23.01.2022 Weight Stigma Awareness Week 2020 Truth #5 from 9 Truths about Eating Disorders: Weight and Weight Stigma (AED, 2019) "All people, irrespective of their weigh...t, deserve equitable treatment in healthcare settings and society. Weight bias and weight-based discrimination are never acceptable." Healthcare clinicians are exposed to the same societal prejudices against weight and body size, arguably more in training, and need to actively create awareness of their own biases alongside the dominant messages of the weight centric western medical system. Weight stigma can show up in the therapeutic relationship between health practitioner and client in subtle ways. Practitioners may not be intending harm, but need to be aware that there are many ways this may happen in unintentional ways. For example, adjusting meals plans for clients in different sized bodies so that clients in larger bodies are prescribed less than smaller clients. Or setting different weight restoration goals with a young person with anorexia nervosa compared to someone with atypical anorexia nervosa. it is ...likely that therapists specializing in the treatment of eating disorders will be treating many individuals with a history of weight based victimization. As a result weight bias inadvertently suggested by these therapists could be especially damaging for these patients and could interfere with the treatment progress. (Puhl et al, 2014). In Puhl et al’s paper investigating weight bias in professionals specializing in eating disorders 56% had observed other professionals in their field making negative comments about o*ese patients. 42% believed practitioners who treat eating disorders often have negative stereotypes about o*ese patients. In this research Mean score on the Fat Phobia scale slightly lower than mean scores on this measure reported among health professionals in other disciplines but still indicative of the presence of negative stereotyping towards o*ese persons. How does weight stigma intersect with body privilege? We all have privileges, which essentially are socially constructed advantages or benefits, which make the way we move through the world easier than others. It can certainly be very uncomfortable to accept and acknowledge our privileges particularly if we have experienced significant difficulty in life. We don't need to justify or apologise for privilege rather be aware of how it may impact on our clients, peers and the broader community. As noted above in the quote from Cora Harrington, if you have never had people insist that you need to be smaller, you have body privilege. You can buy clothes from most shops. You can buy a variety of foods without judgement and you can access healthcare without a side of weight loss advice. You don't have to identify as being in a thin body to experience advantage in life from body privilege. Having thin privilege doesn’t mean that you’ve never had any body-image issues, or that you’ve never struggled with disordered eating, or that you’ve never been bullied or shamed by individual assholes for your size. You can have thin privilege and also hate your body. (Christy Harrison) These can feel like difficult ideas to sit with and may lead to feeling uncomfortable and perhaps even defensive. We invite you to notice any reactions or responses, and take an opportunity to learn more and reflect. Once we know better we can do better and help to dismantle these long held, inaccurate and harmful beliefs about bodies.
