25.01.2022 I decided not to remove this #15 K-File but work around it. Mostly hand-filed & Obturated with EndoSeal MTA using 'hydraulic condensation'.



23.01.2022 Non-endodontic conservative management of Dens Invaginatus. Not an everyday occurrence in an endodontic office.

21.01.2022 Endo treatment on front tooth is supposed to be easy right? Wrong. This patient's 23 was unrestored and asymptomatic originally but she went to her dentist because it was slightly discoloured (It is still discoloured btw). In her own word, the dentist told her the tooth is dead and there is darkness in the root (she refers to the root canal as the darkness), so she ended up with this white line and endodontic infection occurs after only 2 years. The dentist who diagnosed the problem and did the treatment has left that corporate dental office, so she alone is left with the problem. First world problem I know, but still it is very unfair.

20.01.2022 AHPPC has eased the treatment restriction from Level 3 to Level 2 on 21st April 2020. Accordingly, we will recommence clinical practice from the week beginning Monday 4th May 2020. Patients who had their appointment cancelled in late March and April will be contacted from 29th April to re-schedule their appointment. Priority will also be given to patient whose treatment is urgent, for example, dental trauma, acute pain, and where delay may lead to tooth loss.



20.01.2022 FKG XP Finisher is a terrific tool for cleaning the root canal after root canal debridement. I like to use it to clean irregular shaped root canals, especially for endo retreatment cases. It is a safe instrument when used as directed. But I just like to push the limits. Thus, it is reassuring to know that the removal of the broken XPF is surprisingly easy! Just locate the file and hook it out with something like a Hedstrom.

17.01.2022 Root resorption. Patient is 63yo. CBCT was used to assess the lesions in 3-D. Endo access was via the occlusal surface of the crown to save the patient's 6yo bridge. Treated resorptive lesions with TCA and good old bur. Protaper Next X2 for mesial canals, ProFile 60/04 for the distal. Obturated with EndoSeal MTA & GP with Angelus MTA in the distal resorptive cavity. The endo access was then closed with Fuji IX GIC and Composite.

13.01.2022 Patient was referred for endodontic treatment because of significant root curvature. The tooth also has the characteristics of taurodontism, whereby the body of the tooth is elongated at the expense of the root lengths. The pulp floor is 'moved' apically resulting in an unusually deep endo access cavity. The RCT was started elsewhere. Root canal debridement using Dentsply Pathfile (old stock!!), MicroMega One Curve and NiTi hand files - Balanced Force. Obturated with EndoSeal MTA and GP.

12.01.2022 Tooth 21: This ceramic post replaces an older one that has fractured, it is a very retentive post ceramic post but unfortunately, the old RCF was pulled out knowingly or unknowingly. Subsequently, the root canal becomes reinfected and there is now an apical lesion. A X-ray before the post is permanently cemented would have alerted the operator to the violation of the 'root canal seal'. So sad as it is much harder to fix now.

11.01.2022 Some broken files are really easy to remove in the right circumstance, like this rotary file that was stuck between the isthmus of the buccal and palatal canals of the 25. After the removal of the RCF, a few strokes of a #30 Hedstrom file lifted the 5mm broken file right out of the root canal space.

04.01.2022 16 Endodontic retreatment: The 16 was root filled concurrently with a separated instrument in the DB canal. It has become reinfected hence the Endo retreatment. Careful ultrasonication in olive oil eventually caused the instrument to dance out of the canal. Apical blockage was then bypassed and the root canal debridement was completed successfully. The separated instrument was a Hedstrom file. Equipment list: (1)Modified Gates #3, (2)Terauchi File Retrieval Kit, Straight tip, precurved (not-katana modified). (3)Olive oil, (4)NaOCl, (5)EDTAC,(6)D-Finders #10,(7)Micro-Mega One Curve heat treated NiTi 25/0.06 Rotary file, (8)Hand files to finalise the RC prep.
