19.01.2022 The heel-toe drop in footwear - what does the current research say? As you can see in the image, the heel-toe drop is the colloquial term for the difference between the height of your heel in the shoe and the height of your forefoot in the shoe. The heel-toe drop is a very important factor when it comes to addressing walking and running biomechanics. In the coming weeks, well be doing some more posts on this, but today Id like to share some information with regards to ...heel-toe drop and injury risk. This has been a hot topic for many years, as some individuals and companies claim there is an ideal or optimal heel-toe drop... For anyone who understands human variation, youll find this difficult to accept. This notion was very big several years ago with the barefoot running movement. There was quite a push to have everyone wearing minimal (zero drop) shoes, with claims that shoes with a heel-toe drop increased injury risk... Up until the last few years, there hasnt been much data or research on heel-toe drops and injury risk, with claims usually just being anecdotal or marketing-driven... In 2016 Malisoux et al. published this paper in AJSM - https://www.ncbi.nlm.nih.gov/m/pubmed/27501833/ - which concluded that overall injury risk was not modified by running shoe drop, but lower drop shoes could be more hazardous for regular runners. Additionally, in this 2017 RCT by the same group, they looked at running injury prevention and risk, and again concluded that injury risk is not influenced by the drop of running shoes. http://bjsm.bmj.com/content/51/4/356.2?rss=1 Again, a 2017 RCT looking at running biomechanics, suggested that shoe drop did not seem to influence long term adaptation in running biomechanics. http://bjsm.bmj.com/content/51/4/394.2?rss=1 It is good to see some quality evidence supporting the notion that there is no such a thing as the perfect heel-toe drop in shoes. The heel-toe drop is something very important when it comes to addressing walking and running biomechanics, but it is important for us to look at each person individually - and not make claims about needs as a whole.

13.01.2022 Nick spent a week in Sri Lanka recently consulting to the Sri Lankan Cricket Team for their podiatry needs. He had a great trip and enjoyed being a part of the team atmosphere. Cricketers are exposed to a tremendous amount of lower limb stress whether it be bowling, batting or fielding. It is therefore important to understand an individual players needs so that they get expert advice on the correct footwear and on needed cases, orthotics.

This is a concept that should be very important for us as podiatrists, specifically when dealing with (tendon, muscle and etc). If you apply load to a structure that is higher than its capacity, it can result in injury... On the flip side, if you increase a structures capacity, it can lead to the structure being abl...e to withstand higher loads... This is a very important factor to consider when managing the many foot, ankle and lower limb problems we see! A good treatment plan should focus on addressing the load (examples: modifying activity, appropriate footwear, custom orthoses if necessary and etc) and increasing capacity (example: targeted strength-based training). 12.01.2022 Happy new year all! For those of you with kids, youll know that January is the busiest time to be buying school shoes for your children. Theres no doubt that sometimes it can be a strenuous task, but let us put you at ease with 3 simple steps to ensure you get the best possible shoes for your childrens feet. 1. Ensure that your child has roughly 1cm from their largest toe on both feet. Either feel for this through the top of the shoe, or take out the innersole and get your... child to stand on it. Many stores will measure their feet, but dont take this size as gospel. Each brand fits differently, meaning you may be different sizes in different brands. 2. Ensure your child tests out the shoes in store. We all know that kids dont only walk in their shoes Kids like to run, skip, jump and anything else in between! When appropriate, ensure your child properly tests out the shoes to ensure there is no rubbing, slipping or excessive stiffness that they may not discover otherwise. 3. Purchase a quality and reputable brand. There are many brands out there, but from experience we often see brands for school shoes such as Ascent and Clarks being the most consistent and durable. They also come orthotic-friendly, meaning they are easier to work with if your children requires orthotics.
