25.01.2022 Biopsy If your skin lesion appears suspicious, a biopsy might be necessary to evaluate the tissue by experienced skin pathologists. You will receive a local anaesthetic (lignocaine) to numb the biopsy site. This is usually given by injection with a thin needle. The numbing medication can cause a burning sensation in the skin for a few seconds. After the injection you shouldnt feel any pain or discomfort during the skin biopsy for 2 hours.... Shave biopsy, a shave blade is used to cut the tissue. The depth of the incision varies depending on the type of biopsy and the part of the body being biopsied. A shave biopsy causes bleeding. Bleeding is stopped by using diathermy or by applying pressure. Punch biopsy and excisional biopsy, the procedure involves cutting into the top layer of fat beneath the skin, so stitches or steri-strips may be needed to close the wound. A dressing or adhesive bandage is then placed over the site to protect the wound and prevent bleeding. After the biopsy keep the bandage over the biopsy site until the next day. Occasionally, the biopsy site bleeds after you leave the doctors office. This is more likely in people taking blood-thinning medications. If this occurs, apply direct pressure to the wound for 10 to 20 minutes. If bleeding continues, plesae phone the clinic and speak to the nurse. All biopsies cause a small scar. Some people develop a prominent, raised scar. The risk of this is increased when a biopsy is done on the neck or upper torso, such as the back or chest. Initially, the scar will be pink and then fade to white or sometimes brown. Scars fade gradually. The scars permanent colour will be evident one or two years after the biopsy. Avoid activities that might stretch the skin. Stretching the skin could cause the wound to bleed or enlarge the scar. Healing of the wound can take several weeks, but is usually complete within two months. Wounds on legs and feet tend to heal slower than those on other areas of the body. 25.01.2022 Importance of having family check those hard to see areas! While holidaying this patients family noticed a small mole / skin spot that they had not seen before, on returning from their holiday they booked in for a skin check. During the skin check this mole / skin was examined using the dermascope as well as the Mole Max, however another extremely small mole was also picked up by using the Mole Max. Both of these moles where removed at the clinic and sent for pathology, wit...h result coming back confirming that both lesion where Melanoma. However, the mole that was detected during the skin check with the Mole Max was confirmed as a Malignant Melanoma level 2, which required further excision. We can not emphasis the importance that it is not always the moles that you think would be cancerous its the ones that are required to be examined under magnification with specific skin caner detection equipment is which is where we general find the most dangerous and life-threatening cancers. Hence why having a full skin check is important.

23.01.2022 Treatment Options Many different kinds of surgeries can be used for skin cancer. The options for surgery depend on the type , size , location of skin cancer. In most cases the surgery can be done in at the clinic using a local anaesthetics. In skin cancer surgeries, first priority is complete removal of the cancer. Incompletely removed skin cancers can recur and in some cases spread to other parts of the body. Reserving the anatomical function os surgical site ( e.g. preven...tion of eyelid, nostril or mouth distortion) is the second priority. Last but not least, enhancing cosmetic outcome of the surgery is carefully considered during your surgery. After the surgery you will be left with a surgical wound. The wound is repaired in one of the following ways: 1. Closure of the wound with stitches that remain in place between 5 days to 2 weeks 2. Complex defects are closed reconstructive procedures such as graft or flaps. 3. Spontaneous healing by natural granulation i.e. allowing the wound to heal itself which could takes about 4-8 weeks and requires regular dressing. 23.01.2022 Melanoma is a serious form of skin cancer of melanocytes, the cells that produce dark protective pigment called melanin. Individual lesions may appear as a dark brown, black or multi-coloured growth with irregular borders that can become crusted and bleed. An increased risk of developing this disease is seen in people who have fair skin, light hair and eye colour, a family history of melanoma or who have had melanoma in the past. Over the next few images you will see a numb...er of different types of melanomas that we have treated in the clinic. Image 1: This is another great example of how a melanoma can carry the same colour as other freckles around a particular area. As seen in this image the ulcerated area on the right side is to a previous biopsy to confirm diagnosis. Image 2-3: From this image you can identify a dark and ulcerated mole on the patients neck, while at first glace this does have all the trademarks of a melanoma. However, when viewed using the dermatoscope (image provided) it has been confirmed as a pigmented basal cell carcinoma not a melanoma. Image 4: This patient does not have genetics on her side, she is fair skinned, light hair and has multiple freckles that can make detection that little bit harder. This melanoma resembles a benign and innocent freckle that blends in with others on the patients skin. However, this is in fact a melanoma.

