19.01.2022 Case #3 A small coronary artery fistula seen entering the MPA just above the pulmonary valve. Important to check that coronary arteries are not dilated. NB - If you detect abnormal flow firstly determine when the flow is occurring - systole, diastole or continual flow. Tip- coronary arteries fill in diastole.

15.01.2022 Reminder to all Echocardiographers wanting to improve their skills when assessing/excluding Congenital Heart Disease - the Annual Echo Ed "Congenital Heart Disease" workshop will be held on 4-5 April 2019 at the Wesley Hospital, Brisbane. 14.01.2022 Aortic root replacement. A high parasternal window can assist in visualising the root replacement and anastomosis site. Remember the long axis of the aorta is best visualised from a high PLAX view with clockwise rotation - keep on turning!!

14.01.2022 Case study - A 12 year old presents to your lab with a murmur for investigation. ASD, VSD, PDA, CoA and valvular abnormalities were all excluded. Note the colour acceleration seen in the LPA. The PV, MPA and branches are not usually well visualised with 2D imaging. Colour Doppler does not always give us the answer either (however, this case shows acceleration nicely) ... we must use SPECTRAL DOPPLER to exclude branch stenosis. The LPA is particularly challenging - high parasternal windows and SSN SAX images will assist you in visualising the LPA better. The CW Doppler signal shows two traces superimposed on each other - this is a clue to the presence of obstruction somewhere. It is important to interrogate the pulmonary branches when looking for the cause of a murmur.

13.01.2022 Same piece of plasticine = same mass. I think this little activity helps us understand Concentric LVH vs Eccentric LVH. Both have the same mass. It’s important to remember that if LV size is increased then you need more muscle to go around it - Eccentric LVH. If the walls appear thick with a small LV then this it is labelled Concentric LVH. Just remember that not all thick walls are LVH so we usually comment increased wall thickness on our echo report. 09.01.2022 Echo Ed Interesting cases - Case #1 LV non-compaction suspected from this off-axis Apical 4 chamber view. PSAX images showed apical trabeculations but did not show good endocardial definition to be suspicious of non-compaction. A traditional Apical 4 Chamber view was not concerning either. NB to interrogate trabeculations from off-axis imaging planes.

04.01.2022 Sub-aortic membrane in a 38year old patient. The apical 5 chamber is often the best view for detecting this lesion. This is a fixed obstruction and therefore the LVOT will look like an aortic stenosis signal, rather than a late peaking dynamic LVOTO signal. 03.01.2022 Case #2 Left circumflex artery (LCx) originating from RCA. As you scan from an Apical 4 Chamber view to an Apical 5 Chamber view the LCx can be seen travelling across in front of the aortic valve. Once you know what it is... you will see it more often. NB When scanning coronary arteries use the highest frequency transducer - poor images, great coronary arteries!
