25.01.2022 They say it comes in threes... This is 2 fractured tuberosities in one week. Tuberosity is the bone at the back of the upper jaw. The first was referred to me after the referring dentist attempted the extraction and the tooth and tuberosity were hanging by the gum. The second was my own case... just solid bone around the upper left wisdom tooth... the tuberosity fractured despite removing the thick bone around the roots to the tips of the roots. It is seldom possible to p...redict tuberosity fracture. Most upper wisdom teeth come out very simply as the tuberosity is mostly soft bone, compressible and forgiving. 01.01.2022 WE DEAL WITH IMPLANT COMPLICATIONS. Narrow or small diameter implants suffer mechanical failure (breakage) more frequently than wider or larger diameter implants. This is a reflection of physics and mechanics. Here are some recent cases referred or came to us for management of their failed narrow implants, requiring surgical removal and grafting and then new implants to be placed. Narrow implants have their place and are best suited to replace the smaller teeth in the mouth ...and where they share the load and are protected by natural teeth or other implants in close proximity. The placement of a narrow implant is often easier for the dentist and the patient with simpler surgery, not requiring grafting, shorter in treatment time, less pain and less cost. Failure when it happens can negate these advantages many fold, causing severe bone loss necessitating surgical removal, grafting, even longer waiting times, and new implants and prostheses to be remade at even greater cost and surgical morbidity than would have been had it been done initially with appropriate grafting and wider implants from the outset. We will often do preliminary grafting to get better bone before placing implants. This may seem to cost more and take more time but our belief is to do it once. Many of our cases relating to implants will exhibit wider implants. Wider implants suffer less mechanical failure and presents a more natural emergence of the crown from the bone and gums as they more closely resemble natural tooth root dimensions at the bone and gum interface. Remember, the shortest and easiest way may not turn out to be so in the long term.