Having regular skin exams is especially important for people who are at high risk of skin cancer, such as people, who worked out in the son for long periods of time, people with reduced immunity, people with fair skin, those that have ever had a skin caner removed previously. Knowing your own skin is of key important to finding skin cancer early At Ellenbrook Skin Cancer Clinic our doctors recommend checking your skin, preferably once a month. They are b...est performed in a well-lit room in front of a full-length mirror, use a hand-held mirror for areas that are hard to see, such as the backs of your thighs. All areas should be examined, including your palms, soles of your feet, scalp, ears, nails, and your back. Friends and family members can also help you with these exams, especially for those hard-to-see areas, such as your scalp and back. Be sure to bring any concerning lesion to the attention our team during your consult. Any unusual sores, lumps, blemish or change in the way an area of the skin looks or feels may be a sign of skin cancer or a warning that it might occur. 21.01.2022 How to improve the appearance of scars Scars will improve with time. Sutures may be removed at one or two weeks but the process of wound healing continues on for up to two years. Lesions on the face will fade quickly, other areas will be slower. Several strategies can be undertaken to minimise the prominence of scars, including: Pressure: Pressure can be applied by massaging or taping the wound. Massaging is most effective when it is done often, for long periods of time, and... with firmness. The wound can be massaged after sutures are removed and once the tenderness has settling down. Taping can also support the wound, it can prevent stretching as well as applying a small amount of pressure continuously. Avoid UV: Sunlight damages skin and damages healing wounds. Healing wounds exposed to the sun will tend to glow red and may even get hyper pigment. After the wound has healed application of sunscreen SPF30+ is very important. Lasers: This can be effective under the right circumstance to reduce the redness of scars and in certain circumstances to be used as a resurfacing tool to fade scars into the surrounding tissue. Again the particular type of scarring that has occurred will dictate whether a laser is an appropriate adjunct to optimise a scar or not. Not sure what may be the best option for you? 19.01.2022 Wound Care Skin cancer surgeries like any other operations have risks and can develop complications. Some of the main complications from any type of surgery can range from: swelling and bruising, especially if the excision site is around the eyes and nose. bleeding... infection numbness scaring For management of the above we at Ellenbrook Skin Cancer clinics suggest you try the following: Pain: To reducing your pain, we suggest that you take any over the counter pain medication, if we feel that you made need some stronger medication the Dr will provide you with a script. For minor procedure Panadol and Ibuprofen provide enough coverage. If pain and redness dont subside after 72hrs we advise you to contact us your medical BDM and speak to the nurse. DO NOT take aspirin as this may increase the risk of bleeding. Bleeding: The excision site may ooze a little blood or clear fluid especially in the first few hours after surgery, which is normal. If bleeding occurs, apply firm pressure directly to the site for 10 minutes may be helpful. If dressing becomes soaked with bright red blood, remove the dressing, apply gentle but firm direct pressure to the wound for 10 minutes using a piece of clean gauze. Avoid peeking during this period. If the bleeding stops, clean your excision site and re-apply a clean dressing. If bleeding continues despite 30 minutes of pressure, please call us or attend an afterhours doctor. Swelling: Is a commonly occurrence after surgery. Swelling is more common for surgeries on the face, however all surgery can cause swelling deepening on the size of the excision. You can reduce swelling by applying ice packs on the area for 10 minutes, every hour for up to 24 hours after surgery, we also advise you if you have had surgery to your face to limit your bending over head down to as little as possible. Bruising: If your surgical area is near the eye, nose or even the forehead, you may experience some swelling as well as black eye within 48 hours of the surgery. This is nothing to worry about and will improve after a few days. Scars: The excision line is always larger than the size of the skin cancer. From straight excision, to flaps and grafts that come in all shape sand sizes will eventually fade overtime if looked after and scar treatment cream is used. It could take several months for the scar, flaps and graft to look better.

17.01.2022 Youve heard of the pimple popper well this is not a pimple that you can pop! Its not uncommon that we have patients attended the clinic with what they think was a simple pimple or ingrown hair follicle that has become infected. This particular patient came to Ellenbrook Skin Caner Clinic thinking that the pimple shape papule on his nose was nothing more than a nasty pimple that never went away. After putting off having this look at by a Dr came to the clinic for a check wher...e our Dr advised him that the large cancer on his nose was not a pimple and needed to be removed. It pays to always have a growing lesion or non healing / resolving lesion looked at by a specialist, even if it looks like it has improved or decreased in size but has not fully disappearing. 16.01.2022 Melanoma We find many of our patient frequently ask what does a melanoma look like? Patients come in concerned about a mole which looks black or has a darker pigmentation that other moles. Over the next couple of post we will show you a number of different images of Melanoma that we have removed in the clinic. This is not by any means meant to alarm or scare anyone, it is so show and make you aware that a melanoma can present in multiple forms, size, colour, location and sha...pe Image 1: This melanoma located over the right shoulder blade does not show much if any colour variations compared to the patients other moles, it is in fact the irregular shape and size of this mole that makes it a stand out cancerous lesion. (the blanching around the lesion is in fact where the local anaesthetic has taken affect) Image 2: This is an interesting case and we would love to see how many can pin point two melanomas on this gentleman. We are guessing that you would not have a problem be able to find the main melanoma, you are in fact correct it is the large cat foot print lesion in the middle of this patients mid back region. This lesion has unfortunately been confirmed as an advanced melanoma. Image 2.2: This cancer Is not as easily identified. As you can see on the close up it take the appearance of a dry scaly patch of skin without pigmentation. Where as most of the research identifies a melanoma as a dark coloured lesion.

