13.01.2022 Common Foot problems in Children: Ankle and foot problems are common in children of all ages. Some foot problems present as painful symptoms and can become problematic for the child and can cause degenerative pain as they get older. Some foot problems are not painful and often go undiagnosed which can also cause degenerative disease as the child gets older.... Some ankle and foot problems are hereditary, while most are due to strain and mal- alignment of muscles and tendon with repetitive use during sporting activities. Many problems are often overlooked or labeled as growing pains and insignificant to follow up and are not diagnosed correctly. Some common foot problems you should be aware of that can affect children include: Flat feet: Flat feet are commonly a hereditary condition that can affect walking, balance and physical stability. The arch of flat feet can improve as the muscles and tendons strengthen with age and development. If the flat foot persists it can cause alignment issues of the muscles and tendons, not only in the feet and ankles but also in the knees, groin, hips and lower back contributing to damage to joints. In-toeing (Pidgeon toed): In-toeing or an adducted gait is when a childs feet turn inwards instead of pointing straight. This causes the child to trip on their own feet and are usually awkward and un co-ordinated. This condition can improve with age, however, the earlier intervention and treatment is instigated the better the results for improving the alignment of the legs and feet. Severs Disease: Severs disease also known as Calcaneal aphophysiitis. It is a growing pain problem where the bones grow faster than the muscles and tendons where they attach to the heel bone. If a child is active in their sporting activities there can be a repetitious strain on the achilles tendon where is attaches to the heel bone, causing inflammation of the tendon and the growth plate of the bone. Children tend to complain of pain at the back of the heel during exercise, after exercise and may find walking painful after periods of rest. We have four layers of muscle underneath our feet. These muscles help to keep us stable when standing and walking.... But when you put a thick layer between them (hard sole) and the surface you walk on, you can impede the development of the smaller muscles. This is made worse if the sole of the shoe is hard and inflexible. So small toddlers should wear only shoes that have very soft and flexible soles to allow for the natural development of their feet. Children tend to complain of pain at the back of the heel during exercise, after exercise and may find walking painful after periods of rest. Severs disease is most often seen in children between the ages of 8 to 15 years of age. Severs disease is commonly given as the diagnosis of all childrens heel pain. However, in the majority of cases, the heel pain is caused by a combination of flat feet and poor alignment of the muscles and tendons where they attach to the heel bone (biomechanics) and a growth spurt of the bones and soft tissues. 01.01.2022 Back Pain caused by poor foot alignment: Causes of Lower Back Pain: There are many causes of lower back pain.... These can be: - Spine (Bone degeneration) - Nerve damage (compression through the spinal discs) - Poor alignment of the pelvis - A number of diseases However, one of the major causes of lower back pain in incorrect positioning of the pelvis during walking and this is often due to poor foot alignment and mechanical function of the foot, ankle, knees and hips. How can a Podiatrist help with your back pain? An experienced Podiatrist can help your back pain significantly if your back pain is related to your foot biomechanics. A Podiatrist can clinically assess this during a clinic consultation. However, if there is other pathology causing the back pain the success of treating back pain successfully is decreased. Understanding foot Biomechanics: Flat feet or feet that excessively pronate (roll inwards) causes internal rotation at the ankles, knees, groin and pelvis. These rotations cause a forward tilt of the pelvis which increases the curvature of the spine. Ultimately this places strain on the muscles, tendons and ligaments of the lower back causing inflammation and pain. Bunions, or restriction of the big toe joint can reduce the pivot motion at this joint. When the foot can no longer pivot the foot loses its ability to propel forward fluidly, causing the foot to slap the ground reducing the foot being able to absorb shock. Bunions can also cause the foot to turn outwards changing the knee and hip rotation and ultimately changing the spine curvature. This lack of shock absorption and change of alignment can lead to major spine and hip pain. Treatment for back pain related to poor foot function: Custom made orthotics are the ideal choice. The orthotics help to balance the feet and control abnormal motion. This then assists to stabilize the pelvis and enhance normal function of the spine.
