24.01.2022 #RuralGeneralism Today was a very special day for Rural Generalism (RG) in Ravensthorpe. This speciality within the next 18 months will be a protected title (Like plastic surgeon, neurosurgeon, cardiologist etc) when approved by the Australian Medical Council (AMC). ... This pathway has also been approved and endorsed by the Morrison Government and will see specialist rebates attached to some consultations in the future Professor Ayman Shenouda the head of Rural Generalism for the Royal Australian College of General Practitioners (RACGP) purposefully flew to meet Dr. Michael Livingston in Ravensthorpe to discuss the role of the Rural Generalist in WA and see the extended skills and qualifications of Dr. Livingston in implementing this role in Ravensthorpe and Hopetoun as the only RG in this region. During the 2 hour meeting they and Dr Chris Buck (Director of Rural for WAGPET) all discussed various aspects of the role of Rural Generalists in Australia in the future. Professor Shenouda now flies to Perth to meet various government bodies involved in the development of this pathway in WA, armed with a first hand encounter of what life is like in Ravensthorpe for an RG. What is a Rural Generalist? We define Rural Generalist Medicine as the provision of a broad scope of medical care by a doctor in the rural context that encompasses the following: Comprehensive primary care for individuals, families and communities Hospital in-patient care and/or related secondary medical care in the institutional, home or ambulatory setting Emergency care Extended and evolving service in one or more areas of focused cognitive and/or procedural practice as required to sustain needed health services locally among a network of colleagues A population health approach that is relevant to the community Working as part of a multi-professional and multi-disciplinary team of colleagues, both local and distant, to provide services within a system of care that is aligned and responsive to community needs. 24.01.2022 Threats and Abuse We will not tolerate any abuse - verbal or otherwise against any of our staff. This will be reported to the police where any and all legal action will be taken. We are all human beings and deserve to be treated with dignity and respect, especially given the current climate, being kind to each other should be at the forefront of our actions and has never been so important.... Dr. Livingston has been threatened with a violent act by a patient. So much so, that we have to now consider his personal safety and that of those around him. Unlike Gayles Law which protects nurses to be accompanied after hours. No such Law exists for rural and remote doctors. They are the only profession that are expected to respond after hours, in the dark alone, all other professions can respond in twos or have a lone worker policy that logs the persons whereabouts and monitors check in and out times for safety. Therefore Dr. Livingston responding to emergencies after hours will be reviewed for his own personal safety. He has been providing sole emergency after hours care for the last 5 years at all hours of the evening and morning but threats like this provide a stark reminder of the isolation and vulnerability that he provides this under. We have poured our heart and soul into providing some of the best health outcomes in Australia for the residents of the Ravensthorpe Shire. By threatening the very people that provide this service it will only further jepordise the ongoing provision of healthcare and change our recruitment strategy of attracting new doctors to this region if we do not feel it is safe for them to come and work here.

22.01.2022 #TheNextGeneration This is Eilidh. She is a final year medical student about to qualify as a doctor in October 2021. As part of her clinical attachment with The University of Western Australia Eilidh has chosen to come to Ravensthorpe for her 5 week placement and spend time with both Dr. Livingston and Dr. Paget to experience a taste of life as a rural generalist.... The plan is to show Eilidh and other medical students another side of medicine that does not often get showcased to undergraduates before or during their training and more than just a day trip. So far Eilidh is enjoying her experience and since finding out about this unique placement with Dr. Livingston we have 5 other students now apply to come! which makes this a bit of a hot spot for experience. The long term plan is that students experience other options when time comes for specialist training selection and hope one or more of them remembers this experience and chooses life as a rural generalist. 19.01.2022 Have you met Andrew Charlesworth yet? Andrew is an Accredited Pharmacist who is working with our team to maximise peoples understanding of their medicines. This is a service that has never been offered in this region before. Andrew willl be visiting Ravensthorpe and Hopetoun every month, visiting patients in their own homes to review their medicines, talk about interactions and tweaks or changes to make sure that people understand their medicines thoroughly.... His visits are completely funded by the government and he sends us a visit report to action if required. This allow for us to make any necessary changes in your medications and provide education to those that require any medical intervention after his visits! 