15.01.2022 Lets talk Basal Cell Carcinoma, also known as a BCC as we like our abbreviations. Generally, BCC's are the least dangerous of skin cancers. They rarely spread or become life threatening. However the problem is that even when they appear small, they may have invaded deeper into the skin. The below patient was advised by his regular GP to apply a topical cream, as the area of concern was not a cancers lesion. After coming to Total Health Ellenbrook we removed the lesion and had... it confirmed by pathology that it was a BCC. No amount of apply a topical cream was going to make it go away. We can not stress enough the importance of seeing a trained specialist when it come to having your moles, lesions and regular skin check completed. 14.01.2022 How to spot skin cancers in a minute! Lots of people rely on a doctor visit every year to get a skin check. This properly isnt enough as we are missing or delaying picking up lots of skin cancers specially if you have a number of moles and blemishes on your body. In-between your yearly skin check at the practice take note of the ABCDEs of your moles (as shown in the picture), and if you notice anything that looks different make sure to book yourself in for an earlier skin check. The ABCDE approach is what doctors use as well as it is based on any changes to a mole. You should aim to check your body month yourself as this assist during your skin checks if you are worries about any suspicious moles or blemishes.

13.01.2022 Winter Dectect! Skin cancer doesnt conveniently stick to the season, infact skin lesions can often take months or years to develop. That is why it is important to keep checking for any changes to your skin no matter what the season. The day doesnt need to be hot for us to be receiving harful rays. No matter the time of yeat, were being exsposed to UV radiation. The trick is to slip, slop, slap and seek shade in every season.... Our skin clinic can be very busy over summer as everyone is reminded of the importance of skin. 13.01.2022 Wound Care After 48hrs - 72hrs hours you can remove the dressing: Wash your hands with soap and water Wash the biopsy site with soap and water. If the biopsy site is on your scalp, use shampoo. Wash the area gently, do not rub or scrape the site.... Rinse the site well. Pat the site dry with a clean towel. Apply Vaseline ointment (from new tube) on the area a few times a day. You want to make sure the site is always moist. If you see that the area is dry, apply the ointment. You do not want a scab to form. You should keep the area covered with a thin layer of ointment at all times until the site is completely healed (usually10 days). A scab may form, but do not pick it. You may continue covering the site with an bandage in case of bleeding, oozing, friction, or heavy / dirty work. Choose a dressing that allows the skin to breath ( e.g. Primapore, or Fixomull). If other advise has been given to you please make sure that you follow these instrustion as no two patients are the same and post operative care will be tailored to each individual case. Call the clinic if you have: Pain that gets worse a few days after the procedure Alot of swelling Bleeding which persists. If you have bleeding, apply firm pressure over the gauze pad for 10 minutes. If it doesnt stop after 10 minutes, apply pressure again for 20 minutes. If bleeding persists, call the clinic. It is normal to have some blood on your gauze pad after the procedure. A temperature above 38 C for 24 hours Redness, warmth, swelling, puslike drainage at site.

