03.01.2022 Executive Dysfunctioning and Challenging Behavior: Example It is amazing how many executive functions come into play that can result in children misbehaving. ... Since these deficits are common in autism, it is important that we step back before judging the intent of the childs misbehaving. They are often seen as oppositional, defiant, and purposefully trying to upset or manipulate us. Especially for the very capable children who should know better! Most children will do well if they can. If they are not doing well, please stop and assess if their misbehavior is not a reflection of these weaknesses. Do not blame the child, but seek ways to bridge these deficits. As we discussed earlier in the chapter, since most of these functions are brain wiring differences, the best approach to helping the person is to provide external supports (accommodations) that help bridge the brain deficits in executive functioning. Lets look at an example of how we used external accommodations to reduce severe behavior challenges in a classroom. Tony At school, Tony had frequent problems with remaining in his seat, finishing his work, and getting aggressive (hitting, kicking) when teachers tried to redirect or correct him. According to staff, Tony was unmotivated, oppositional and would act out to get his way. He would get bored quickly, refused to work, get up, and move about the room. When the teacher tried to redirect, Tony would throw materials, tip over desks, and physically attack the teacher. Aids would often physically restrain Tony to keep everyone safe, which only aggravated the problems. Tony was a bright boy and had the knowledge to complete most of the tasks, which frustrated the staff even more. However, Tony would not complete his work, became negative about trying and resistant to everyone attempting to help him. Our assessment revealed that Tony had strong executive functioning issues, which may underlie his challenging behaviors. He seemed to have difficulty maintaining attention, staying focused, and emotionally regulated. His working memory was weak, as well as poor organizational skills. His sensory processing evaluation revealed he had sensory modulation difficulties resulting in an under-aroused nervous system (meaning he needed ongoing stimulation to stay aroused and alert). Tony needed frequent doses of movement and physical activity to stay aroused. Knowing that he liked playing basketball, the teacher tried using time in the gym shooting baskets to reward completion of work. It did not improve his performance or behavior. It just made Tony more frustrated. Since he could not complete his work, he often did not earn time playing basketball. Assuming that Tony would do good if he could, we started looking at which accommodations would help him complete his work, allow him to frequently earn basketball time (which was also important to keep his nervous system aroused and alert) and allow him to build self-esteem through experiencing success. Instead of trying to force Tony into complying, we assumed that it was difficult for him to meet expectations and focused on bridging these deficits. The following accommodations were added. We moved Tonys desk to the front of the classroom, close to the teacher. This minimized distractions and allowed the teacher to provide easy monitoring and assistance. Tony could sit on an exercise ball and chew gum to arouse his nervous system. If he chose to, Tony could stand while working. The teacher also gave Tony some up and moving activities (erase the board, take items to the office, sharpen pencils, do wall push-ups, etc.) at least once an hour to alert his nervous system. He enjoyed helping the teacher and being up and moving. This helped keep Tony aroused and alert and gave him short breaks from sitting. Tony would become overwhelmed and freeze if given a lot of work at one time. Academic tasks were shortened or broken down into smaller steps. For example, 20 math problems would be reduced to ten. To help with working memory, for both the directions for the task, as well as the reward he was working for (basketball), on one side of the desk, we placed an index card with written directions to reference. In the middle of the desk was a photo of a basketball to remind him that he could play once the task was completed. On the other side of the desk was a break card that he could use when losing focus and needed to get up and move. To help with emotional regulation, Tony had a written menu of activities (wall push-ups, jumping on a mini trampoline, getting a drink of water, going to the bathroom, etc.) he could use to take a break and escape demands when he started to become overwhelmed. If he got tired, he could go to a mat and lie down. At all times, if Tony raised his hand, the teacher or aid would assist him. They also monitored him closely to offer assistance and remind him to take breaks when he started to lose focus. All these strategies were about supporting Tonys executive functioning problems, not treating his challenges as intentional behavior. We gave him choices, allowed him to pace his work, respected his voice when needing help or movement breaks, and provided assistance as needed. The staff did not pressure, demand, or continually prompt Tony. We provided him with the tools to be successful and gave him the control to pace himself. There were no time constraints or ongoing prompting to stay on task. The supports gave him tools to be successful and, with reminders, was given control over how and when they were used. All attention was provided on what Tony was doing right, even when he was struggling. Tony began to see staff as a working partner with him instead of instructing, directing, and policing him. As Tony developed greater feelings of competence and trust in staff, the staffs perceptions and attitude toward Tony became more positive as they saw him succeed. Tony was given a voice, felt respected and valued. He was not seen as unmotivated, indifferent, and oppositional. Staff saw that with the right supports, Tony was successful and happy. The more Tony felt safe, accepted, and competent, the more successful he was. Staff learned that when Tony was struggling, they needed to pause, look closely for what deficit was presenting the challenge, and with him, design accommodations for bridging the problem.
