22.01.2022 Is anyone else confused by all the complicated names you read on the back of probiotics? Let me clear a few things up. Probiotics are named by a 3 level system.... For example, Lactobacillus acidophilus NCFB 178, is derived from: 1) genus Lactobacillus 2) species Acidophilus 3) strain NCFB 1748 .... WHY IS THIS IMPORTANT? There are several thousand bacterial strains under the lactobacillus genus (above Ive named just one). Just like humans, each strain is unique with different skill sets. This is why we cant broadly generalise benefits of probiotics- each product contains different strains capable of different things! .... Dont settle for a take a probiotic recommendation. Ask for the exact type, and which study it was used in, if they cant give you one, Id second guess the advice. Remember if youre generally healthy, have a varied diet (including feremented foods) the total body of evidence suggests, you dont need to take a probiotic capsule. That being said on a case-by-case basis I do recommend some of my patients take specific ones. .... For the majority. _____ "Weight loss following lifestyle intervention is maximal at 6-12 months. Regardless of the degree of initial weight loss, most weight is regained within a 2-year period and by 5 years the majority of people are at their pre-intervention body weight" _____ Those who do not regain weight are statistical outliers, even if they do publish pretty books and end up in front of our eyes and ears ALL THE DAMN TIME, and contribute to this misconception that lasting weight loss is available if people would only just try hard enough. 10.01.2022 Here is an interesting opinion piece by an American Pediatrician. It echos the sentiments of the Non-Diet Approach which I embrace in my practice. We shouldn't label foods as good or bad or restrict. This creates an unhealthy relationship with food which can lead to diet cycling and weight gain; impacting on our emotional health and body image. We should aim to lead a healthy, active life and trust our internal hunger and satiety cues. Our bodies are amazing and we need to work at being kinder to ourselves and try not obsess over calories and food. 08.01.2022 Fibre: Are you getting enough of the right types? The CRISO recently announced that in Australia we have a "fibre paradox". We are eating more fibre than we did 20 years ago and yet we our rates of bowel cancer are largely unchanged. The CSIRO believes the answer lies in the types of fibre we are eating. It seems us Aussies are good at eating roughage (insoluble fibre) to keep our bowels regular, but we don't include enough soluble fibre (slowes digestion and reduces absorpt...ion of cholestetol) and resistance starch (feeds our gut bacteria). To get the right balance of fibre types, the key is to eat a wide variety of wholegrain cereals, legumes (lentils and beans), and fruit and veggies (preferably with skins on). 03.01.2022 It aggravates me no end when I see BMI being treated like a behaviour in research. There are no health behaviours (or combination of habits) that predict BMI in... any way that is useful. _____ Using BMI in this way reinforces the incorrect assumption that people with higher BMI's eat and move in different patterns from people with smaller bodies. It also erases the fact that changes in BMI can come about from a huge of combinations of factors, some health behaviour related, but many are related to not-great things like eating disorders, illness, grief and poverty. _____ This study is useful to demonstrate this point because it looks at fruit and vegetable intake, regular physical activity AS WELL AS BMI (as a behaviour). The assumption that has been used to justify 'healthy weight' as a behaviour is that people with a BMI less than 25 must 'eat well' and do 'correct' amounts of physical activity (this means even by their own justification that the use of BMI as well as fruit/veg/activity is redundant ARGH). If 'healthy weight' were actually reflective of health related behaviours, they would have found a high overlap between the 'healthy weight', 'fruit/veg' and 'physical activity' groups. But they didn't. Only 3% of the population didn't smoke, ate 5+ fruit/veg, was physically active and 'healthy weight'. The strongest overlap of 'habits' was in non-smokers, and that's because of the high general prevalence of non-smoking. _____ While you're there, check out the top of Table 1 for the age related relationships - people gain weight as they age (no surprise), but they also eat more fruit and veg and get more leisure time physical activity...... 01.01.2022 This is a significant issue for overweight/ obese people and can prevent them from seeking care. Medical practitioners should "do no harm" and blaming weight for every healthy issue can lead to a worsening of mental and physical health for patients.
