25.01.2022 PERSISTENT PAIN Episode 1 Pain does not always equal damage Pain has been designed to protect the body. When we get injured the body systems intensify the message for pain perception to rest/protect the affected area during the healing process. Usually, most of the musculoskeletal injuries would heal within 3 months and after this period the systems that intensify the pain messages should turn off and the pain experience should end. 24.01.2022 PAIN FACTS Episode 2 Do I need a scan? Maybe, but it is more likely that you don't.... Findings on scans are very common also in people without pain and in young people. In fact, only a small percentage of musculoskeletal injuries have findings on scans that are relevant. That means that incidental findings are extremely common and most of the time poorly reported findings (the use of 'scary' words like wear and tear, degeneration, bulging, protrusion...), can have a negative effect on people in terms of pain and recovery. Furthermore, it has been showed that scans most of the times do not explain pain and disability. Therefore, the answer to the question is is that there are specific clinical indications for when scans are needed, and for what types of scans should be performed. 10.01.2022 Osteoarthritis: what it is and what you can do Osteoarthritis is a very common condition that affects the whole joint and mainly the articular cartilage, a shock absorber that spreads the load over its surface. In a normal cartilage there is a balance between the degeneration and regeneration process encouraged by healthy joint loading, but when this balance fails, osteoarthritic changes start. The main joints affected are knee, hip and hand that besides being painful with we...ight-bearing or movement, are also stiff mainly in the morning with restriction of movement and often clicking and crunching, warm and swollen and the surrounding muscles are tight. There are some factors that raise the probability to get osteoarthritis that are not modifiable such as age, sex and genes. Other causes can be changed to decrease the risk of osteoarthritis or improve the symptoms, like obesity (more load on the joints), physical inactivity (little or no load on cartilage is not enough for regeneration), muscle weakness (not enough support to the joint), sports related joint injuries, or work related joint injuries. How can you treat osteoarthritis? Education: learning about the condition s a good way to improve knowledge and confidence to manage it. Exercise: has multiple benefits for hearth and vessels, muscles, bones, weight reduction, pain relief. Everyone regardless age, injury or illness should be physically active 30 minutes a day that can be done in 10 minutes bouts. Weight reduction: for decreasing the load on the joint and improve function. Medications: for pain relief. Aids or assistive devices: such as braces, insoles, walking sticks. Self-management is the most important treatment method for osteoarthritis. This is the ability to care of yourself through your own behaviours. 02.01.2022 Tendinopathy, what you should know Tendinopathy can affect any tendon, but it is more common in the Achilles tendon (heel/calf), rotator cuff tendon (shoulder), patellar tendon (knee), elbow tendons (Tennis and Golfer's elbow) and base of the thumb tendon (de Quervain). Here's a list of what you should know about this condition:... - Tendinopathy can be caused by different factors: Sudden change in certain activities, biomechanics, systemic factors (age, menopause, high cholesterol..). - Rest only is not enough: the tolerance of the tendon to load is not increased, although the pain might settle. - Exercise is the most evidence based treatment as progressive loading promotes and increase tendon load capacity. - Passive treatment such as massage, ultrasound, injections, can give a temporary pain relief but for long term results exercise is vital. - Tendinopathy improves very slowly with exercises, therefore it is important to be patient and wait for more invasive treatments like injections or surgery.