13.01.2022 Excision In a excision, the skin cancer and a margin of healthy skin around the initial biopsy site are removed, usually in the shape of an ellipse. The ellipse is often designed so that the resulting scar runs parallel with existing skin creases. This usually provides a wound under less tension and orientates the scar in a direction which is less noticeable to the eye. Scars are usually 3 times longer than the original lesion. The tissue is then sent for processing and margi...n evaluation by a pathologist. Once the tissue has been removed, the edges of the wound are sutured together. There are usually two layers of sutures (stitches) a layer underneath that is absorbable and a layer of sutures on the surface which will need to be removed in 5 14 days or longer depending on the location of the excision. While excision are a safe and effective procedure, all medical procedures carry some risk, though they are minimised in the hands of our qualified team. 12.01.2022 Second Opinions Can Save Lives! This lady was no stranger to skin cancer, she has previously had a large melanoma removed from her leg by plastic surgeon who uses skin graft. She has since been left with a hideous skin graft that she was self-conscious about. This patient was concerned about having any further moles removed if she was as going to be faced with the possibility of having another skin graft. During her initial skin check a full history was taken about her pre...vious skin cancers as well as family history. From the consult two suspicious lesion where found on both of her triceps, these where sent away for histology and came back confirming both lesions where melanoma. Further excision was completed to ensure that clear margins where attained. We pride ourselves on our cosmetic outcomes for patient and very rarely use a skin graft as a closing technique as the final cosmetic appearance is not always what the patient was expecting. This is why it is very important if you are not sure during your consultation to either book a additional appointment to run through your surgery or seek a second opinion, these are both options that this patient has fished she had done. 12.01.2022 Post Operative Care Lets talk post operative care! It is important to look after your excision site post skin surgery to help prevent infection as well as to promote good wound healing. Depending on the location of your excision site and the complexity of your skin cancer surgery we may also advise you to avoid heavy lifting and exercise, especially in the first 24-48 hours after surgery. ... We also advise our patients to keep the area dry and covered for 24 hours. While you have sutures in we do advice that you stay away from swimming pools and spas as they are can be breading grounds for bacteria which can easily get into your excision site. Sutures are generally removed in 5 to 21 days after the surgery, this will once again depend on the location and size of the excision, however our team at Ellenbrook Skin Cancer clinic will advise you when to best have them removed. We suggest to adhere to the following post excision: Avoid strenuous physical activities such as going to the gym, swimming and running for at least 2 weeks after surgery Reduce or stop smoking as smoking causes poor wound healing Sleep with your head elevated to reduce swelling if surgical site is on the face If the surgery was on your face, avoid bend over for the first few days but rather squat if you need to reach down. Try to avoid applying makeup or powder directly on a fresh wound unless the surface is fully healed. You may notice discolouration of skin (pinkness/redness) at the site of surgery for up to a year after your surgery. Once the site has fully healed, you should apply a broad-spectrum sunscreen with an SPF 30+ and moisturiser to protect it from scarring

10.01.2022 Why does Local Anaesthetics Sting? It is one of the great ironies of life that Local Anaesthetics should cause a degree of pain before it start to work to block out further pain. But why does it sting when first injected? The local anaesthetic works to block the function of nerves carrying pain sensation but they take time to do this, during this period a number of aspects that cause the pain.... Firstly there is the inital sting of the needle going in, although this is more of a discomfort that actual pain. The actual injection of the local anaesthetic fluid exspands the tissues and this is what causes the pain. The pain can be reduced by slowly injecting and making sure the needle is positioned deep enough in the skin to allow this exspansion. Finally, and probably the main cause of the pain, is the local anaesthetic solution is more acidic then the body tissues. Ithas to be this way in order to preserve the solution allowing it to be stored. The pain caused by this acidity can be reduced if the doctor or nurse mixed some sodium bicarbonate with the local just before injecting.

10.01.2022 Some types of skin cancers are a challenge in diagnosing as well as providing the best treatment options. A good example of this is treating an Infiltrating Basal Cell Carcinoma (BCC). They usually have a nonspecific scar type pattern not at all easy to differentiate from an old scar. Even after confirmation of diagnoses, the other challenge is how big is the cancer? With an Infiltrating Basal Cell Carcinoma what is seen on the surface is generally just the tip of the iceber...g. The cancer can be much bigger than what is presenting on the skin. The pictures below are of some examples of this type of cancer that we have seen and treated at Ellenbrook skin cancer Clinic. The first two images show an Infiltrating BCC on the face. The other images are from another patient which had a BCC behind the ear. From removing this cancer which has infiltrated deep to the cartilage we were required to remove part of the cartilage involved plus more of the skin from behind the ear to ensure that a clear margin was obtained. WE achieved a wonderful cosmetic outcome for this patient.

