OT... and other support services to allow Marcia to maintain her independence and live as independently as possible in her new home. Congratulations Marcia and thank you for allowing Axcess to assist you in this journey We look forward to assisting you long into the future

08.01.2022 Well its that time of the year again where we all unwind from the busy year we have had at Axcess Disability head office. We would like to say a BIG thank you to all of our Staff, Clients and there familys for this past year of support, we could not have dont it with out you all. This year at Axcess Disability we have successfully:... - Moved clients from Group homes to independent living -Moved clients from Hospital who thought there would be no place for them to call home into Group homes where they now have friends and extended families. - Supported clients to link to Therapy services to be able to Walk again, be assessed and approved for electric wheel chair to be able access the community, Attend holidays they have been dreaming of for years, Achieve life goals, Attend main stream sporting competitions, Overcome addiction, Learn to speak and so much more. We would also like to extend a huge thank you to the external services that we have worked closely with throughout the year to be able to achieve these amazing goals. From our family to yours we wish you a Merry Christmas and a safe New year.
