25.01.2022 Reposted from @kinectmd . An ankle sprain is the most common acute sports trauma, accounting for 14% of all sport related injuries. Among these, 80% are ligament sprains. Inversion injuries are four times more common than eversion injuries, due to the instability of the lateral joint and weakness of the lateral ligaments in comparison with the medial. (Lateral - outer surface of ankle, Medial- inner surface) .The usual mechanism of lateral liga...ment injury is inversion and plantar flexion, which damages the anterior talofibular ligament before the calcaneofibular ligament. (AS SHOWN IN THE IMAGES ABOVE) . The anterior talofibular ligament can tolerate ~50% the strain of the calcaneofibular ligament before tearing, and it is also taut in plantarflexion, meanwhile, the calcaneofibular ligament is relatively loose. Complete tear of the anterior talofibular ligament, calcaneofibular ligament, and posterior talofibular ligament results in a dislocation of the ankle joint and is frequently associated with a fracture. . Grade 1:injury involves Stretching or slight tearing of the ligament with mild tenderness, swelling and stiffness. . Grade 2: refers to a more severe sprain, but incomplete tear with moderate pain, swelling and bruising. . Grade 3:a complete tear of the affected ligaments with severe swelling and bruising. . All three grades are associated with pain and tenderness. Grading of these injuries guides prognosis and helps determine the rate of rehabilitation. . Initial management requires RICE treatment (Rest, Ice, Compression, Elevation). Nonsteroidal anti-inflammatory drugs (NSAIDs) can be administered for reduction of pain and swelling, followed by soft tissue therapy and mobilization. The next phase of rehab includes restoration of full range of motion, followed by muscle conditioning, proprioception training, and lastly functional exercises to ensure return to sport without pain. Unfortunately, any time there is an ankle injury, this will put the individual at an increased risk for more chronic ankle instability and repetitive ankle injuries. . See more

It really is not that simple and many factors contribute to the chronic pain Pain Is Multifactorial Pain is NOT tissue damage but in actual fact is the perception of danger.... Pain is influenced by many factors. These include prior experiences with pain, emotions , stress - sleep patterns, environment and beliefs which can all lead to heightened pain responses. Educating people on the basics of pain allows them to be able to control it better and therefore reduce the perceived threat to the person. As the threat reduces and the capacity builds , so does the pain. This does not mean ignore your pain. But understand that behaviours like excessively guarding it , obsessing, refusing to move it, attaching negative emotions to it CAN be detrimental to your recovery. So be careful about the things others/u tell yourself: My back will never be the same again My ankle is fucked don't ever deadlift/squat/play a sport again With all the factors in play , it is important to note that rehabbing any injury involves both mentally and physically building the capacity for tolerance of the musculoskeletal structure as well as the nervous system. If one gets left behind , the progress falls short. Finding the sweet spot between building capacity progressively whilst not overdoing it is what ultimately leads to recovery. 21.01.2022 Great info & analogy Reposted @vinnierehab WHY KNEE PROBLEMS HAPPEN!! [dislocation explained] .... Did you know that the knee is the most commonly injured joint with an estimated 2.5 MILLION sports-related injuries every single year?! . Funny part is, most of the time, knee pain happens not because there is something wrong with the knee, but due to dysfunction in the joints ABOVE (hip) and BELOW (ankle) the knee . When patellar dislocations happen (left knee pictured) many therapists will WRONGLY emphasize the importance of taping the patella in place or strengthening the VMO muscle in order to "stabilize" the patella . Let's use an analogy to explain why overstabilizing the patella is a dumb idea! . If we were to use the analogy of a train rolling on rails, most of the time when an accident or malfunction happens it's because of the rails, not because of the train. (Especially in Bond Movies. Villains always to blow up rails ) . Similarly, the patella is a train that just follows whatever myofascial "rails" you give it!! You don't try to fix a train going off the rails by "stabilizing the train" or "adding strength/power to the engine of the train". You do it by fixing the . If you look at the mechanics of a knee dislocation, they happen when the femur is excessively internally rotated in relation to the patella/tibia . If your leg bone (femur) is going inwards and you can't control it's deceleration, the patella will end up LATERAL to the knee and "derail" . Situations that can derail your patella 1you can't control the speed at which your femur decelerates. Femur moves faster than patella = derail 2you can't externally rotate your femur fast enough when your extend your knee. Femur moves slower than patella = derail 3you get hit and it causes your femur to go in a direction faster than your patella = Your ACL explodes and your patella...DERAILS! . Basically people f*ck up their knees when the femur and the patella don't travel at the speed required to keep the train glued to the moving rails . A better rehab protocol would involve neuromuscular drills to recondition the knee to move in a strong+coordinated fashion . See more

14.01.2022 High foot arch Affecting Knee Before and after 30 mins later. No knee pain. Martin @martini.fitness is a jumping athlete that plays volleyball and basketball and came in with right knee pain in landing . ... Jumping athletes spend a lot of time on their toes and big toe , therefore would observe high arches , big calf bulk. (Not always- this is dependent on their jumping technique) A single leg jump landing requires force absorption through knee flexion and ankle dorsiflexion and three points of the feet making contact with the ground to stick the landing (tripod foot) Martin was not able to do this without laterally hopping due to his FHL, tib post ( muscle that forms the arch) being too strong . This affects a chain of reaction upwards to the knee as he lands with the knee traveling too far out in a varus position causing knee pain on the lateral structures . What we worked on? 1 Soft tissue release to the strong muscles - tib post , FHL , plantar fascia medially (would also look at releasing glute med to allow more IR hip) 2 Toe splaying & abductor digiti minimi to work on thr lateral intrinsic foot strength and pronation of feet 3 Pronated feet walking 4 Peroneal strengthening banded Focusing on strengthening the pronators of the foot to allow more lateral translation of the knee. This is why biomechanics and functional assessment is so important . NO athlete or patient presents the same but muscles consistently work the dame way with regards with concentric, eccentric or isometric. Martin was prescribed orthotics for an arch lift which he was told to remove as it was giving him more knee pain obviously because it was enforcing the problem. Watch your athlete , figure out their movement and the end goal vs prescribing COMMON exercises . This is what we strive on as clinicians in @pbphysio and what to continually teach and develop future generation of physios to think this way . I like to compare this drug to duct tape covering a hole in your cars tire. Its NOT fixing the root cause of your symptoms. This is the reason I have seen too many athletes get a cortisone shot, notice a decrease in pain for a few weeks or months & then their pain returns. They assume that because the pain is removed their ...injury is healed! . Now, my friend Kelly Starrett of @thereadystate once said, sometimes were grateful for the duct tape over our tire to help us get our car on the highway and to the repair man. 05.01.2022 Mid back or lower back pain with rotation? @pace_physio shows you a simple band or cable exercise that lets you build up your rotation - this doubles as a strength and mobility exercise as it’s easier to turn out with the band close to you, and then pressing away makes things harder, so all you have to do is control it pulling you back. If sore, start with turning away from the more painful direction. I.e. it hurts to turn left, use this exercise turning right to help c...lear that movement up, once turning left starts to feel better, use the exercise to strengthen going that way too! Control the movement and try to rotate from your back, keeping your hips as still as you can! Some people fear the movement of rotation - the body is made to move!
