23.01.2022 Hi, Everyone is talking about their sore Upper Traps aren’t they?! While there are lots of effective ways to temporarily release the stress and strain in the trigger points that build up in them, they are forever returning to haunt us after intense exercise sessions, or after sitting at the desk for too long. In the photo above, my two good friends Dan and Tarek are looking very serious with some awesome Kettlebell Upper Trap release techniques from the Rehab FX course (more ...gentle than it looks thankfully). Anything but the thumbs, we reckon! On some clients there always seems to be the struggle with tightness of the Upper Traps, even if they aren’t exercising. They may also experience headaches, occasional nerve symptoms, and really limited mobility in the neck sometimes.Upper Traps are a pain in the neck seriously! Surely there is more going on? Could Upper Trap soreness be the tip of the iceberg? Is it hiding other things? What is it protecting? 23.01.2022 Guess what? It’s actually true! There’s now real evidence of the obvious: Client’s generally DON'T DO their Home Exercises we give them. You might sweat and bleed over how many and which ones to give them (as I used to do, drawing fancy stick figures sometimes), but evidence from Danish Physio Michael Rathleff and his colleagues has shown that only 15% of participants in his Adolescent Knee Pain Study actually did their exercises. Only 15% of clients actually DID their prescr...ibed exercises?? WTF? Ok so they were all 10-18 years old in the study - but does it really change when someone middle-aged is given a programme? This finding correlates perfectly with my experience of course - those clients who have marked improvement in their pain usually (9/10 times) can show me the exercises I gave them a week earlier with no problems. The good news is that there are many ways to improve adherence to home rehab programmes, and have a positive attitude about the challenge. 23.01.2022 This week's subconscious Holding Pattern is "Closing the Groin" - perhaps weird when you first hear it, and I am not talking about an over-active pelvic floor! Rather, the Anterior Hip joint is being held closed at the front through excessive Tensor Fascia Lata and Adductor subconscious tension. The movements involved are foremost Hip Flexion, Adduction and Internal Rotation (hence it may also be caused the FADIR Movement Dysfunction" by some experts). Over the years, the h...ip joint often becomes stiff, causing a ripple of problems up and down the kinetic chain so it is good to have up your sleeve a simple MOBILITY TEST to see how severe it is (learn a simple mobility test for the hip on the second of our videos today - it will give you an idea of how blocked or even degenerative the hip might be if there is significant asymmetry and sharp pain on the test.) Females are more prone to this Holding Pattern due to them naturally having a wider pelvis, and because socially they sit with more modesty! First thing is to recognise it through visual observation, though! Look for a Closed Hip during standing, knees drifting medially, Trendelenburg Sign and excessive Anterior Tilt of the pelvis. With any serious pain / injury in teenagers, you almost have to presume they have one of the conditions that Adam discusses in our video today. Guilty until proven innocent! Growing pains and all the possible related conditions refer to the bony endplates where the growth is taking place - tendons attaching to the...m may inflame them (Traction Ephysitis) or pull off a chunk of bone they attach to ("Avulsion Fracture); the growth plate may even slip in the hip joint ("Slipped Femoral Capital Epiphysis) under chronic load, or of course fracture completely through if there is a fall onto an outstretched arm. The most common situation in gyms and clinics would have to be Osgood- Schlatter’s Disease, which concerns the Patellar Tendon pulling on the tibial tuberosity and causing pain and a tender lump right on the kneeling part of the knee.
