25.01.2022 TOE WALKERS A 2018 study of 1,401 Swedish 5.5 year old children found that 63 of them (4.5%) had been toe walkers. 26 of them were still toe-walking at 5.5 years of age. The study set out to follow the natural progression of this common paediatric condition. At 8 years of age, 6 of 26 children had ceased toe-walking, and by the age of 10 years, 50 (79%) of the original 63 patients had spontaneously ceased toe-walking. One subgroup of children displayed early contracture of t...he ankle which explained their practice of toe-walking. Four of the children who still toe-walked at the age of 10 years demonstrated some neurodevelopmental comorbidity. 24.01.2022 RESULT OF KIDS CHIRO INQUIRY The inquiry received 21,824 submissions, the largest of any public stakeholder engagement. Of those, 99.7% reported a positive experience with chiropractic care of their children. 98% said that chiropractic care helped their child, indicating improvement after treatment. As expected there was a low level of chiropractic research into areas of health care for children that parents acknowledged improvement. The Australian Chiropractors Association ...has openly acknowledged this lack of research. However, it is this anecdotal experience through word-of-mouth that has assisted many Australian children. Fortunately the inquiry has recommended allocation of funding for further research that may further clarify the role chiropractors have to play with paediatric health care. As for the impetus that initiated this inquiry, the safety of paediatric chiropractic care, the inquiry revealed what chiropractors have always purported. In Australia, there has never been a serious adverse event reported in the literature or in insurance claims, involving the injury of a child by a chiropractor. This is despite the fact that there have been an estimated 12-15 million visits by children to Australian chiropractors in the last 20 years. No statistics of harm exist because chiropractors modify their techniques to suit the age and developmental stage of the patient across their lifespan. The inquiry highlighted overwhelming support by parents and their great appreciation of chiropractic care for their children. The inquiry clearly determined the level of safety by Australian chiropractors providing paediatric care is commendable and extremely high. Intra-articular corticosteroid (IACS) injections are frequently performed to treat OA and other joint-related pain syndromes; however, there is conflicting evidence on their potential benefit. There is a lack of prospective and large retrospective studies evaluating potential joint findings, including... increased risk for accelerated OA progression or adverse joint events, after treatment with IACS injection. Four main adverse joint findings have been structurally observed in patients after IACS injections: accelerated OA progression, subchondral insufficiency fracture, complications of osteonecrosis, and rapid joint destruction, including bone loss. Physicians, including radiologists, should be familiar with imaging findings and patient characteristics that may help them identify potential joints at risk for such events. The purpose of this report is to review the existing literature, describe observed adverse joint events after IACS injections, and provide an outlook on how this may affect clinical practice.
