25.01.2022 Are eggs ok or not for your health? First, it’s not good, then it’s Ok and now we are not sure. Maybe it is, maybe it isn’t. We are talking about eggs and the association with cardiovascular disease and mortality. It’s stuck in many of our and our patients’ minds that eggs are no good. It doesn’t help when the 2015-2020 Dietary Guidelines for Americans came out with somewhat contradictory recommendations: (1) Cholesterol is not a nutrient of concern for overconsumption and (2) Individuals should eat as little dietary cholesterol as possible while consuming a healthy eating pattern (1). 24.01.2022 One of the things I remember quite clearly from medical school all those years ago that still resonates in my mind is this: if you are on the way to an emergency, walk briskly. Do not run. Running will worry your patient or the family and makes the situation worse. Running may get you there a few seconds earlier but it makes very little clinical difference. So, the Australian government rush to make sure mass Covid-19 vaccination is rapidly rolled out is just like running to an emergency. 24.01.2022 An update: Heathcote Road still closed tonight. Have checked the whole family (and doggie smuggled in) for the night. Have to work out the logistics for tomorrow morning with everyone having to go everywhere. Hopefully, road will be opened tomorrow. Thanks again to all who have offered their generosity. 23.01.2022 Learning lots in Berlin at the European Association for the Study of Diabetes (EASD). But took time out last night in the cold (12 degrees!) to participate in a 5km charity run. Achieve a personal best (PB) of 24 mins 15s.

23.01.2022 I have been holding off writing about the coronavirus so far as I didnt think that I have the expertise. I would be out of my depth. Its been coming up to three months since the world woke up to Wuhan. Its been 6 weeks since Australia banned all flights from China. It was only a matter of time before the virus run rampant in Australia causing mayhem. It was a matter of when rather than if. You would have thought and expect that with a lead time of 3 months that we would ha...ve a disaster plan in place by now. According to the Collins dictionary, a plan is: a method of achieving something that you have worked out in detail beforehand. Was there ever a plan? A reactive response is not a plan! 22.01.2022 Every so often I tell friends and colleagues that I run marathons and they look at me strangely and ask "why". Then the usual questions about my heart and my knees etc. Nope so far so good. Yes, a few runners of all distances do suffer from sudden death from time to time but in general, the number are significantly less than the general population who dont run dropping dead. What about triathlons? Now thats a different category altogether. 20.01.2022 People who dont exercise sometimes argue that fit athletes die at sporting events and thats why they "dont exercise". Actually, runners overall have a lower sudden death rates than the general population. 19.01.2022 The fluvaccine seems to be late again this year. There are a number of good reasons for that. We are now recommending people have their vaccination as close to winter as possible for better protection during the peak flu season late in winter. 19.01.2022 You must be utterly confused about the whole Covid-19 vaccine roll out by now. There is conflicting messages out there from different arms of the government - all very scary but yet it's ok to still go ahead? They then say, if you are under 50 years old, just wait for the Pfizer but it will come towards the end of the year. Maybe, maybe not. We have signed up contracts for up to 140 million doses of all sorts of vaccines but some have not have their Phase 3 trials approved, s...ome we have contracts previously that were not enforceable and therefore means nothing with or without a contract anyway while others we are not sure can be manufactured here or not. All these sounds very political to me. Is it more about looking good for the next election? Is it more about covering up the mess in the roll out? 19.01.2022 You dont have to be stuck with diabetes. Its frustrating isnt it when managing patients with type 2 diabetes (T2D). Patients adhere to diet and lifestyle the best they can, the sugar control is on target but yet we have to keep layering more tablets or injections over time to maintain the constantly deteriorating glucose control. 17.01.2022 The Covid-19 vaccines will become available in a matter of weeks. It is likely that the first trance of people getting it will be frontline health workers and people >65 years old. Because of storage problems, most likely the healthcare workers in the first trance will be getting the Pfizer vaccine via the hospital. The rest including those in the nursing home will get the AstraZeneca/Oxford vaccine. The second trance of people will get theirs after the first trance is finished. Should you have it? 17.01.2022 We often assume that getting "old" equates to getting weak hence the term "old and weak". It doesnt have to be part of getting older. More often than not, we tend to do less, lift less and slow down as we getting older and as a result, muscles wither and we are no longer able to do the things we used to do. As we slow down, we get weaker and are unable to do the things we would like to do. See the vicious cycle that is the beginning of the end. 16.01.2022 How to protect yourself from Covid-19 Social contact - zilch. Apart from people living with you, dont go visiting ANYONE. This stops you from getting infection from someone or you giving someone if you have the infection Handwash after each task. Gloves doesnt work. You dont get Covid-19 through the skin when you touch contaminated surfaces. Its when you touch surfaces and then contaminate the rest of your body and clothing which ultimately ends up on your food then gets ...you Covid-19. If you cant wash your hands, sanitiser will do. Facial masks. With the number of cases in Australia still limited (for now?), its debatable when masks for healthy people is necessary but if and when the numbers soar beyond 10,000, would be a good idea to wear one when you go out. Make your own. Leave the commercial ones to healthcare workers. Keep your home clean from the outside world. Leave your shoes outside. Be careful where you "park" your shopping bags and take them home onto your kitchen benchtops. Wash or sanitise hands when you get into the car and before you open your home doors. Dont bring germs home. Always wash hands before you eat whether at home or outside.

