19.01.2022 If you read nothing else please read this Birth trauma week starts on the 6th and goes until the 12th September and I will be focusing on this a lot in the c...oming week. There is a common myth out there that is also often politically hijacked that birth trauma is just physical trauma and hence the focus ends up on the pelvic floor and manifests as anti normal birth. As over a decade of ours and others research has shown this is just the tip of the iceberg. Birth trauma is so often about not being listened to, disrespected and losing control. It often leaves no physical scars and so is dismissed as something women should just get over and be glad they had a healthy baby. I have heard leading health professional and others say no one is ever traumatised by Caesarean section. This reveals the reductionist, narrow thinking on this issue. We must work to reduce all kinds of trauma whatever the woman or her partner says that trauma is. Our book this year, Birthing outside the system: the canary in the coal mine, showed clearly that trauma is so often deeply psychological and also can be physical and we must work to reduce both. The safest birth is one that is supported by a respectful, competent and known midwifery care provider backed by an equally respectful and component obstetric provider. Doulas also play a critical role in reducing birth trauma. I have become a great advocate of these amazing people.This kind of supported birth has been shown to be both physically and psychologically safe. 17.01.2022 RESULTS: Of the 474 participants initially enrolled, 342 (72%) children had a neurocognitive assessment at 11 years of age. After adjustment for mode of delivery, probiotic treatment group assignment, income and breastfeeding, children who had received antibiotics in the first 6 months of life had significantly lower overall cognitive and verbal comprehension abilities, increased risk of problems with metacognition, executive function, impulsivity, hyperactivity, attention-deficit hyperactivity disorder, anxiety and emotional problems. CONCLUSIONS: These results provide further evidence that early exposure to antibiotics may be associated with detrimental neurodevelopmental outcomes.

16.01.2022 Many women are told that if their water breaks (at term) that they will need an induction if they dont go into labour within a few hours. They are often fright...ened into believing that their baby could develop a serious infection that could cause stillbirth. Its understandable that in these situations women would consent (almost without question) to an induction of labour soon after their waters broke. To be clear, I am NOT talking about a woman whose water breaks preterm (before 37 weeks of pregnancy) OR if there is meconium in the amniotic fluid (an indication that the baby may be experiencing distress). I am talking about healthy, low-risk women whose labours just happens to commence with a spontaneous rupture of membranes (approximately 10-15% of labours start this way). The recommendation to start an induction less than 24 hours following the rupture of membranes is incredibly outdated and NOT evidence-based! Research shows that with proper care, waiting for up to 48-72 hours after the water breaks does NOT increase the risk of infection or death to babies who are born to mothers who meet certain criteria. However, waiting means that birthing person may have a higher chance of experiencing infection themselves (Hannah et al., 1996; Pintucci et al., 2014). Statistically, up to 95% of women will go into spontaneous labour within 24 hours of their water breaking. 14.01.2022 How can you open the pelvis more when pushing?? Usually youll hear spread your knees wide to open the pelvis. But what part of the pelvis opens? When the knee...s spread apart, this tends to bring the femur into external rotation which opens the inlet, or the top, of the pelvis. But external rotation tends to close the outlet, aka where the baby is trying to move through during pushing. So, what should you do to create space in the pelvis while pushing? Internal rotation of the femur, or ankles out further than knees. Knees can still be wide, but focus on moving your ankles out just a bit further than your knees are! This can be done on your back, side, all fours! If someone is holding your leg, ensure they are gentle w the movement and not trying to force your leg. Just a little goes a long way! So when someone tells you to spread your knees to create space for your baby, its a statement that makes sense in the beginning of labor but maybe not anatomically correct when pushing. "The forward-leaning inversion potentially makes room for a good ...fetal position by untwisting any ligaments to the lower uterus and cervix that may be twisted from sudden stops or a habit of a twisted posture. Kneeling afterward allows temporarily lengthened ligaments to align the uterus with the pelvis. The cervix may become less tipped or tight and then able to let babys head to position well, and the cervix to dilate in labor more easily." One brilliant and insightful article (SWIPE) by two Public Health Speciality Registrars in England lays out the facts clearly - Entonox is contributing to global warming. ‘Climate change is one of the biggest threats to the health of a child born today. Yet most babies in the UK are born into a room full of greenhouse gases, with nearly 80% of women using nitrous oxide (mixed with 50% oxygen, also known as Entonox) during labour. The carbon footprint of an average birth using Entonox as analgesia is 400kg CO2e3 this roughly equates to driving an average-sized car 1,400 miles’. ‘Unfortunately, many healthcare professionals, and labouring women, remain unaware that the nitrous oxide in Entonox and other anaesthetic gases are major greenhouse gases’.
