25.01.2022 Here’s Hayley casting and fitting a Richie Brace for our lovely patient Gina who has a painful flat foot due to tendon dysfunction of the main supporting structure of the arch. A Richie Brace is a custom-made ankle foot orthosis (AFO) designed to treat complex and chronic conditions of the foot and ankle. It incorporates and custom foot orthotic with adjustable semi-rigid lower leg uprights to stabilise rotational forces in the foo...t and lower limb. Different modifications can be made to add further restriction of motion depending on what is required. The Richie Brace is made from a plaster cast taken of the patient’s foot and lower leg. The lightweight, low-profile design is preferred by patients over traditional longer leg solid shell ankle foot orthoses. Richie Brace AFOs are well suited to treating a number of conditions, including: Adult acquired flatfoot secondary to posterior tibial tendon dysfunction Lateral ankle instability Degenerative joint disease of the ankle or rearfoot Peroneal tendinopathy Dropfoot

24.01.2022 Shin Splints series Medial Tibial Stress Syndrome MTSS is perhaps the most common presentation of shin pain (lower leg exertional pain) associated with running or high impact activities. The pain is localised to the lower 1/3 of the tibia (shin bone) on the medial aspect (inside of the leg) there may be no pain at the beginning of the run, but after a period of time the pain starts to build and may continue to increase as the run continues... Sometimes the pain goes away during the run depending on the stage of injury. There may also be pain after cooling down and on walking for several days following the run. Pain and symptoms for medial tibial stress syndrome exists on a spectrum with less severe symptoms dissipating quickly and more severe symptoms resulting in stress fracture of the tibia.

23.01.2022 Running Economy Tip #5: Add in some short interval and tempo runs. Aim to include some deliberate practice running which includes short fast intervals and tempo running. This type of running places more demand on the cardiovascular system (amount of oxygen consumed and delivered to the working muscles) thereby increasing the efficiency of oxygen consumption over time. Improvements in the efficiency of oxygen consumption lead to improvements in running economy. ... Warning! Short intervals and tempo running should be combined with plenty of easy running (See previous post on endurance running).

23.01.2022 ! Common cause of forefoot pain as a nerve running between the metatarsal becomes compressed and aggravated. Shooting, burning pain in ball of foot extending into toes (usually toes 3&4) Often caused by collapsed metatarsal arch or poorly functioning big toe joint... + Made worse by tight fitting footwear, prolonged walking & running Treatment often involves avoiding aggravating footwear, improving big toe joint mobility, re-creating metatarsal arch all to reduce compression of aggravated nerve. If conservative treatment is not successful this would suggest the neuroma is very large in size, then ultrasound-guided cortisone injection, nerve ablation or neuroma surgery may be required. The earlier conservative treatment is introduced the greater chance of avoiding big needles and surgery!

In Rheumatoid arthritis, the immune system targets the lining of the joints, causing inflammation and joint damage. RA usually affects smaller joints, such as the joints in the hands and feet, however larger joints such as the hips and knees can also be affected. 12.01.2022 Shin Splints Series Anterior tibial stress fracture This is a less common presentation of shin pain and is localised on the front of the shin bone in the centre third of the lower leg. This is one of the high risk stress fracture sites of the foot and lower limb high risk indicating that the complication rate is higher for non-union healing. ... The cause of an anterior tibial stress fracture is similar to MTSS however the location of pain is different and the risk of delayed healing is much greater.

The plantar plate is a thick ligamentous structure located in the ball of the foot and connects the bottom of the base of the toes to the corresponding metatarsal head. Injuries to the plantar plate include partial tears through to complete rupture. ... Plantar plate injuries can occur from an acute incident, such as landing heavily on your forefoot playing squash, however they mostly develop slowly over time from a progressive degeneration of the plantar plate from repetitive overuse or biomechanical abnormalities (such as bunions, dysfunctional big toe joint, long 1st metatarsal, collapsed metatarsal arch, heavily over-pronated foot type). >Pain and swelling under ball of foot >Toes may splay apart (especially toes 2-3), may become clawed or elevated >Pain is worse in strong toe flexion, such as back foot in a lunge or squatting down to garden Treatment depends on whether the injury is acute or chronic in nature. 09.01.2022 Shin Splints Series Compartment Syndrome Compartment Syndrome is another version of shin pain that presents with running or high impact activities. It is also referred to as Chronic Exertional Compartment Syndrome. The lower leg is divided into 4 muscle compartments, each with its own blood supply, nerve supply and function over the footand leg. During exercise the bloodflow to muscles increases thus enlarging them. Each compartment is surrounded b...y a fascia that contains the compartment and during exercise the fascia would expand with the muscles. Compartment syndrome occurs when the fascia is not expanding with the muscles which causes an increase in pressure within the compartment. The increased pressure results in pain in the leg, a feeling of heaviness or high pressure and as the activity continues there may be symptoms in the foot (such as pins and needles, numbness or weakness). As soon as exercise stops, the pressure reduces and the pain dissipates. There is usually very little residual pain once exercise has stopped.

04.01.2022 | Low Level Laser therapy on Scar tissue | Laser therapy is effective for tissue healing and reducing scar tissue build up. Our lovely patient had a plantar fibroma surgically removed from the bottom of her foot which did not completely heal and then required a skin graft. The painful scar kept splitting open and made the arch of her foot very tight and therefore difficult to walk. With laser therapy the scar softened and became more pliable so she could walk properly.

04.01.2022 The importance of your big toe in maintaining arch stability The big toe needs to be straight with the 1st metatarsal to allow your arch muscles (in particular your abductor hallucis) to engage. When your big toe starts to move towards your 2nd toe you lose stability in your arch. Its also really important to have adequate big toe joint mobility and a straight big toe to have healthy feet - something we can definitely help you out with

Laser therapy is particularly helpful in the more acute, flared up stages of heel pain as it is a completely painless treatment with no side effects.
