23.01.2022 A great session - bringing research into practice Take away points: 1. High peak braking force can increase risk for running related injury in female recreational runners. ... 2. Runners can reduce peak braking force by increasing cadence by 5-10%. 3. Increasing cadence may improve asymmetries. 4. Changing foot strike pattern has limited evidence for benefit and is a high risk change to make. 5. Wearable technology, such as foot pods that measure forces placed on the body, are becoming cost effective to use clinically and for runners to use as a training/rehab tool. They are now valid and reliable in measuring meaningful metrics (not a wash of info that means nothing). See more

20.01.2022 Fitted these today for a gentleman with a 6month history of painful big toe joints. Hes most concerned about the pain limiting his ability to play golf He has symptomatic osteoarthritis of the big toe joint with a bony limitation in joint extension. Pain is easily replicated as the heel lifts during push-off phase of gait. Our goal is to keep him playing golf pain free + avoid/delay a joint fusion surgery. ... So how do these orthoses work? The extension of the carbon shell under the big toe is a modification to stress-shield the joint from excessive compressive load at the dorsal margin of the joint during push-off phase of gait. Basically, reducing load on the overloaded part of the joint. This is exactly how I prescribe orthoses for any painful condition - alter loads in target tissues, nothing to do with alignment. I plan to review his progress in 6 weeks!

14.01.2022 A quick (just 4 questions!) survey for those that have received foot orthoses for plantar heel pain. Your answers will be anonymous and you will be helping a colleague of mine who is doing some novel research into foot orthoses! Thanks and please tag/share with anyone that you think this may be relevant for.

14.01.2022 Running requires a certain level of joint range of motion. If youre restricted in one particular joint, your body will find a way to compensate - usually by increasing the range of motion at joints proximal or distal to the restricted joint. Today I saw a runner with a hip that internally rotated a whole lot more on one side during stance phase of running. One could say; oh look at that, you have poor lumbo-pelvic control and glutes thats arent firing when they should. Th...at may be the case, it often is. In this runners case, I think the extra hip motion on that side was a compensation for a lack of motion at a joint more distal to it - his ankle joint. He had ~25% less ankle joint dorsiflexion range of motion on the side with greater hip rotation and medial knee collapse. Will exercises to increase ankle joint range of motion improve hip control? I would argue, yes. Probably still need to do some proximal strength and motor control work but at least treating the ankle restriction may make this more effective and sustainable. Oh and all this is assuming he has an injury that is linked to the asymmetrical hip motion... which I think he does. Thoughts?

13.01.2022 Foot orthoses shouldnt be used to simply re-align the foot or lower limb, or just because someone has a flat foot. They are load modifiers. Not about putting a foot into its neutral position. Be wary of marketing that says orthoses/orthotics are needed to re-align you! Foot orthoses can be very effective at reducing loads to overloaded areas, I just dont like the over prescription of them via the alignment paradigm.

11.01.2022 Phillipa Ignacz and I had a great time at the Australasian College of Podiatric Surgeons cadaver workshop over the weekend. Its not often we get learning opportunities like this to enhance our skills in clinical diagnosis and management of foot & ankle injuries/conditions. I have a deep gratitude to those that donate their bodies for this purpose - truely invaluable.

09.01.2022 There has been a lot of chat about this in relation to the spine and how imaging relates (or doesnt) to lower back pain. Good to see a little info-graphic as it relates to the foot & ankle! We all know what a placebo is right? What a about a nocebo?! Nocebo: a detrimental effect on health produced by psychological or psychosomatic factors such as negative expectations of treatment or prognosis. Imaging can be a great diagnostic, prognostic and interventional tool BUT we ...need to be careful not to let it become a nocebo! An example at the foot & ankle is Achilles Tendinosis (degenerative tendon with weak bits) - they can look horrible on imaging and may promote fear in loading for some when in fact the weak bits are surrounded by strong bits of tendon ready and willing to take up the slack for the other parts of the tendon. In actual fact, controlled loading via rehab is vital to improved outcomes and potentially better tendon remodeling in the long term (although pain and function is the primary outcome, not if the tendon looks better on imaging). So, dont let imaging results scare you. Listen to your health practitioner surrounding what you should or shouldnt do in light of your imaging WITH a thorough clinical history (which in my opinion,is most important). Treat the (wo)man...not the scan.

02.01.2022 Hoka launched at the peak of the barefoot/minimal boom in 2009. It spawned the maximalist boom in the years to follow. In August 2017 Nike released the Vapourfly 4%- a complete overhaul of what marathon racing flats were up until that point in time. The marketing for them via the Nike Breaking2 project was classic Nike dropping big $$$ creating a groundswell and release hype not seen for a very long time. At around the same time Adidas released their Sub2 shoe which was a li...ghter version of the ever popular Adios model (4 of the previous marathon world records were broken in the Adios) - it was released with essentially little to no fanfare and retained the minimalism of a traditional racing flat with a low stack height midsole. So, whats next? So far Brooks, Sacouny and Sketchers are playing catch up with the maximalist road racing shoe trend. Not all are following suit. Time will tell if this is the new standard of road racing flat. It will be interesting to see how the other brands respond and if my gallery of VF4% catch up will expand. What do people think? Thick soled/High stack heights and carbon plates are the new standard or a fad?