18.01.2022 #Coronavirus #Update The last few weeks have been hectic both at work and behind the scenes.... From #Flu Clinics to preparing for the arrival of COVID. The role is relentless. The biggest challenge will be managing the severe cases of Coronavirus and ensuring that secondary infections from the virus does not affect frontline staff or indeed kill any of us. Dr. Livingston has been exploring options from getting a prototype 3D BIPAP mask sent down for testing from Perth to getting a custom 3D printed mask for healthcare providers made by Ninja Shark (for which he will review when he gets a minute) to even making his own version with a N95 mask and piped oxygen! Ninja Shark are trying to crowdfund the development and 3D printing of these masks for frontline staff to prevent the spread of COVID via the 3 routes (Eyes/Nose/Mouth). https://ninjashark.com.au/pages/air-filter-pre-order They have so far raised $25000 of a $50000 goal. These options are good and offer better options the current PPE on offer - a set of gardening goggles and N95 duckbill mask. So it was with this that Dr. Livingston got speaking to Keith Muller from Galaxy about what other PPE could be obtained in order to offer those of us with direct patient contact as much safety as possible. This is where the idea of a Powered Air Purifying Respirator (PAPR) came to life and following a review of the literature and filtration option by Dr. Livingston, Keith offered to very kindly purchase 2 pieces of PAPR (at $2600 a piece). Today was handover day and Dr. Livingston was very, very grateful for the donation which in turn will be used in the hospital for the benefit of staff and in turn the community. So that he and the staff member on duty will have as much facial protection as possible when managing a patient with Coronavirus. Dr. Livingston has also ordered custom scrubs so that he doesnt have to wear normal clothes and have the associated infection risk. He has also completed a University of Melbourne course for the Ultrasound scanning of the Lungs for patients with suspected COVID which will allow earlier pickup of the lung changes which often precedes those requiring ventilation as well as doing the Sonosite modules on different scanning techniques and finally having access to an expert sonographer for backup. Lastly he has worked every single day for the last 3 weeks, 7 days a week attempting to get ahead of this. With increased patient demand in and out of hours and the extra hours studying, attending meetings and reviewing literature in preparation. Fingers crossed for a safe and healthy #Easter weekend. 17.01.2022 Exciting news Been a while since an update but as usual busy, busy, busy behind the scenes. Dr. Livingston has been given surgical approval by the WACHs surgical credentialing committee. ... This means that Dr. Livingston can undertake advanced surgical procedures locally at Ravensthorpe Hospital. Normally these procedures would have to go to plastics or the general surgery list at Albany or even Perth. However and unique to this area only, these procedures can now be done here exclusively by him. Next Dr. Livingston has signed 19AB paperwork with WACHs. This means that for each and every patient he sees in the emergency department, inpatient or otherwise he voluntarily gives up 100% of the income that would be billed to Medicare. The income derived exclusively from his provider number then goes into a pot. A panel is then convened with the sole purpose of deciding how that money is then used to buy new equipment for the hospital and the benefit of shire residents. Annnnnd This Tuesday - West Australian General Practice Education and Training (WAGPET) are sending down a crew to Ravensthorpe. The purpose of this day is to showcase the role of a Rural Generalist in order to inspire the brand new West Australian Rural Generalist Program (WARGP) and future doctors/medical students. Dr. Livingston has been handpicked for this. So there we are. We continue to push the boundaries of what Rural Generalism can offer and more importantly give back in some way selflessly to the community. 16.01.2022 The WA Government have advised Health Practitioners to begin Immunising patients for influneza as soon as the government vaccines arrive. We have received our allotment of Flu Vaccines for the OVER 65's ONLY from the government today, therefore we will be holding our Flu Clinics starting next week. For OVER 65's. Please call the practices to book in.... Please also be patient as we will be adhering to the social distancing policies which will mean staggering appointments and a number of other distancing, PPE and environmental hygiene policies will be in place. 16.01.2022 #NewYear #2021 Happy New Year ... We have hit the ground running starting back on the 4th January after a short break and already we have exciting news. We are still on track to have Dr. Felicity Paget start on the 13/01/2020 and we know she willl be an amazing addition to our team. In the background Dr. Livingston has been squirrelling away working, studying and doing courses and as a reward for his hard work he gets to add another string his bow. As well as having: Advanced Specialised Training in Emergency Medicine (AST EM) Diploma in Skin Cancer (Dip Skin Ca) Remote Area Ultrasound Exemption (RAE) He was awarded a timely and welcome Christmas Present on the 24/12/2020. The paperwork has been submitted and the approval granted with notification today. Dr. Livingston has now been approved and formerly recognised as a GP Surgeon. This is a rare qualification with less that 300 recognised GP surgeons in the whole of Australia. That’s less than 0.8% of the total number of GPs in the country. Quite an accolade. This additional ability and skill only further bolsters his skill and ability to work in his role as a Rural Generalist (RG) with an AST in EM & Surgery. This is just the beginning. Dr Felicity Paget is an Australian Born and Trained Doctor who is embarking on her journey to become a Rural Generalist. Of all the places Dr Paget could have taken she specifically chose to train under Dr. Livingston due to his reputation amongst his RG colleagues across Australia. Special thanks also goes to Rachel Livingston who worked hard in the negotiation, communication with the regional training organisation, application process, Medicare process and every other process in between which kept Dr. Paget on track to apply here. Dr Livingston has advanced qualifications in skin cancer medicine and surgery and performs skin checks in the Hopetoun and Ravensthorpe practices and simple and complex skin surgeries including grafting at Ravensthorpe hospital.... Did you know that Australia has one of the highest rates of skin cancer in the world, including melanoma and non-melanoma skin cancers. Every year in Australia skin cancers account for around 80% of all newly diagnosed cancers and around 2000 people die from this disease every year. However, more than 95% of skin cancer cases are caused by unprotected exposure to UV radiation from the sun, meaning it is a nearly entirely preventable cancer! 