However I think I get an even greater satisfaction when I have the opportunity of removing a prosthesis to reveal this. 19.01.2022 2021 Postgraduate Certificate and Diploma programs in Dental Implantology offered by Cambridge Academy of Dental Implantology in delivery partnership with The Implant Institute, are commencing in *September 2020*. This is a unique opportunity for comprehensive didactic learning in implants. Year 1 (PGCert) consists of two terms supervised online theory (from Sept 2020), 1 Practical Skills Week (July 2021), and supervised clinical casework (from July 2021 onwards), culminating in a PGCert or PGDip (Dent Imp) UK Award. We have already received solid enrolment numbers for the 2021 program, often from recommendations of our amazing and engaged current student cohort. 19.01.2022 Building Bone. Many ways to skin this cat. Sometimes its hard to choose the best regeneration or grafting method for a site, but with different tools at our disposal, we make a choice that we feel will best work in our hands for the particular cases unique anatomy. Here, autogenous bone block from the external oblique ridge (my last post) was used to augment alveolar width and height at this mandibular first molar site. Autogenous particulate was harvested w...ith a bone scraper from the block cortex to supplement lingually, in combination with xenograft/sticky bone (APRF). A Jason porcine pericardium membrane was held with Meisinger pins buccally and a stabilising suture lingually, to secure the site. Tension free closure was achieved with vertical mattress sutures at the papillae and horizontal mattress suture across the site. This patient is a medical doctor with important international pandemic responsibilities, and will be undergoing his implant placement back home. So we had to be on our A game! Things to look out for in this case: A high mental nerve position; thin biotype; lingual and buccal flap advancement; distal vertical recession/bone loss correction at the second premolar. 19.01.2022 Top shelf implant! Following up from a recent action shot from a mandibular full arch immediate implant procedure, I didnt want the maxilla to feel left out - from the same case. In she turns, aiming for high primary torque, prosthetic alignment, healthy wide peri-implant tissue, and arch spread. Like the opposing site in this All-on-4 (5) case, alveoplasty was performed with a canine in situ, to assist with angulation for prosthetic emergence of the anterior implants as cl...ose to the cingulum of the proposed restoration as possible. A lot has to go right if immediate loading is the aim, but with the appropriate protocols in planning, surgery, implant osteotomy technique and design, and case appropriate prosthetic considerations, it can be achieved successfully in most terminal dentition cases, with no compromise to outcomes, but in most cases with benefits compared to delayed loading. 12.01.2022 Full mouth All On X implant reconstruction in zirconia on a titanium framework. The Story is an arduous one: Initially, this patient had a terminally carious dentition replaced with maxillary All-on-4 implants with textbook layout, with bone augmentation to the right anterior maxilla. There was catastrophic failure of the right upper implants and graft, which was devastating because it had been meticulously done. Patient admitted to being non-compliant with post op i...nstructions - in particular, heavy smoking, sucking hard on the graft area, and a hard diet. He committed to following instructions the second time, understanding he was fast running out of options. I wanted to avoid further bone grafting for this patient at all cost to minimise his risk. After first repairing the soft tissue of the anterior segment with a connective tissue graft, we salvaged the case with alveoplasty, deeper implant placement anteriorly and a posterior tubero-pterygoid position implant. This allowed us to temporarily modify his acrylic hybrid bridge to the new implants, and after osseointegration, upgrade to zirconia. Every case has its story. As long as we achieve the final outcome the patient seeks, its a happy ending! 05.01.2022 Clean, polish, punch and stitch! Here I used a gingival graft from the maxillary tuberosity as part of a successful implant rescue effort. Partially epithelialised, partially de-epithelialised, punched and secured for the task of creating healthy tissue following implantoplasty... The story: A few years after full mouth implant restoration, this patient presented with severe peri-implantitis at this lower canine site, in an otherwise healthy mouth of implants (maxilla ...and mandible full arches). This site was always more vulnerable due to thin biotype and lack of keratinised tissue, and after the patient had not attended for a few years, he presented with severe circumferential peri-implantitis at this site. Regeneration was deemed impossible considering the anatomy of the defect and the surrounding tissue. Replacement was not feasible due to severe and unusual anatomical restrictions. The apical half of the implant remained in healthy dense bone. So I aimed to establish tissue health and preserve the implant, as it was a crucial abutment for the full arch restoration. As it turned out, luck was on our side, tissue health was established, and the implant has continued years of service since. These days we have a much better understanding of optimising soft tissue around implants pro-actively. Today in my practice, this site would have been grafted at the outset, despite patient preferences to avoid further procedure! Locator abutments for denture stabilisation with dental implants have been a staple in my implant practice for almost 12 years. Reliable, cost-effective and versatile - specially for patients who are unsuitable or not financial for fixed restorations. I have generally used 2.4IH connections with multiple implant brands for this purpose, allowi...ng me to keep a stock of Zest Locators for that connection at various collar heights. Having a decent stock allows the flexibility to try different sizes when needed, so there is optimal selection for every patient. As with all implants (but even more so from my observations than in standard implants), good keratinised tissue and medium-thick biotype goes a long way to ensure long term hygiene and health. 03.01.2022 1PART A (Immediate Result) Creative plan, yes we can! The outcome of yesterdays virtual planning post. Post op OPG (immediate, same week as surgery) and 4 month review intra-oral photograph of the immediate prostheses. Noting the spread of implants (trans-sinus nasale with sinus lifts, pterygoid and short anterior), healed altered ridge anatomy and flat intaglio surface. I chose a compressing tissue fit on the maxillary and open gap on the mandibular prosthesis ...for optimal hygiene. In my last post I opened a discussion on potential treatment solutions for an especially challenging edentulous maxilla. Some great treatment proposals and ideas were received. Some colleagues had great ideas for grafting solutions, zygomatic solutions and overdenture solutions. Most felt that a staged approach is required, one way or another.
