20.01.2022 This week at Flight Safety Solutions. The cable or Zip tie. What a great invention. It is important to trim the tails. Here is a picture of our customers aircraft where a zip tie has not had the tail trimmed and it has been wearing on that panel for years. Measuring 0.008 inches, who would have thought a little piece of plastic would do this. On an RAAus aircraft, built to a weight limit, you may find your aging aircraft with a nice new hole one day. Oh, and while we are on the subject of zip ties, PLEASE trim the tails with flush cutters. Those razor blades behind the dash are very annoying. FLY SAFE - HAVE FUN

13.01.2022 This week at Flight Safety Solutions. Earlier we wrote about a poorly placed coolant hose clamp and highlighted RAAus Airworthiness Notice 270810-1 regarding the mandatory requirement to inspect the engine coolant hoses before the first flight of the day. Last week, during the 3rd Pre-flight inspection of the day, a student identified a coolant leak from the #4 Cylinder head, upper coolant hose mating inlet fitting. The o-ring was replaced and an EGR carried out with no... leak evident. Following a short test flight, the fitting was again inspected and found to be weeping. The seat of the cylinder head that houses the o-ring was inspected with a magnifier and a crack was found on the inner shoulder. The aircraft was removed from service and a serviceable cylinder head assy was obtained from Bret Flood Imports. Cylinder Head replaced IAW the Rotax Maintenance manual and test flown serviceable. So what went wrong?? Hard to say but possibly in the history of this aircraft, we think that someone may have removed the o-ring with a screwdriver or some other metal devise which could have scored the shoulder and then over time the crack has appeared. What can we do to prevent things like this in the future? Always use plastic or some other soft tooling when working with mating faces. The flight school operating the aircraft has now adopted a standard practice that the cowlings are to be removed adequately to inspect the cooling system component prior to EACH FLIGHT. Remember, the aircraft was inspected prior to the first flight of the day as required by the RAAus AN, but the defect was detected prior to the 3rd flight of the day. The school had scheduled another 3 flights following the discovery which could have continued unnoticed. Worst case was complete loss of coolant and possible engine failure. Most cowls are designed for easy removal so be safe and take the time to ensure all systems are functioning correctly before each and every flight. FLY SAFE - HAVE FUN See more

07.01.2022 This week at Flight Safety Solutions, we are continuing the recovering our Decathlon. She is a well loved 1979 8KCAB Bellanca. When ever you have access to an area of your aircraft you may not normally see, always take the opportunity to inspect to make sure all is ok. In the first picture, this is how we found the wiring loom laying inside the wing. Note how the wires are rubbing against the rib. Closer inspection reveals the start of damage and potential electrical fault that may develop. Refer to AC43-13 for wiring loom and other safe practices. The last picture is the issue resolved by standing-off the loom away from the rib lightning hole . FLY SAFE - HAVE FUN

02.01.2022 Today at Flight Safety Solutions Whilst performing a "Pre-Purchase Inspection on a A22LS Foxbat, we observed that one of the coolant hoses was not secured appropriately with the constant pressure clamp. Picture 1 shows the clamp positioned incorrectly downstream of the fitting flange. The clamp in this position offers absolutely no seal for the hose and has the potential to allow the loss of engine coolant whilst in flight, resulting in a engine overheat and possible eng...ine failure. Picture 2 shows the clamp re-positioned to the correct location. Owners/Operators, Pilots and Maintenance personnel should make themselves aware that It is a requirement under RAAus Airworthiness Notice AN270810-1 that all engine cooling hoses are inspected at each daily inspection. If this means taking the cowls off to perform this inspection then please do so. Loss of coolant whilst in-flight can be a hazard to flight safety. RAAus Airworthiness Notices are MANDATORY and are available on-line via the Members Portal under the "Maintenance" menu. We encourage everyone to review them all to determine if they affect you, your aircraft or your operation.
