14.01.2022 PILES / HAEMMORHOIDS Piles are the swollen and inflamed veins around anus i.e. varicosity of the anal veins, which occurs due to increased pressure on the walls of these veins due to any cause ( like constipation) leading to loss of firmness, and veins becoming weak , tortuous and dilated at last. Due to this, there is stagnation and accumulation of blood in the dilated veins, so veins swell up and start becoming more and more weak. Now, as more and more pressure increases ..., the veins may rupture and the blood oozes out. i.e why there is bleeding in piles and it is said to avoid any strenuous work and prevent constipation. TYPES OF HAEMMORHOIDS (a)Internal haemorrhoids- If the anal veins get swollen at the lower part of the rectum and anal region due to increased pressure is called internal piles or internal haemorrhoids. It lies deep within and can’t be seen outside. Usually they are painless, or have very less pain sensing nerve in the rectum. Bleeding is common. Sometimes , enlarged haemorrhoid may protrude from anal sphincter but they usually recede into rectum. It hurts because of rubbing from clothing or sitting. (b)External Haemorrhoids- If the anal veins get swollen at the anal region under the skin due to increased pressure is called as external piles or external haemorrhoids. It lies within the anus and if it is prolapsed outside , during the passage of stool it can be visualized. Painful , then internal piles. Appearance- it looks blue or purple when thrombosed and could bleed sometime. It looks frightening but usually not serious and can resolve on it’s own. CAUSES- Increased pressure in veins is due to more pressure outside the veins, i.e. increased intra abdominal pressure, which could be due to 1. 1)Constipation-Straining during stools or hard stools which is due to lack of fibre rich food or insufficient water intake 2. 2)Chronic hyperacidity or flatulence problem 3. 3)Prolonged sitting for long hours together- lack of physical activity 4. 4) Fast pace lifestyles- 5. 5)Obesity and aging- 45 TO 65 6. 6) Pregnancy 7. 7) Chronic Diarrhoea 8. 8)Unhealthy life style habits and unhealthy diet- junk food , fast food, alcohol intake, smoking 9. 9)Hereditary. 10. 10)Pelvic tumors SIGNS AND SYMPTOMS- 1. Bleeding 2. Pain during and after stools- it occurs if there is inflammatory condition developed in venous area that causes vein to swell up which initiates muscular spasm around it. Or a thrombosed pile can cause pain 3. Anal itching 4. Prolapsed piles 5. Sticky and bad smelling secretion 6. Anaemia
