25.01.2022 Last week we delved into what Menopause is and we also mentioned that the most reported symptoms requesting treatment throughout peri-menopause is hot flushes. As you approach Menopause, progesterone and oestrogen are declining. While progesterone levels tend to drop on a steady slope down, oestrogen can fluctuate during peri-menopause. 19.01.2022 The most common causes of vaginal infections include bacterial vaginosis (BV), aerobic vaginosis (AV), cytolytic vaginosis (CV), vulvovaginal candidiasis (VVC) and trichomoniasis. These infections may present individually or simultaneously as co-infections. The normal vaginal ecosystem is a complex environment that features high levels of lactobacilli species that produce hydrogen peroxide. This maintains a low and acidic pH (between 3.8 to 4.2), which supresses the growth of... gram-negative and gram-positive anaerobic bacteria. In BV, the loss of hydrogen-producing lactobacilli combined with pH disturbances (>4.5) can lead to the overgrowth of anaerobes. BV features an increased production of amines by these anaerobes, which produce a fishy odour with a raised pH (i.e. more alkaline) vaginal environment, along with increased vaginal discharge(grey-white in colour) . AV corresponds to another type of disturbed microbiota with an abnormal pH (>4.5), in which the lactobacilli are replaced by aerobic facultative pathogens (i.e. intestinal microorganisms such as Escherichia coli, enterococci, staphylococcus spp. and group B streptococci). These cause inflammation, resulting in higher levels of thick discharge, with a yellow / green tinge, accompanied by a foul odour. CV is associated with imbalance of lactobacillus overgrowth. It typically presents with normal vaginal pH, and is associated with vaginal irritation and thick discharge. CV is often misdiagnosed as VVC. Candida spp. normally exist in small numbers within the vaginal microbiota, however when host environmental conditions permit, these commensal organisms can grow, resulting in episodic, or recurrent VVC symptoms. Various species of candida are associated with VVC, including C. albicans, C. glabrata, C. tropicalis, C parapslosis and C. krusei. VVC is characterised by a normal vaginal pH (4.04.5) and increased levels of white vaginal discharge with a yeasty or sweet odour. Trichomoniasis is caused is an anaerobic flagellated protozoon, Trichomonas vaginalis. This infection is a highly contagious sexually transmitted infection (STI), which causes profuse and unpleasant discharge and vaginal irritation. Symptoms of acute trichomoniasis often appear during or immediately after menstruation, however, its incubation period may range from 3 to 28 days. 14.01.2022 Testosterone is a male sex hormone, which belongs to the class of hormones known as androgens. In women, testosterone is primarily produced by the ovaries (in small amounts compared to what the testes produce in men). Other hormones that come from the ovaries and the adrenal glads such as DHEA and androstenedione can also make testosterone. The body can convert testosterone to oestradiol, the main sex hormone in females. 12.01.2022 Usually occurring between 45 and 60 years of age, with an average age of 51 years, menopause marks the end of the reproductive years of a woman's life. Menopause occurs as the number of oocytes (eggs) a woman carries reaches their end point with a marked decrease in ovarian oestrogen and progesterone. The transition into menopause, called peri-menopause, commonly lasts for 2-3 years, with common signs of irregular menses, skin changes, mood changes, depression, memory loss,... insomnia, fatigue, vaginal dryness and atrophy, changed libido, night sweats and hot flushes, which may be moderate to debilitating in their effects. During peri-menopause ovarian sex steroid synthesis slows and ovulation becomes unpredictable, causing fluctuating monthly patterns until menses ceases completely. Menopause is confirmed with the cessation of periods for 12 months with consistently elevated serum FSH levels and low serum oestradiol. The most common complaint with peri-menopause is hot flushes. The drop in oestrogen is a catalyst but not the only cause. Chronic stress is also a major contributing factor, as it decreases DHEA levels, resulting in the loss of thermoregulation. (we will delve into the signs and symptoms more next week) Factors that may speed up the timing of menopause and worsen the symptoms experienced include: > Premature ovarian failure > Hysterectomy retaining the ovaries (thought to advance menopause by approximately 2 years) > Bilateral oophorectomy > Smoking > Radiation and/or chemotherapy used to treat a malignancy > Hypothyroidism > Chronic or acute stress > Toxicity > Certain medications . . . .
