These pictures show gout involvement in the 2nd digits with the tophi being so severe that it has pierced through the joint capsule and overlying skin. She presented for a general treatment with her husband, she currently is being managed with custom footwear and custom orthotics. 09.01.2022 Chronic ingrown toe nails - With some patients a conservative care or less invasive methods such as a PNA is not successful and a more invasive procedure is required. A wedge resection (aka Winograd) is a surgical procedure where the outer ingrown nail, outer skin and nail matrix are all excised. This procedure aims to prevent the ingrown fragment from returning. The wound is closed using non absorbable sutures in primary intention healing. 08.01.2022 A Silver procedure is an older bunion procedure what consists of only removing the medial eminence of the metatarsal/bunion. As you can see from the X-ray - this procedure alone inadequately corrects the HAV deformity as it fails to address some key factors including - HV angle, IM angle and soft tissue rebalancing causing the bunion to come back. Current HAV treatment involves removing part of the medial eminence but importantly focuses on other key factors - realigning the joint over the sesamoids, correcting key angles and soft tissue rebalancing.

06.01.2022 Todays patient has chronic heel pain - multiple failed conservative care so I gave her a corticosteroid injection. Prior, I always get the patient to mark the area of most pain (usually it is medial tubercle as expected) Tibial nerve block is relatively comfortable and if I have time I usually give it for the patient as it makes their experience a whole lot more comfortable. ... 2 ways I do a tibial nerve injection 1. 2 finger spaces up from the medial malleolus, palpate the artery and insert your needle from the posterior aspect (Achilles) towards the tibia. You want to hit the tibia, pull back slowly and give the injection. 2. Mark the PT artery at the sustentaculum tali and just underneath it advance your needle towards the bone and give the injection.

06.01.2022 First metatarsophalangeal joint injection - corticosteroid injection with long acting local anaesthetic. - identify the patient who will benefit from the injection. In this case, this patient is suffering from hallux limitus, she has difficulty walking, currently using custom orthotics with rocker bottom jogger. Managing with ROM exercises yet wants future pain relief. - My steps: - Identify your anatomy > EHL tendon and find your joint space,... - Assess your x rays and identify where your injection will be, I usually go lateral as I find it easier to get into the joint. - Prepare the skin - chlorhexadine prep - Distract the toe/joint - Remember the concave/convex nature of the metarsal head and base of the proximal phalanx - so you will need to orientate your needle on an angle - Aspirate before injecting - Go shallow into the joint and inject your substance (I used 0.5% marcaine and 1ml/4mg dexamethasone) - Apply compression afterwards and dress the toe - Advise to avoid strenuous activity for a few days and review 4/52. #podiatry #podiatricsurgery #podiatrist #footdr #feet #foot #pain #medical #instahealth #surgery #medicine #ankle#sprain #bunnion #injury #foothealth #sydney #moorebank #Australia #australianpodiatrist #sportsinjury #footsurgery #bone #HAV#injection #shockwave

This particular patient had been suffering from plantar warts for many years - years of conservative care failed, the wart numbers were increasing and she was referred to me by another podiatrist for a curettage procedure.
