24.01.2022 ...continuing the single central theme for september are a few single tooth replacements with implants using the same immediate replacement protocol. I first heard this technique described by Joseph Kan in the mid 2000's. Thank you Szabi, once again!



22.01.2022 This type of case require patience and teamwork. It took a while to finish, the patient had a bridge supported by two lateral incisors that I had fabricated close to 30 years ago, one of the laterals fractured and when she re-presented both lateral incisors had been removed. I felt that to achieve a nice result the canines would need to be moved either distally or mesially, as you can see they were moved mesially and the orthodontist, Michael Whyte (orthodontist) also did a s...ensational job of improving the posterior plane of occlusion on the left hand side. The central incisors were replaced by one implant and two unit cantilevered bridge. A connective tissue graft was placed crestaly with the help of one of Inaki’s slim abutments. The canines and first pre-molars were veneered to look like the teeth who’s places they had been substituted into. Thank you both Michael and Szabi for your wonderful work. See more

19.01.2022 Achieving gingival symmetry in implant cases is very, very difficult. Achievable in a single tooth situation in the correct circumstances and under any circumstances always difficult to achieve when there are two or more missing teeth, this is one of the few occasions where it has really worked out well for me. Keys... thick biotype, starting from an ideal situation, Inaki's concepts regarding CT grafting and a great ceramist, thank you again Szabi. The patient had a root fracture in 12, my intention was to section her splinted crowns and place an implant in 12. When I sectioned the crowns the crown on 11 just fell off ...there was some caries and little tooth structure remaining and so the plan changed.

16.01.2022 It is always a challenge to match a single anterior tooth, ceramists have a little more practice than dentists and are generally much better at it, although, of course, we have some advantages working intra orally. Im not sure whether materials play such a big role, I think it is much more about what a particular individual is capable of seeing and what his/her hands will let them do. I got close this time I hope you enjoy the case.

15.01.2022 Over the last month or so we have delivered a few single teeth, I thought I'd post a couple this month. Above anything they reflect Szabi's extraordinary talent. Here is the first, a discoloured tooth and exposed margin. While the discolouration is not severe it was a grey hue, that always makes life difficult. The critical steps here are good shade photographs and a try- in.

15.01.2022 When I first saw this young man he wasn't quite a teenager. In the beginning treatment was about the strategic placement of permanent teeth so as to provide him with a sound long The term solution. On the RHS that meant placing the premolar in the canine position, creating two single tooth spaces in the longer term. On the LHS we turned a four tooth space into a three tooth space by removing one deciduous lateral incisor. The orthodontics was expertly carried out by Victor L...alieu. At about 25 yrs we began replacing his deciduous teeth with prosthetic replacements. A three tooth space is always a big challenge and achieving symmetry about the contra-lateral side wasn't easy. I was lucky the patient had good bony volume to begin with... It was there for me to loose. Many great teachers have influenced my practice of implant dentistry, none more than Inaki Gamborena and Joseph Kan. As usual, Szabi, thank you! 15.01.2022 I thought this would be a nice follow up to the previous case. I saw this patient for the first time in about ten years, about a week ago. There are a couple of interesting things about the case. Firstly, when I reviewed her records the first note ever in her history says that.... she thinks she would never be able to wear a splint... the first note! After 15 years she tells me she has never slept a night with out it, no doubt it is why the resins have performed so well in t...his type of case, with out the splint I would have expected them to be worn well down by now. Secondly, At the time I was really happy with the case although I was disappointed with the transition between resin and tooth structure, I prepared a bevel and the finish line is straight (once you see it you'll never unsee it) This case is why I always recommend wavy horizontal finish lines. 07.01.2022 Lateral incisors are small teeth, this sometimes complicates implant placement, additionally if an implant fails in this position ...the potential to loose bone from the proximal root surfaces of the neighbouring teeth exists. Is it better to avoid those risks completely? In younger patients particularly resin bonded bridges are (at least in my view) a great choice.

06.01.2022 ...continuing with the 'Central September' theme another case that we completed recently. The radiograph interfered with my original plan and so we decided on a simpler path. A resin bonded bridge in combination with a connective tissue graft.



05.01.2022 The replacement of some old resin veneers, an intra oral "wax-up" and a little tip for keeping the floss ligatures out of the way. I hope you like them.

05.01.2022 management of a single front tooth is always about shape and colour, In both instances there is a clear reference in the contralateral tooth...it's just a case of how close we can get, perfect is impossible.

04.01.2022 I guess the goals for this case were to improve incisal edge position and at the same time the form of the individual teeth. Abrasion and bruxism appeared to be the primary causes of the loss of tooth volume.