16.01.2022 It’s expensive to build pavements for pedestrian use. But if they are so poorly maintained that they are known to the local community to be dangerous and they avoid using it then whole purpose of building pavements is not realised. In the end though, residents still walk on the road and are in danger of being run over by motorised vehicles. It’s a lose-lose situation. So, what does that have to do with Medicare? Medicare began life as Medibank in 1975. Successive governments tinkered with it but basic tenets remain the same. 15.01.2022 The younger you are when you are diagnosed with type 2 diabetes the worse your long term outlook is. 15.01.2022 Sorry guys, off to Berlin for a week conference on Diabetes. Its the European Association for the study of diabetes Annual Scientific conference. This is no golf trip (not that I play golf). They are all pretty much 13 hour day, starting from 7.30 am (except Monday) to 6.00 pm followed by more sessions till 8.00 pm. Then some dinners till about 10 pm. Even lunch times are filled with lunch meetings and posters to read. All work no play. Wednesday night is the 5km charity ru...n from 8.30 pm. Couple with 30 mins train ride to conference hall makes the whole day very long. But worth it as there are a lot of networking with similar minded doctors - GPs and diabetes specialists. Will be back on Monday 8th October.

14.01.2022 Its all deja vu, isnt it? I thought we did all this few months ago and with the number of community acquired Covid-19 almost non-existence, we are on the home run. No. We are back to square one. We all did our bit of social distancing, social isolation and hand hygiene and we got there. Suddenly, we are looking at a second wave. Yes, we always fear that there will be a second once we relax the restrictions. In this case, the second wave is primarily related to breach of hot...el quarantine procedures in Victoria which did not even have their health department overseeing the running of the quarantine process. To make things worse, we are know that NSW took too long to close the border and close the border with ample advanced warning to make it pretty much useless. 14.01.2022 Do all overweight kids develop diabetes? If not, who does? We know that high BMI at a young adult age is associated with type 2 diabetes (T2D) later on in life (1,2). High childhood BMI or low birth weight is also associated with increased risk of adult T2D. The physiology during childhood and during puberty are actually very different. Is the childhood BMI the risk factor or the adult BMI the risk factor? Does the degree of weight gain matter? The BMI Epidemiology Study Got...henburg (BEST Gothenburg) studied the impact of birthweight, childhood and adolescent BMI on adult diseases. 11.01.2022 Most of you would have read about the recent study linking some medications used to treat blood pressure to the risk of lung cancer. This was published in the British Medical Journal last week. it was a bit of a surprise to some extent although once the dust settled, it wasnt that surprising after all. A class of medication called Angiotensin Converting Enzymes Inhibitors (ACEI) causes a build-up of bradykinin in the lungs and bradykinin has been reported to stimulate growth of lung cancers. ACEI also results in an increase in another chemical, substance P and that has been linked to tumour growth. Suddenly, there may be some substance to the study after all. We better explore more. Dear chee khoo, "Commiserations, your application to run in the 2018 Virgin Money London Marathon has been unsuccessful. A record number of 386,050 hopefuls applied to take part in next years event." 386 000 people applied for 40,000 places! 10 times oversubsribed. ... Well, there is always next year.

03.01.2022 Thank you to all those who has offered accommodation to me and my family while the fire along Heathcote Road is raging along. I live at Sandy Point and was out when the fire closed the roads yesterday afternoon. Incidentally, everyone was out including the dog. We were having lunch at the Bone Cafe at Leichhardt yesterday which is good and bad. Good in that everyone is safe, bad we cant get home. So far, our home is not threatened and Heathcote Road is still closed. Not sure when we can get home if at all by tonight. Worse of all, just need to check in somewhere for a shower and change of clothes. Streamed movies and pizza is fun so far. Tomorrow will be surgery as usual but come in pair of jeans and (new) T-shirt. Will keep everyone posted.