09.01.2022 There is no I in team and what a performance today I would like to thank those wonderful St John Ambulance who give up their time unpaid for the betterment for the residents of the Shire of Ravensthorpe. Today was a Herculean effort by the volunteers and our stand in community Paramedic Doug. Twice in two days called to critically unwell people and getting them there.... Next was the combined effort of the nurses at Ravensthorpe Hospital, Dr Livingston and WACHs Emergency Telehealth Team consisting of EM physician and critical care doctors coordinating and arranging transport as well as assisting with critically important steps in the management of the patient. Finally to Royal Flying Doctor Service of Australia for coming in, retrieving and patient out #Teamwork

07.01.2022 COVID VACCINE CLINICS We have been advised that we should start to receive the vaccine on the week commencing 12th April. We are eligible to receive 50 doses per week only.... We will be hosting a clinic once a week in Ravensthorpe, initially for people who are eligible for the 1b roll out. Then more vaccines should become available over time and further roll out stages. 07.01.2022 2020 FLU CLINICS Government recommendations for Vaccinating against flu in 2020 is to start immunising in May. (WA Specific advice, if travelling interstate, April vaccination is advised) Our initial clinics will be:... Hopetoun: 6th, 13th and 20th May 2020 Ravensthorpe: 7th, 14th and 21st May 2020 Government Vaccines are fully funded by the government at no external cost to patient. Private Vaccines are available at a cost of $15 per vaccine. We will open more dates as required. In 2020, influenza vaccines funded through the NIP are available to the following groups. This is due to their increased risk of complications from influenza. All people aged 6 months to less than 5 years (this cohort is newly eligible in 2020) All Aboriginal and Torres Strait Islander people aged 6 months and over Pregnant women (during any stage of pregnancy) All people aged 65 years and over People aged 6 months and over with medical conditions which increase the risk of influenza disease complications. Timing of vaccination Timing of vaccination should aim to achieve the highest level of protection during peak influenza season.Optimal protection against influenza occurs within 3 to 4 months following vaccination. Providers should consider vaccinating in May and early June to ensure optimal protection throughout influenza season which typically spans July to October in WA.

This will not be a simple cold This is not scaremongering 8 out of 10 will recover 1 will be be severely unwell with it 1 will require Intensive care bed and will die We have a large proportion of people in the region in the at at risk group the fatality rate is 3.7% globally. In our population alone that equates to 55 expected deaths. Expected! We need to look at strategies to reduce its spread. This is why we must take this very seriously and over the next week we will look at moving to more Telehealth and Telephone consultations given the inherent risk of person to person spread.

She is unfortunately no longer with us but the words ring true every day. Your health is the most important thing for you and your family and you should always put if before all else. Whether it is something that has been bothering you f...or a long time, something that is sudden onset, something that is going on in your mind, or a pain or something that you cant pinpoint! Never wait, put yourself first, seek professonal help, make the appointment, get advice from a Doctor or Nurse, go to the hospital, get that lesion looked at, talk to that psychologist, go and get the blood test, follow up on that MRI result, do the bowel screen! 02.01.2022 #Varley #RFDS #5Years Today was the first day for Aimee and I as we did our inaugural clinic in Varley as part of the RFDS (outfits awaited)... Numbers were excellent, patients wonderful and thank you to the Varley Progress Association for Smoko and Lunch it was delightful and wonderfully appreciated. Till next month! Today is also an important milestone for another reason. On this day 5 years ago we started operating from Ravensthorpe and Hopetoun respectively. We were repeatedly asked when we started how long we would hang around for or how long did we ‘have to do’ 5 years later the answer is clear. We have taken 2 practices which started with below average clinical performance to not just one of the best clinically performing practices in Western Australia but Australia itself. We have moved forward from outdated clinical software into the 21st century and adapted 8 different new technology platforms within our practices. We have one of the first rural generalists trainees in WA. Dr Paget is the only Australian trained registrar in the region and is part of the WA rural generalist programme through WAGPET We have achieved 4 different types of accreditation for the practices (AGPAL, WAGPET, HDAA, Vaxhub) We have implemented online booking as standard and have the lowest waiting time to be seen in rural and remote Australia. Some rural locations and even regionally the wait is 3 weeks and not even to see the same same doctor. We have localised care through Telehealth and been instrumental in allowing immunotherapy and chemotherapy agents to actually be given in the community without leaving the region. We were the first to offer this.