07.01.2022 Lets talk Skin Lesions! Moles and Skin Cancers come in a vast array of shapes, size and colour. Melanoma on the face can be slower growing than other body sites, you need to beware of any non-healing lesion on a sun-exposed area such as the nose, lips and ears. Whereas Basal cell carcinomas (BCC) may be a flat, solitary, non-healing red patch, or it can appear as a white scar-like area without history of injury. If a skin lesion on the head or neck is not responding it is a... good idea to have these reviewed. Unlike pigmented melanoma, a melanotic melanomas are often very faint. They may be pink or red, with some lesions even resemble a tiny scar or acne that isnt healing. Which you an see from the pictures of patients that we have treated. 06.01.2022 Wound Repair Wound healing is a physiological phenomenon that has a time course over six months. Scar continues to change for up to two year. The first phase of wound healing is called the inflammatory phase which occurs within the first 3 days of the wound healing; it is the bodys normal response to injury causing heat, redness, swelling and oozing. Second phase of wound healing, within 3 24 days is when the wound is healing and the body makes new blood vessels covering t...he surface of the wound. The wound is reconstructed and become smaller as it heals. Third phase is called maturation (24 days to 1 year), when building blocks are aligned and reorientated into optimal configuration to best resemble the normal tissues. Scars will remodel and improve for a period of several years. The wound at this stage is still at risk and should be protected where possible. 06.01.2022 Malignant Melanoma Diagnosing malignant melanoma can be a challenge. More often than not a malignant melanoma can look just like an innocent freckle hidden among the other freckles on our skin. We have provided some images of what we have seen come through the clinic and diagnosed as malignant melanomas.... The first image below belongs to one of our patients that attended for an initial skin check. He had noticed a growing lesion on his forearm that he was concerned about, this patient was lucky despite the ugly shape of the lesion it was confirmed to be a harmless freckle. However, this is why we state that it is so important to have a skin check with a trained professional / company who have all the appropriate equipment when it comes to skin checks. The second image is of another lesion on the arm from different patient that was detected during their general skin check. This one is completely different from our first case as has been diagnosed as a malignant melanoma even though it has a harmless appearance.

06.01.2022 Wound Care after Graft surgery Sometimes not all excisions are able to be closed during the initial procedure and we may use a temporary graft, until we are able to fully close the excision. Below is some advice to help you care for a graft and donor site. Graft Site... Leave the dressing dry and intact until suture removal is booked. You may shower, but cover the dressing with plastic or a temporary waterproof dressing It is important the dressing does not get wet. Donor Site This is the area that the skin was taken from. Leave the original dressing dry and in place for 2-3 days You may shower but cover the dressing with plastic or temporary waterproof dressing After 3 days follow these steps: 1. Remove the dressing soak dressing off in shower 2. Wash gently as normal or bathe with saline solution (salty water); 3. Allow to dry; 4. Apply a thin layer of Vaseline over sutures 5. If exposed to a dirty/dusty environment cover wound with a dry, non-stick dressing; 6. Repeat steps 1 to 5 every day until removal sutures. Bleeding If Bleeding occurs, apply firm pressure to the area for 30 minutes Stay still If persists, call the surgery Infection Please contact the surgery as soon as possible, if any signs of infection occur such as: Increasing redness / swelling Increased Pain Hot to touch Oozing Our friendly staff are alway available and able to assit you with your post operative needs.

The most important thing you may do for your health this winter is get your skin checked. While a great number of people take on-board the sun-smart message during summer, it is often neglected during the winter. Skin cancers can develop at any time, so sun protection measures and skin checks should not be neglected during the cooler months. People need ...to be aware that skin lesions from sun damage during summer months wont show up overnight. In fact, it may take months or years for the damage to develop into a skin cancer. During summer you are far more likely to notice moles or unusual lesions as more of your skin is exposed. During the winter months we are heavily clothed and as a result often less likely to notice suspicious moles. Additionally, winter may prove to be the best time for a full skin check. As most people are covered up during the winter months, this minimises sun exposure to skin lesions and moles. This can allow for better dermoscopy viewing. Sun exposure can cause a mole to transform slightly which can make a normal mole appear suspect. Remember if you notice a spot on the skin that looks different from the others or is changing, bleeding or itchy, it should be examined by a doctor. 03.01.2022 Surgical Outcome: Cosmetic outcomes are important! This gentleman presented with a lesion to his ear that we initially biopsied as we waited for the results which confirmed. Results came back confirming that the lesion was an invasive squamous cell carcinoma, meaning the lesion had invaded / infiltrated adjacent structures. With these results we where required to take further margins and ensure that it could not spread any further to adjected organs or structures. A second su...rgery was booked for this patient where we performed a wider margin removal from around the initial biopsy site. This is a procedure that we perform on a regular basis, however a number of other Drs would have used a graft technique to close this excision which would not have result in a good cosmetic outcome. We used the existing skin from around the excision site and removed it from the cartridge to allow extra elasticity which allowed us to close the excision resulting in an excellent cosmetic outcome for the patient. Not all Drs are perficient with closure techniques, and a number of Drs would have used a skin graft of a flap technique which would have left the ear disformed and with a visible difference between the two ears. It pays to check all Drs work and ask for example pictures of similar previously surgerys.
