Acne has an equal sex incidence and tends to affect woman earlier than men, although the peak age for clinical acne is 18 years in both sexes. The primary cause of acne is still unknown however, there can be no acne without sebum and this requires the sebaceous gland. Sebum forms a protective coating over the surface of the skin and preserves the skin barrier b...y preventing water evaporation from the skin surface to the environment. Acne results from: Increased sebum excretion seborrhea. This provides nutrition for bacteria growth Pilosebaceous duct hyperkeratosis and comedone formation sebum does not flow freely from the sebaceous gland to the skin surface Colonisation of the duct with Propionibacterium acnes (P.acnes) bacteria consume sebum as nutritional source for reproduction Release of inflammatory mediators (including cytokins) body responds to free fatty acids release by bacteria and inadvertently damages the skin causing scarring.
